The accumulation of many shipboard operations on the bridge of the ship demands that a high level of efficiency must be ensured. Discusses the efficiency of the interface which depends on the success of incorporating the human factor in the engineering systems of ship control. At the time of diminishing crew quality, the bridge operator's confidence, competence and communication capability must be in co-ordination with the technology provided by these systems. Suggests that the areas of limitation in the bridge-operator interface must be identified and the course of action for optimizing this critical relationship for safety must be determined.

    Notizen: In the USA, the Federal Emergency Management Agency (FEMA) provides support to State and local governments in fulfilment of their responsibilities for preparedness, response, recovery and mitigation of disasters. One method FEMA has used to support State and local emergency communication functions was to sign and implement a Memorandum of Understanding with the American Radio Relay League (ARRL) for amateur radio operators to provide electronic communications for State and local governments in disasters. The Federal Communications Commission (FCC) has licensed more than 600,000 amateur radio operators in the USA. The national organization of amateur radio operators called the American Radio Relay League (ARRL) was formed in 1914. More than 80,000 of these amateurs have registered their availability for emergency communications in disasters in the ARRL's Amateur Radio Emergency Service (ARES). Amateur radio operators have been providing communications in natural disasters such as floods, hurricanes and earthquakes since 1910. Since amateur radio operation was prohibited during the years of both World Wars I and II, FEMA has sponsored a new branch of the amateur service called Radio Amateur Civil Emergency Service (RACES). RACES operators are authorized to operate if the President invokes his War Emergency Powers while all other amateur operation would be silenced. Examines the role of amateur radio in providing emergency electronic communications for disaster management and explores future contributions.

    Notizen: Proposes the establishment of a management information system (MIS) for the promotion of safety in shipping. Considers the information technology in shipping to be the combination of satellite systems and computers onboard and ashore. In this combination, which enables the provision of an optimum selection and management of data for automatic or human decision making, the role of satcoms is information transfer and that of computers is information processing. Acknowledges the importance of the human factor in shipping safety. Human decisions affecting shipping safety often depend on the quantity and quality of the available information. In the proposed MIS the need to improve the limited ability of humans to receive, store, process and interpret information, which is usually vast and complex, is acknowledged and fulfilled. Among the available maritime satcom systems and computer services, the proposal reveals those services which satisfy the criteria of useful, interactive, flexible, fast, reliable and low-cost transfer and management of information in support of safety-oriented decision making in shipping operations. The proposed MIS integrates the information resources of shipping office(s), the ship(s) and the maritime-related industry as a whole and provides the shipping company with the capability to document, monitor and control the safety-critical aspect of "technical reliability" towards promotion of safety in shipping and compliance with the International Safety Management Code.

Suggests that this can best be effected at the level of middle range theory based on analysis of the comparative structure of a workplace organization. Offers an adaptation of an approach from anthropology and cultural theory, that supplies four distinct, exclusive and archetypal workplace structures. Each is associated with a distinct cluster of sustaining and justifying values and attitudes that are manifest as four associated patterns of behaviours. Particular attention is directed at the normal kinds of workplace deviance, including sabotage, shown to be typical of each of the four archetypes. Their incumbent attitudes to risk are delineated, as are their typical patterns of industrial relationships.

    Notizen: Explains that while some non-governmental organization (NGO) managers report that international staff are more difficult to manage than national staff, others report the opposite. Such different perceptions stem from different expectations of staff from different cultures. Describes a study in which NGO managers working in field programmes were surveyed on their perceptions of the differences between national and international staff. International staff were regarded by respondents as being: more likely to act on their own initiative; less likely to need close supervision; and more demanding of themselves and of others. Those reporting that international staff were more difficult to manage were more likely to have more than five years' work experience in developing countries. This may be explained by managers adapting to the culture in which they work. Such a cultural adaptation on the part of managers may in turn explain why relief workers often report that they are dissatisfied with their managers.

