    Notizen: The transport of urea and other biologically interesting solutes in aqueous solution across poly(2-hydroxyethyl methacrylate) (p-HEMA) hydrogel membranes has been studied. The results show that p-HEMA membranes have a high permeability to urea due to a strong interaction between the polymer and urea, and, in addition, they are consistent with a pore flow model in which solute transport takes place through water-filled regions essentially unaffected by the surrounding polymer matrix. The results indicate that such p-HEMA membranes might be suitable for hemodialysis applications.

    Notizen: This work represents a preliminary study of one case history of a silicone heart valve. The laboratory techniques employed have proven useful in characterizing the polymer before and after implantation. Before any model explaining ball variance can be proposed it must be determined whether the polymer loses its integrity because specific constituents in the body fluids interact with primary valence bonds in the polymer chains, or if the fluids simply cause swelling and plasticization of the crosslinked system and the damage is the result of mechanical work on the weaker polymer system. Detailed kinetic and thermodynamic studies are underway to answer these questions.The work summarized in this paper refers to the evaluation of only one specimen and should not be taken to mean that every silicone ball from a heart valve prosthesis will show this variance. The incidence of functionally variant silicone balls as reported to one manufacturer represents approximately 1.5% of the aortic and less than 0.05% of the mitral prostheses which have been supplied by this manufacturer. A small percent of valves of this type have the potential of becoming functionally variant and this is one failure mode which should be considered in the implantation of silicone ball valves.

    Notizen: Splat cooling has been used for the preparation of dental alloy in the form of ribbons of 5-25 μm in thickness. The powdered alloy was produced by shaking the ribbons with a dental pestle in a mechanical amalgamator. One minute shaking produced a powder all of which passed a standard 400 mesh sieve. Only phases expected at room temperature have been produced thus far.

    Notizen: Male mongrel dogs were divided into two groups and placed in urine collection cages for 24-hr urine samples. One group was implanted with sterile radiopaque polyethylene catheter and stainless steel guide wire fragments in the venous vasculature via an incision in the left femoral vein and the remaining group in the arterial vasculature via the left femoral artery. The catheter fragments, made radiopaque by an admixture of 25% lead oxide and 75% polyethylene, were located by roentgenograms and along with the guide wire fragments removed at necropsy 18-46 weeks following implantation. Gross and microscopic tissue lesions consisted of fibrous tissue tubes and slings which completely enclosed and anchored the fragments and intimal hyperplasia with fibrous tissue organization of thrombi, respectively. Tissue staining techniques for lead, catheter fragment weights, blood lead levels, and urine lead content all revealed no evidence of lead loss from the catheter fragment implants.

    Notizen: The rate of diffusion of pyrimethamine, a pyrimidine-based antimalarial drug, from silicone rubber is compared to the diffusion of this drug from a flexible epoxy. Specific amounts of drug were mixed into the epoxy resin prior to the addition of an aliphatic diamine curing agent in the reparation of the epoxy-drug capsules and into the silicone rubber prior to vulcanization with stannous octoate in the preparation of silicone drug capsules. These capsules, in the form of discs, were separately immersed in water which was replenished at regular time intervals. The amount of drug released was determined spectrophotometrically (λ max = 275 mμ). Silicone rubber discs with 20 mg of drug incorporated/disc showed a much higher initial release rate than epoxy discs with an equivalent amount of drug incorporated. The rate of drug release is shown to be a nonlinear function of the amount of drug initially incorporated. The amine functional groups on pyrimethamine, and the possibility of its participation in the curing of the epoxy is noted. In silicone rubber, such interaction is not expected. The addition of a silica filler to the epoxy drug mixture is associated with greater uniformity in the observed drug release rates of capsules made from the same batch of polymer drug mixture. The Shore A durometer hardness of these drug capsules are 30 and 70 for silicone rubber and epoxy, respectively.

    Notizen: Significant hazards to the use of femoral head prostheses-infection, corrosion, fatigue failure, interface motion, latent failure-dictate new standards for new prosthesis models. This study investigates ceramic-coated metal hip prostheses which have the theoretical advantage of tissue ingrowth. In long-term analysis, ingrowth did not occur in this study in the pygmy goat. Sintered metal prosteses were inserted in two dogs with bilateral congenital hip dysplasia. Excellent clinical results were obtained with radiologic evidence of bone envelopment of the sintered metal. Based on previous applications of silicone rubber to weight bearing by the author, the advantages of a silicone rubber prosthesis or silicone rubber-covered prosthesis are tissue compatibility, new bone formation, peak stress dispersal, and simulation of normal articular cartilage. A silicone rubber prosthesis was fabricated for the pygmy goat. It was inserted and initially has given good clinical results.

    Notizen: Since the accidental rediscovery of the endosteal implant by Formaggini in the late 1940's, the field has witnessed a tremendous evolution in theory and clinical application. Chercheve, in his efforts to improve on earlier spiral designs, proposed several theories on the relationship of the metallic endosteal implant to its osseous environment. Some of the problems he experienced still have not been overcome in our present designs.The development of the blade implant marked a true advancement in endosteal design. This implant proved to be much more functional and reliable than the spiral concept; however, it too had certain disadvantages which became obvious after a relatively short period of clinical trial. The anchor endosteal implant was developed as a result of efforts to correct the various problems associated with the blade.Preliminary animal research using both anchor and blade implants was begun. Four dogs had each type of implant inserted in edentulous areas. The animals were sacrificed four months post insertion for histologic examination. More sophisticated animal implant studies are being performed at the Brookdale Hospital Medical Center.Clinical evaluation of the anchor implant over a 3-year period has been very encouraging. Results have indicated a higher level of clinical success than previously achieved with blade or spiral designs.

    Notizen: The energy absorbed in the impact fracture of bovine metatarsal and metacarpal bone was shown to depend on the aspect from which the inpact specimen was obtained. In a majority of cases caudad specimens were more than an order of magnitude tougher than cephalad specimens and the fracture surfaces of the tough specimens were fibrous or woody in appearance due to osteon pull out. This pull out phenomenon was identified as a major factor in determining the toughness of bone and is analogous to the toughening effect produced by fiber pull out in synthetic composites. Whenever a substantial component of the applied stress was resolved across the osteons, the fracture cracks were seen to propagate in an interosteonal mode similar to the delamination of synthetic composites with weak interfaces. Plastic deformation of the matrix is the third mechanism commonly employed to effect toughening in synthetic composites. Although no similar phenomenon was demonstrated in this study, it is probable that this also makes some contribution to the work-to-fracture of bone.

    Notizen: The two systems for artificial hip joints in use at present are metal-on-metal and metal-on-plastic. There are many variables affecting friction and wear, which include diameter, sphericity, relative geometry of the components, and surface roughness. Tests in a simulating machine incorporated cyclic load and motion. Frictional torque of metal-on-plastic joints increased with diameter; Charnley 22 mm diameter producing lowest friction. For metal-on-metal the contact situation produced variable results, but friction was higher than for the plastic joints. Reciprocating friction machine tests suggested that high molecular weight polyethylene and AP4 polyester gave low friction and wear. Vitallium gave at least three times the friction of the plastic using γ-globulin solution as lubricant; but wear appeared to be much less.

    Notizen: Scanning electron micrographs were made of the surfaces of silicone rubber finger joint prostheses which had been subjected to 107 bending cycles at 37°C in bovine serum. Preferential attack at the regions of highest stress and the formation of microcracks of length 10μ and less were observed. By way of analogy with the corrosion fatigue phenomenon in metallic alloys, it is suggested that these may be initiation sites for the mechanical failures which have been observed, and that the acceleration of lipid absorption by tensile stress provides potential mechanisms for both initiation and accelerated propagation of fatigue cracks. It is demonstrated as a consequence of mechanochemical thermodynamics that any constituent of the body fluids (e.g. lipids) which can swell the elastomer will in fact be preferentially absorbed at the point of highest tensile stress, i.e., the crack tip, and that swelling will be accelerated by the applications of tensile stress.

    Notizen: Calcium aluminate bars were formulated to study the effect of real and simulated biological environments on strength. Static fatigue tests revealed that the strength of these potential internal bone prostheses decreased to ca. 60% of their initial strength after less than 100 min while being held at constant stress in solution. Calcium aluminate bars aged for 12 weeks in water or Ringer solution were also found to exhibit large losses in strength, while bars implanted in vivo for the same period of time weakened somewhat less. Semi-quantitative analysis of the solutions in which the calcium aluminate bars were aged revealed that Al3+ and Ca2+ were being leached from the bars in both the saline and water baths, while the concentration of Mg2+ was decreasing in the Ringer solution.Scanning electron microscope (SEM) studies revealed extensive microstructural changes in specimens aged in water or saline solution for 1 and 12 weeks. Similar but far less extensive changes were seen in bars aged in vivo in rabbits for 12 weeks.