    Notizen: For many years there has been increasing concern about the effects of human error in complex system safety and reliability. This concern has been increased owing to accidents such as Chernobyl, Bhopal, Herald of Free Enterprise, Three Mile Island and the Kegworth air disaster. In the vast majority of these accidents, human error has played a critical role in the events precipitating the accident. Such accidents can in theory be predicted and prevented by risk assessment, in particular assessing the human contribution to risk. However, the collection of human-error data has proved a difficult field for the past 30 years, and yet industry would benefit from the existence of a robust human-error database. Provides therefore a brief historical résumé of past human-error databases, and discusses data collection and the inherent problems associated with data-collection schemes. Goes on to outline a human-error database currently being developed at Birmingham University, and presents information on regularly quantified human-error types. Finally, gives a general synopsis of the research and provides a selection of real human-error data points.

Suggests that, within a climate of sustained economic boom, policy makers as well as Malaysians from all walks of life are understandably less concerned about floods than they are about the financial gains that can be reaped from a booming economy. States that official solutions for flood control are largely engineering based and are ineffective to combat extensive monsoon floods.

    Notizen: Investigates what people are prepared to do in the way of disaster preparation, and examines how these assessments may be related to personal factors and attitudes. Draws on a theoretical model of the area, developed in a previous study using a qualitative grounded theory approach. Surveys 925 persons representative of the Swedish population between the ages of 16 and 74. Data were collected in a postal questionnaire. Shows that the preparations for disasters which had been carried out by the greatest number were installation of smoke detectors, participating in practice at school or work, and learning first aid. The least performed preparations included joining a voluntary organization or training programme, stocking up with tinned food at home, taking extra insurance, and learning how to deal with psychological crisis reactions. Finds considerable subgroup differences; and that the two key dimensions of the model - sense of personal meaningfulness and societal commitment - account for the differences. Suggests actions to be taken by the Swedish authorities.

    Notizen: Following civilian disasters such as Piper Alpha, Hillsborough and King's Cross, personnel professionals are rethinking the types of individuals needed to fill senior posts, considering the demands of responding to a serious emergency. Presents the results from a project designed to examine the current selection, training and assessment procedures for managers of offshore oil and gas installations in the North Sea, with particular emphasis on their ability to take command in the event of a serious offshore incident. Personnel and operations managers in 38 oil and gas exploration and production companies in the UK were interviewed. Characteristics sought in an offshore installation manager were leadership and command ability, communication skills, sound judgement, decisiveness and a stable disposition. Selection decisions were based predominantly on the candidate's previous performance, appraisal reports and managerial recommendations rather than more formal methods such as interview panels, assessment centres or psychometric tests. An industry-wide concern regarding managerial competence has led to increased use of emergency management simulations for training and assessment. The lack of formal assessments during the selection procedure is surprising, but the recent introduction of high fidelity, emergency management simulations present an additional source of valuable information on future candidates if assessment data are collected rigorously.

Describes a study in which such responses were cast against Quarantelli's minimum criteria and Levin's notion of experience as a powerful source for change.     Notizen: A reprint of a classic article chronicles the simulated incident of a plane crashing into the John Hancock Tower in Boston, Massachusetts. Recounts how the plan was developed and practised by the scenario team. Inclusion by the practice team of the local fire department and paramedics paralleled reality. The exercise demonstrated that the Tower, even with an emergency plan, needed stronger preparation. This was effective because on 11 August 1992, an actual emergency occurred in the facility and the building was prepared as a result of the simulation.

In 1997, an Australia-wide study of CI in manufacturing was initiated by a benchmarking survey examining the CI activities of medium to large manufacturing firms. Reports on selected findings of the survey and presents an analysis by industry sector of the firms' main motives for CI, content in the CI process, support for CI and problem-solving tools used to support CI. Initial findings indicate that there are no significant differences between manufacturing organizations in different Australian states. However, significant differences were found on comparing different industry sectors.

The readings obtained were used to calculate the percentage weight loss of pipe per metre and deduce the flowline sections that need to be replaced to avoid rupture of the steel pipe or fitting in service, using residual life estimates. The reduction of the wall thickness of the pipes/ fittings is caused by a combination of corrosion initiated by physical deformation anomalies and the presence of corrosive media along the flowline tracks (inside and outside the pipes). Offers new measures to help the company mitigate the various flowline problems.

Analyses the present status of these components in Swedish manufacturing firms through a survey of 284 respondents. Shows that fewer than half have written maintenance strategies or computerized maintenance information systems and several give maintenance low status. The figures are lowest in small firms and in the timber industry. Preventive maintenance at fixed intervals and corrective maintenance are the most common maintenance techniques. However, condition monitoring is common in large paper and chemical firms. Also indicates that Swedish firms have not fully made maintenance a company-wide issue, and that centralized maintenance departments dominate resources in large firms, but outsourcing has become important in small firms. Many of these figures are considered not to be optimal, but the average firm should be able to improve.

Although maintenance education in the UK is still in its infancy, the work undertaken at the MIRCE Centre of University of Exeter has demonstrated that education and industry can work closely together to develop programmes which meet the demands for highly skilled and trained maintenance engineers.

    Notizen: For high speed shear layers, variable density extensions of standard incompressible turbulence models have not proved to be adequate in explaining the experimentally observed reduction in growth rate with increase in the convective Mach number. Recently, Sarkar et al. suggested that, in addition to modelling the pressure dilatation, another dilatational correlation - the compressible dissipation - should be considered because of the enhanced dissipation known to be present in compressible turbulence. They have used the compressibility-corrected model - limited to the second power of the turbulent Mach number - with the SPARK code for the computation of high speed shear layers and have obtained satisfactory agreement with some of the available experimental data. This simple algebraic compressibility model has been modified to include a fourth order turbulent Mach number term. Comparison of the predictions with results of several analytical models and experimental work shows good agreement.

    Notizen: Direct and indirect time marching boundary element methods often become numerically unstable. Evidence of, and reasons for, these instabilities is provided in this paper. Two new time stepping schemes are presented, both of which are more stable than the existing standard schemes available. In particular, we introduce the Half-step scheme, which is more accurate and far more stable than existing methods. This scheme, which is demonstrated on a simple crack problem for the displacement discontinuity method, can also be introduced into the direct boundary element method. Implementation of the Half-step scheme into existing boundary element codes will allow researchers to attack more challenging problems than before.

    Notizen: In metal forming, there are problems with recurrent geometric characteristics and without explicitly prescribed boundary conditions. In such problems, so-called recurrent boundary conditions must be introduced. The present study deals with non-steady-state three-dimensional finite element analysis for helical extrusion of twisted clover and trocoidal gear sections through a curved die. A boundary-directed remeshing scheme based on the modular remeshing technique has been proposed to reduce the errors arising in mapping of variables between old and new mesh systems. The computed extrusion pressures in reaching the near steady-state loading stage are compared with the results of the experiments as well as the steady-state analysis. The three-dimensional deformed pattern involving warping at the extruded end due to torsional deformation mode is demonstrated. For twisted clover and trocoidal gear sections, the twisted angle of an extruded product is smaller than that of the die, and the simpler the shape of the sections, the larger the amount of the deviation.

    Notizen: In the inelastic stability analysis of plated structures, incremental-iterative finite element methods sometimes encounter prohibitive solution difficulties in the vicinity of sharp limit points, branch points and other regions of abrupt non-linearity. Presents an analysis system that attempts to trace the non-linear response associated with these types of problems at minor computational cost. Proposes a semi-heuristic method for automatic load incrementation, termed the adaptive arc-length procedure. This procedure is capable of detecting abrupt non-linearities and reducing the increment size prior to encountering iterative convergence difficulties. The adaptive arc-length method is also capable of increasing the increment size rapidly in regions of near linear response. This strategy, combined with consistent linearization to obtain the updated tangent stiffness matrix in all iterative steps, and with the use of a "minimum residual displacement" constraint on the iterations, is found to be effective in avoiding solution difficulties in many types of severe non-linear problems. However, additional procedures are necessary to negotiate branch points within the solution path, as well as to ameliorate convergence difficulties in certain situations. Presents a special algorithm, termed the bifurcation processor, which is effective for solving many of these types of problems. Discusses several example solutions to illustrate the performance of the resulting analysis system.