    Notizen: New synthetic anionic membranes have been developed as replacements for the cellulose membranes currently used to remove blood toxins and water in hemodialyzers. The membranes are solvent-cast on to polyethylene sheets from copolymers of acrylic acid with various alkyl methacrylates. Membranes from copolymers of acrylic acid and n-butyl methacrylate show most promise as alternative hemodialysis membranes. The copolymers are prepared using modified emulsion copolymerization techniques with sequential monomeric additions in order to yield homogeneous polymers with the high molecular weight necessary for good film-forming characteristics. Permeability of the synthetic membranes to blood components, rates of water removal by ultrafiltration, and mechanical properties of membranes are measured and compared with standard 150 and 300PT cellulose films. Results for cellulose films confirm previous conclusions that permeability is principally proportional to the inverse molecular volume of the diffusing solute, while new anionic synthetic membranes show selectivity to basic blood solutes and operate by an “assisted diffusion” mechanism. A method is reported of improving mechanical properties while maintaining high permeability rates for blood solutes and high water ultrafiltration rates, by treating membranes with ethylene oxide gas, a clinical sterilizing agent.

    Notizen: The long-term biological tolerance of hydrophilic gels containing ionogenic groups was investigated on animals (rats and pigs). The basic polymeric skeletons of poly(glyco1 monomethacrylate) and poly (N-ethyl methacrylamide) were modified by introducing acidic (carboxylic) and basic (tert-amino) groups, or both groups at a time. Introduction of both groups at a time in the molar ratio yielded an electroneutral polymer of ionogenic character.In all groups investigated here the implant was encapsulated by a fine capsule of collagen fibrous tissue; no signs of proliferation were observed. No calcification took place.The second part of the work consisted in an investigation of the tolerance of heterogeneous poly(glyco1 monomethacrylate) gels containing ionogenic groups,basic, acidic, and ampholytic, with continuously changing porosity. The microporous polymers (50-60% water in the starting mixture) were healed-in as compact gels by encapsulation with collagen fibrous tissue. The macroporous polymers (70% of water and more in the starting mixture) were encapsulated with a capsule of collagen fibrous tissue, from which newly formed capillaries,serous exudate and numerous cells penetrated into the implant. The more porous the material, the broader the zone of cellulization. In microporous polymers there was no calcification; in macroporous, calcification occurred in the margin of the implant or in its center. In contrast with other authors' findings,no effect of the chemical composition within the concentration range investigated upon the way of healing-in or calcification could be observed; only the effect of physical factors was evident.

    Notizen: The configuration of epiphyseal lines in the growing mammal is extremely variable depending upon the principal stresses applied to the specific epiphysis in question. Because man is not highly specialized for rapid locomotion, the configuration of epiphysis in the human is not as well developed as that seen in animals specialized for rapid running, such as the artiodactyls. The several different typical configurations of the epiphyseal line are described and demonstrated in photographs using the deformed coordinate method. It is suggested that when coupling of porous structures, such as ceramics to bone, has been adequately worked out, configurations similar to those seen in the epiphyseal lines may be a successful method of attaching the prosthetic part to bone.

    Notizen: Problems encountered with existing total knee prostheses are briefly summarized. Existing prosthesis designs giving anatomically correct relative planar motion between femur and tibia are reviewed. A design incorporating a Rolamite suspension is proposed and evaluated. The design has the advantages of being closely anatomical in its motion having negligible friction or wear and requiring no lubrication. If substantial tendon damage is present the Rolamite design is easily modified to include a gear and rack.

    Notizen: Three types of dense alumina oxide ceramic (Al2O3) 99.7%, MgO 0.3%; density more than 3.9 g/cm3 differing only in their average grain size (5-15μ) and stainless steel (V4A) plates were implanted into the left femur of male Wistar rats 6 months old. Light and scanning electron microscopic techniques were used to investigate tissue reaction and the ceramic surface. Both ceramic and stainless steel plates were surrounded by a soft tissue layer that differed greatly depending on the implant type in respect to the fiber/cell ratio. While the body tended to enclose the metal plate with a small more or less avascular tissue mostly composed of fibers and some inactive fibrocytes, around the ceramic implants a highly cellulated soft tissue of mesenchyme character developed with signs of osteoblast and chondrocyte metaplasia. Adjacent to this mesenchyme a dense cuff of newly formed woven bone arose, showing numerous active osteocytes. Both metaplastic tendency of the newly formed messenchyme and osteocyte activity in the surrounding bone did not decrease remarkably towards the end of the experiment (70 weeks after operation), while the tissue around metal implants reached a steady relatively reactive state 12-24 weeks after operation. Intimate ceramic surface-mesenchyme contacts without foreign body reactions supported the idea that alumina oxide ceramic tends to be incorporated into the body's physiological environment while metals are “excluded” by fibrous tissue layers of low biological activity. Corrosion on the ceramic surface was not detectable.