The computation for post-tensioned structures is organized in phases: the phase preceding prestressing (Phase I), the prestressing phase (Phase II) and the phase following prestressing (Phase III). The load is applied incrementally until failure. The model is tested on a number of examples.

The method uses an Arbitrary Lagrangian-Eulerian (ALE) kinematical description of the fluid domain, in which the grid points can be displaced independently of the fluid motion. In addition, it employs an iterative implicit formulation similar to that of the Implicit-continuous Eulerian (ICE) technique, making it applicable to flows at all speeds. A deforming mesh capability that can move the grid to conform continuously to the instantaneous shape of an aeroelastically deforming body without excessive distortion is also included in the algorithm. The unsteady aerodynamic loads are obtained using inviscid Euler equations. The model for the solid is general and can accommodate any spatial or modal representation of the structure. Determines the flutter stability of the system by studying the aeroelastic time response histories which are obtained by integration of the coupled equations of motion for both the fluid and the structure. Develops and demonstrates in 2D the formulation, which includes several corrections for better numerical stability. The cases studied include NACA64A006 and NACA0012 aerofoils and the EPFL Configuration 4 cascade. Finds the results from the numerical indicate good overall agreement with other published work and hence demonstrates the suitability of an ICED-ALE formulation for turbomachinery applications.

    Notizen: FEM analysis has been increasingly employed to simulate sheetmetal forming processes for industrial application purposes. From the simulation results, finite element analysts are able to predict the occurrences of splits and wrinkles therefore they can make recommendations of changes to the die design and/or to the part design to avoid possible stamping failures. The number of real die tryouts can be reduced, thus, the design cycle is shortened and manufacturing costs lowered. In the early times, application analysts were mostly concentrated on simulation of the stamping process itself starting from simple models, later running full size 3D models with large number of elements.

    Notizen: Achieves efficient structural optimization of plate structures while the design constraints are multiple frequency constraints. Reduces the computational cost of optimization by approximating the frequencies using the Rayleigh quotient. Uses an optimality criteria method to solve each of the approximate problems. The creation of a high quality approximation is the key to the efficiency of the method. Also, with the great number of design variables, the optimality criteria methods are robust approaches.     Notizen: Some frictional contact problems are characterized by significant variations in the location and size of the contact area occurring in the process of deformation. When this feature is combined with strongly non-linear, path-dependent material behaviour, difficulties with convergence of the typically used iterative processes can be encountered. Demonstrates this by analysis of press-fit connection, a typical problem in which both of those characteristics can be present. Offers an explanation as to the possible source of those difficulties. Suggests in support of this explanation, two simple modifications of the usual iterative schemes. In spite of their simplicity, they are found to be more robust than those usual schemes which are normally used in numerical analysis of similar problems.

In many cases, the system under investigation is linearized about an operating point using the linear part of the Taylor's series expansion. This allows local representation for use in incremental or iterative methods using well-established computational tools for linear algebra. Demonstrates a different linearization technique that generally provides a higher quality fit to a certain class of functions than the standard Taylor linearization. This class of functions is general enough to represent all systems of algebraic equations. Presents a graphical demonstration of the quality of fit along with a discussion of why this alternative linearization provides a high quality fit.     Notizen: Presents an interpolatory subdivision scheme to generate adaptively refined quadrilateral meshes which approximate a smooth surface of arbitrary topology. The described method differs significantly from classical mesh generation techniques based on spline surfaces or implicit representations since no explicit description of the limit surface is used. Instead, simple affine combinations are applied to compute new vertices if a face of the net is split. These rules are designed to guarantee asymptotic smoothness, i.e. the sequence of refined nets converges to a smooth limit surface. Subdivision techniques are useful mainly in applications where a given quadrilateral net is a coarse approximation of a surface and points on a refined grid have to be estimated. To evaluate the proposed approach, shows examples for FE-computations on surfaces generated by this algorithm.