    Notizen: An in vitro organ culture method was developed for the study of biodegradation of surgical implant materials. This method gives valuable information in less than 24 hr in the case of rapidly biodegradable materials such as poly(lactic acids) and poly(alkyl 2-cyanoacrylates). These data correlate well with the results of long-term in-vivo implantation studies. The organ culture technique can be used for the study of metabolism of polymers including the formation of volatile metabolites during biological degradation. Finally, histological and electron microscopic studies of the exposed organ slices can give indication of the formation of toxic metabolites during the biological degradation process.

    Notizen: Skeletal ingrowth to vitreous carbon was tested by means of bicortical corrugated vitreous carbon dowels of various groove sizes in dog femora and humeri. After a 4-week ingrowth period, resistance of the implants to push out loads ranged from 66 to 100 kg. Comparable data was obtained from similar sized polymethylmethacrylate and vitreous carbon-coated graphite implants. Histologic acceptability of the vitreous carbon appeared excellent in terms of minimal foreign body giant cell and fibrous tissue responses. Axial loading tests with femoral bone bridges demonstrated the vitreous carbon to be too brittle to withstand these functional loads. Experience with a canine skeletally fixed transcutaneous limb prosthetic device is presented.

    Notizen: We have developed a prosthetic replacement for the intervertebral disc. Although current management of a patient with degenerative disc disease is usually effective, certain inadequacies in permanent pain relief do exist. Most of the persistent symptoms seem to occur because of vertebral settling with subsequent posterior joint incongruities followed by arthritic changes. Theoretically, the replacement of a diseased intervertebral disc, prior to the onset of irrevocable changes, with a spacer would maintain anatomical congruity and physiologic function. This study is the preliminary report of the development of a silicone-Dacron prosthesis and its experimental trials in chimpanzees. Our early evaluation shows that the prosthesis allows motion, maintains the intervertebral space, and absorbs the compressive stresses placed on the spine. The artificial disc has a biomechanical behavior comparable to the normal chimp disc. Resorption of adjacent bone, reactive bone formation, and infection are complications that we have experienced. Continued evaluation is necessary.

    Notizen: Identification of an appropriate biomaterial which would allow an artificial tooth implant to function in a stable manner without eliciting a toxic response is one of the major challenges facing dentistry today, and has been the subject of an increasing number of investigations in recent years.Because of questions which still remain regarding the biocompatibility of candidate materials, extensive animal research in tooth implantation is an absolute necessity. Primates, providing dental models closely approximating the human condition, are especially valuable for undertaking that systematic scrutiny of experimental implant materials which is so clearly prerequisite to human clinical trials.In the current study, 26 jaw block sections, containing calcium aluminate root implantations, were surgically removed from 13 baboons. The implantation time periods ranged from 2 weeks to 10 months. The controlled porosity of the ceramic roots varied from 50 to 200 μ.Ingrowth of periodontal tissues into the porous ceramic root implants was demonstrable clinically, with light microscopy, and with the scanning electron microscope. Implant retention and stability, resulting from adequate ingrowth of fibrous/vascular elements into the ceramic pores, were optimal when the calcium aluminate root implants had a porosity of 100-150 μ. The problems of toxicity and biodegradability were not encountered using calcium aluminate.

    Notizen: The history of passage through the skin and of attachment to bone to treat fractures is briefly sketched since the work of Malgaigne in 1843. Attachment of metal to bone for many months is now routine after open reduction of fractures and is quite successful for many years for completely buried hip prostheses. Passage through the skin, the gums, or the cornea has been used in many specialties. At least half a dozen investigators have attempted skeletal attachment of artificial limbs.Even if absolute solutions are impossible, perhaps temporary successes might be extended by one or two orders of magnitude (adequate for a lifetime) by increased attention to engineering details. The role of ASTM Committee F-4, Surgical Implants, is noted. An important principle may be matching of stiffness, or at least tapering of the transition in stiffness, between implant and tissue. This precaution should reduce local stress concentrations tending to loosen attachment to bone or to reopen mechanical adherence of tissue to implant, opening a pathway for infection.