Classes the profiles of orifice into two cases; and develops the orifice adaptive module for each case and then carries out numerical examples by using the orifice adaptive modules.

    Notizen: SCECO-EAST introduced total quality management (TQM) to achieve three main goals: increase customer satisfaction; reduce cycle time of the processes; and minimize total cost. Applying a TQM programme in service companies is very different from manufacturing in organizations since it involves subjective measures, while in manufacturing companies the end product is tangible and can easily be measured against preset standards. Explains in detail how TQM concepts can be applied to the process of "maintenance of medium voltage substations". First, the team was formed to break down the process and assign actual time involved in each activity which helps to calculate the existing cycle time of the process. Second, value added and non-value added activities were identified in order to eliminate/improve non-necessary non-value added activities. Third, the relationship value between customer needs and the work process was found by multiplying improvement ratio by the associated relationship weight to let the team focus on process activities which have the highest rating and biggest impact on customer satisfaction. Finally, the proposed recommendations/ improvements were evaluated and analysed. Concludes that applying TQM in a maintenance of S/S will reduce cycle time by 40 per cent which also results in a reduction of materials and manpower that correspond to more than one million Saudi Riyals per year.

    Notizen: Outlines the defects and faults during the design stage that affect building maintenance in Saudi Arabia and their relative degree of importance. Performed a survey of a randomly selected sample of 90 contractors, 30 architectural/engineering firms (A/Es), and 20 owners from the Eastern Province of Saudi Arabia. The survey included 35 defects and the respondents were asked to indicate their degree of importance. The defects were grouped into six groups. The level of importance of the defects and the groups were measured and ranked by their severity index for contractors, owners and A/Es. The following results were obtained: contractors, A/Es and owners generally agree on the ranking of the individual defects; contractors and A/Es agree on the ranking of the defect groups whereas contractors and owners, A/Es and owners do not agree; the construction drawings group of defects was ranked highly by all three parties whereas the architectural design group of defects received a low ranking.

    Notizen: Many systems in use today are not performing as intended, nor are they cost effective in terms of their operation and support. Manufacturing systems, in particular, often operate at less than full capacity, productivity is low, and the costs of producing products are high. In dealing with the aspect of cost, experience has indicated that a large percentage of the total cost of doing business is due to maintenance- related activities in the factory; i.e. the costs associated with maintenance labour and materials and the cost due to production losses. Further, these costs are likely to increase even more in the future with the added complexities of factory equipment through the introduction of new technologies, automation, the use of robots, and so on. In response to maintenance and support problems in the typical factory environment, the Japanese introduced the concept of total productive maintenance (TPM), an integrated life cycle approach to factory maintenance and support. TPM methods and techniques have been successfully implemented in Japan through the past decade, and more recently in the USA. Inherent within the TPM concept are the aspects of enhancing the overall effectiveness (efficiency) of factory equipment, and providing an optimal group organizational approach in the accomplishment of system maintenance activities. Both the equipment and the organizational sides of the spectrum need to be addressed in fulfilling the objectives of TPM. It is believed that while many successes have been realized in structuring organizations to respond better to the maintenance challenge, very little progress has been made relative to the influence of equipment design for minimal maintenance and support (i.e. the incorporation of reliability, maintainability, and supportability characteristics in design). Briefly addresses this aspect of the problem, identifies some design analysis/evaluation tools that can be used, and recommends an approach for the continuous improvement of manufacturing systems in terms of operation and support.