    Notizen: The possibility of evaluating the elastic moduli of the bone-porous biomaterial interface by treating it as a two-phase composite is discussed. Methods of placing bounds on the elastic moduli of such a composite are presented and applied to five materials currently being considered for use in porous prosthetic devices: 316 stainless steel, titanium, cobalt-chromium-molybdenum alloy (Vitallium), alumina (Al2O3), and high density polyethylene. The results show that when the prosthetic material becomes completely ingrown with bone, the elastic modulus of the composite becomes more compatible with bone. Titanium appears to be very favorable. Young's modulus for bulk titanium is about four times that of bone, while the fully ingrown composite has a Young's modulus approximately twice that of bone. Similar results obtain for the other elastic moduli. It is noted that the results for 316 SS apply to all surgical stainless steels, those for Co-Cr-Mo apply to other Co-Cr alloys and those for titanium apply to alloyed titanium.

    Notizen: An attempt has been made to define the conditions with respect to movement under which porous metal will bond to bone. It has already been shown that bone will grow into the pore of porous Vitallium under static conditions and under conditions of micromovement, as when stressed by the pull of a tendon. An experiment is described in which porous Vitallium staples were inserted across an unstable osteotomy site. No bonding occurred indicating that bone ingrowth will not occur in the presence of movement of this order of magnitude.The implication of this observation in the design of porous coated endoprostheses is that some form of initial mechanical fixation is required, and this fixation must remain rigid for four months to allow bone ingrowth.

    Notizen: Permeability data were obtained for commercial regenerated cellulose (Cuprophan PT-150) in isotonic saline at 37°C in a dual closed-loop dialysis cell. Eighteen radioactively labeled monodisperse solutes of widely differing chemical and physical properties, and ranging in molecular weight from 60 (urea) to 1355 (vitamin B12), were evaluated and the permeabilities of these solutes have been successfully correlated with various solute characteristics.Correlations were established between membrane mass transfer resistances (or effective membrane diffusivities) and:(1) characteristic molecular radii determined from both molal volume at normal boiling point (MVNBP) and hydrodynamic theory and (2) molecular diffusivities in saline. Membrane resistance data reported elsewhere for other cellulosic membranes were also found to correlate well with both MVNBP radii and liquid diffusivities.The correlations for Cuprophan have the following form: \documentclass{article}\pagestyle{empty}\begin{document}$$ \begin{array}{*{20}c} {\ln\; D_{{\rm eff}} = - 10.23 - 2.53\;\ln\; r_2 }&&{\left(1 \right)} \\ {\ln\; D_{{\rm eff}} = 5.06 + 1.64\;D_s }&&{\left(2 \right)} \\ \end{array} $$\end{document} where Deff = effective membrane diffusivity (cm2/sec), r2 = MVNBP radius (Å), and Ds is saline diffusivity (cm2/sec). Similar correlations were established for other membranes. It should be feasible to use these and similar correlations to predict membrane permeabilities (for a given membrane) from known solute characteristics without resorting to laboratory measurements.Membrane activation energies were also computed for eight solutes by measuring the variation in permeation rate through Cuprophan between 5 to 50°C. Unexpectedly, it was observed that the solute activation energies were not dominated by the activation energy of the membrane but revealed significant differences between solutes.

    Notizen: Some of the chemical and physical properties of three different hydrogels are analyzed as they relate to biological compatibility. The importance of the permeability and diffusion coefficients, porosity, and the possible role of quasiorganized water within the hydrogels are emphasized. It is suggested that the biological environment with its dissolved components such as ions, proteins, carbohydrates, lipids, and enzymes will influence the ultimate biological performance of hydrogels. This biological performance will depend not only on the hydrophilicity of the system but on numerous other parameters including the chemical composition, types and number of crosslinks, presence of functional groups, quasi-organized water structure, porosity, and the thermodynamic interaction parameters between the components of the biological environment and the gel. It is suggested that the presence of anionic gorups on certain synthetic hydrogel surfaces may be not essential for blood compatibility, provided that such materials are carefully distinguished from other hydrophilic gels.

    Notizen: Quantitative investigation of the rate of diffusion (in vitro) of chloroquine diphosphate, an antimalarial drug, from silicone rubber (Silastic) discs is reported here. The viscosity of the polymer is reduced to facilitate the formation of a more homogeneous drug-polymer mixture from which the discs are made. The average quantity of drug released daily from these vulcanized silicone rubber discs is measured as a function of the amount of drug initially incorporated in the polymeric mass. Discs with 9 and 36 mg of chloroquine diphosphate incorporated continue to release drug in microgram quantities for a period of over four months. The presence of low viscosity silicone fluid (20 centistokes) in the discs was found to cause no significant alteration in the drug release rate. The effect of agitation of the fluid in which the discs are immersed is also investigated. Results from samples prepared from different batches of polymer showed comparable results, indicating the rather homogeneous nature of the polymer-drug system. All samples showed high release rates during the first few days followed by much lower and more or less constant drug release on subsequent days over a period of months.Concentrations of the drug in the eluant is determined spectrophotometrically by utilizing the 220- and 343-mμ peaks of chloroquine diphosphate as well as spectrofluorometrically by measuring the percent fluorescence at 405 mμ (360-mμ excitation).

    Notizen: The mechanical strengths of four types of ingrown porous carbon and two types of porous metal bone implants were correlated with the type of osseous ingrowth into the porous interfaces. Intracortical cylinders were implanted into canine long bones and examined at 3, 6, and 12 weeks. Histological techniques were used to study bone ingrowth. Tetracycline labelling and electron microprobe analysis were used to characterize the degree of mineralization of the ingrowth. Shear strengths of bone-porous material interfaces were measured on cylinders forced out in compression. Histology revealed readily identifiable bone in most porous specimens. Mineralization was found to various depths in all porous specimens. After ingrowth, the porous metals, possessing appreciable inherent strength, displayed interfacial shear strengths from one-third to two-thirds that of cortical bone. The very porous carbon lattices, possessing little inherent strength, developed interfacial strength after ingrowth approaching that of the ingrowing cortical bone. The less open, more rigid porous carbon implants developed smaller interfacial strengths, averaging between one-eighth and one-half that of the ingrowing cortical bone.

    Notizen: The fracture surfaces of specimens of a cobalt-chromium alloy fractured in a conventional testing machine have been compared with similar specimens fractured by fatigue. Notched bar impact tests were also carried out and the surfaces of the specimens compared to those fractured in tension and by fatigue. Results show a brittle failure in all cases, although some change in the rate of crack propagation was evident in the fatigue specimens. Clinical evidence suggests that failure of partial dentures is due to bad castings, but some evidence is produced to show that fatigue cannot be eliminated as a cause of failure.

    Notizen: The behavior of aqueous solutions of poly [N-(2-hydroxypropyl) methacrylamide], poly(N-ethyl methacrylamide), poly(N-ethyl acrylamide), and poly-(triethyleneglycol monomethacrylate) was investigated and characterized in tests required for synthetic infusion solutions. The above solutions were characterized from the viewpoint of toxicity, pyrogenic properties, antigenic properties, effect on the determination of the blood groups, and effects on hemocoagulation. The elimination from blood circulation was studied on several fractions of the 14C-labelled poly[N-(2-hydroxypropyl) methacrylamide] having different molecular weights. It was found that in all tests the properties of the polymers investigated do not depart from the expected behavior.

    Notizen: An investigation was undertaken to correlate mechanical strengths of various bone-porous material interfaces with the type and degree of osseous ingrowth. Intracortical cylinders of porous and nonporous metals, ceramics, polymers, and carbon were implanted into canine long bones and examined at 3, 6, and 12 weeks. Reflected and transmitted light microscopy were used to study bone ingrowth. Electron microprobe analysis and tetracycline labelling were used to characterize the degree of mineralization of the ingrowth. Shear strengths of bone-porous material interfaces were measured on cylinders forced out in compression. Microhardness traverses were made across the interfaces. Readily identifiable bone was shown by histology in many porous specimens. Mineralization was found to various depths in all the porous specimens. The porous metals displayed the greatest interfacial shear strengths and hardnesses, with values from one- to two-thirds that of cortical bone. Changes occurred in the ingrowth region, a bone-metal composite, which suggest a reduction of elastic mismatch.

    Notizen: The objective of this research is to achieve direct chemical bonding of structurally strong implant materials with hard and soft tissues. This objective has been achieved through the development of a series of surface-active bioglasses and bioglass-ceramics. A controlled release of Ca, P, and Na ions from the surface of the materials produces an alkaline pH and nutrient constituents at a time when the body can incorporate the ions into newly formed tissues. Optical and electron microscopy of in-vivo implants in rat femurs and muscles show the development of stable chemically bonded physiological interfaces after 6 weeks. Microstructure and crystallinity of the materials do not influence the bonding achieved. Mechanical measurements of the bond strength in bone show that the bone fractures before the interface does. In-vitro studies of protein epitaxy, hydroxyapatite crystallization, and solubility kinetics correlate with the proposed interfacial bonding theory.

    Notizen: The mastoid process of the temporal bone can become infected with possible serious complications. If the infection does not respond to antibiotics, the infected area must be surgically removed, with the result that a cavity is formed. This cavity must be obliterated before the soft tissues can be closed over the site.Many biological tissues have been used in attempts to obliterate the mastoid cavity. Fat grafts and diced cartilage have shown less than optimum success because of their tendency to suffer various degrees of resorption. The most popular obliterative technique has been the musculoplasty in which a flap is cut from the temporalis muscle and rotated into the cavity. This method shows a great deal of success, but is a difficult procedure to do since the flap must be provided with adequate vascularity and must be cut to exactly the correct length. Hence this procedure works best for otologic surgeons who have had a great deal of experience in this method.Various forms of ceramics (either impervious spheres or porous pieces) have been used to obliterate the surgically produced cavity in the feline tympanic bulla. This bulla is a common model for the human mastoid. Since the ceramics must only fill the cavity and no stress is involved in this application, the brittle nature of ceramics is not a factor in their usefulness.The ceramics are held in place well by the ingrowing soft tissue and hence obliterate the cavity. Porous pieces of alumina seem to be best from the point of view of rapid tissue ingrowth holding them in place faster than the impervious spheres which require tissue to grow around them.The use of bioceramics to obliterate mastoid cavities would lead to an obliterative procedure in which an otologic surgeon would only have to excavate the infected cavity, fill it with ceramics, and close the site. He would not have to be involved with the tricky business of tailoring muscle flaps or obtaining homologous or autologous tissue.

    Notizen: Our 4-year clinical experience with reconstruction of worn metacarpo-phalangeal and proximal interphalangeal joints with silicone-Dacron prostheses in 121 patients is presented.Emphasis is on preoperative assessment and proper selection of patients.Three hundred sixty-nine metacarpophalangeal joint prostheses and 27 proximal interphalangeal joint prostheses have been inserted. Most of the patients had rheumatoid arthritis; several had degenerative arthroses, and a few had traumatic arthrosis.Particular attention is directed to minor modification of surgical techniques and emphasis is on postoperative management. Suggestions for implant redesigning and modification are made.The silicone-Dacron arthroplasties have been successful in decreasing pain, improving stability, increasing hand function, and in providing an adequate range of motion.Complications have included five infections, two recognized fractured prostheses, and two instances of early dislocation of a prosthesis. The patients have consistently been relieved of pain: on the basis of this short-term experience, we believe that the implant principle in the hand is successful, but improvements in design and material are needed.

    Notizen: The central theme of our studies is concerned with finding a better means of tooth replacement. This goal is being realized for a “tailor-made” polymer and associated techniques of usage have been developed for this purpose. In the course of doing this, we have introduced the polymer implant concept by which materials can be tailor made to serve different functions.The polymer composition currently considered best for these studies contains clear polymethacrylate powder and liquid monomer, blowing agent (dinitrosopentamethylene tetramine), anorganic bone, and N-tributyl phosphate. This mixture is heat processed in a plaster mold at 300° F for 30 min. The processed implant is sandblasted to remove its outer skin and to open the pores on the implant's surface to the host's tissues when implanted. This formulary has resulted in the totally self-supporting replica implant becoming a reality in baboons. Two Papio-Anubis baboons', both with 8 incisors (4 upper and 4 lower) were replaced with polymer replicas, and after 3 months the splints were severed and the 16 implants remained totally self-supporting. Metals have been coated with this polymer formula, and polymer coated pins have been used successfully as supports for fixed prostheses and as endodontic stabilizers. The internal metal piece imparts increased strength without bulk, while the polymer coating allows for attachment to the implant. The untoward effects of metals, such as heat transfer, electrical and corrosive phenomena are diminished or eliminated. This “happy union” of the two materials allows the implant to be strong and well integrated into the host. The Polymer Implant Concept holds a genuine hope for the future. In this instance it provides for a better means of tooth replacement: one which is cosmetically and functionally excellent. Most probably it will have an important place in the neurosurgical, orthopedic, and medical fields as well.

    Notizen: The carbonization of certain polymers under carefully controlled conditions yields glassy carbon bodies which may have potential application in the dental profession. Considerable evidence is now available for the good compatibility of a variety of carbonaceous materials with tissue and bone. Many carbons remain relatively inert in the human environment compared to metals and polymers. Disordered carbons with strengths exceeding that of graphite are commercially available with elastic modulus in the same range as that reported for bone(∼3 × 106 psi). The object of this study was to determine mechanical properties and characterize the structure of a new porous glassy carbon material.Compressive and tensile strength data were obtained for carbon samples with a wide range of pore structures. A scanning electron microscope and a mercury porosimeter were used to observe variations in pore structures. The porous materials tested exhibited compressive strength values up to 48,000 psi and ultimate tensile strength values up to 7,500 psi. Sonic modulus values from 1 to 4.6 × 106 psi have been measured. Some carbon samples had interconnected porosity exceeding 50% and median pore diameters greater than 50 μ. Material with an interconnected mean pore size as small as 46 Å has also been produced. Preliminary animal implant studies are in progress.

    Notizen: Properties of injection-molded high-density polyethylene reinforced with carbon fibers were investgiated. With 20% by weight carbon fibers tensile strength of polyethylene was almost doubled. Further increase of volume fraction of fibers did not produce as dramatic results. With 40% by weight carbon fibers, elastic modulus increased one order of magnitude: from 0.225 × 106 psi to 2.48 × 106 psi. This value is close to the value of the elastic modulus of bone, hence the material may substitute for bone where the same stiffness is required. The total strain at fracture was reduced two orders of magnitude, but still stayed comparable to total strain at in bone failures (1-2%).No appreciable changes were noted in shear strength, impact strength and wear resistance. The work to fracture increased from 0.9 to 2.9 kg cm/cm2 with 40% of carbon fibers.The most significant improvement of polyethylene with addition of carbon fibers was demonstrated in measurements of viscoelastic properties of the material. Creep characteristics were dramatically improved.In summary, it can be stated that injection-molded reinforced polyethylene has much superior properties to the nonreinforced P.E. which can be further improved if better adhesion between P.E. matrix and reinforcing fibers can be secured.

    Notizen: This study was undertaken to find a means for a long lasting and uniform release of the male hormone, testosterone. The rate of release of testosterone was determined over periods up to 4 months. The factors which affect the rate of testosterone release were studied and a mechanism for the release of testosterone was proposed.The first means providing a constant release of testosterone was the use of silicone rubber membrane around testosterone cylinders. It was found that a constant rate of release was reached after an initial unsteady state period ranging from 1 to 2 weeks.It was theorized that a water layer had formed between the membrane and testosterone cylinder. This layer caused the concentration inside the capsule to be that of saturated water.Several testosterone cylinders were coated with especially prepared membranes. These membranes were made by dissolving testosterone and silicone rubber in a mixture of tetrahydrofurane and toluene. It was hoped that an amount of testosterone might be incorporated into the silicone rubber matrix. The samples were run and the permeabilities determined. The permeability showed little or no increase over the range of 10-30% testosterone.Next, a membrane was prepared by mechanically mixing testosterone and silicone rubber. The amount of testosterone ranged between 5 and 200%, by weight. A definite increase in the permeability was observed with an increase in the percentage of testosterone.The second method of obtaining a uniform rate of release of testosterone was by mechanically mixing testosterone and silicone rubber and molding a sheet. After an initial unsteady state a constant rate of release was obtained and such rate of release increased with higher percentages of testosterone in the sheets.

    Notizen: An investigation has been conducted to observe the effect of heat treatment on mercury loss and structural transitions in clinically used amalgams, as well as the individual phases composing dental amalgam. Pure Ag2Hg3 readily loses mercury and transforms to the β1 phase while the remaining amalgams are somewhat more stable. Since essentially no weight loss is observed in clinically used alloys, the mercury, liberated by the γ1 decomposition, reacts with excess Ag3Sn to form a self-perpetuating reaction, explaining the increases in the β1 and γ2 phases in the Ag3Sn and “True Dentalloy” amalgams. That is, the mercury has redistributed itself within the amalgam.

    Notizen: To serve as a component of a load-bearing skeletal prosthesis, a porous material should offer the following attributes: (1) pore channels over 100μ in diameter; (2) continuity of pore channels; (3) matrix material inert and compatible with animal tissues; (4) manufacturable as thick coatings or thin-walled complex surface assemblies; (5) manufacturable to precision dimensions; (6) large compliance comparable to bone; (7) crack resistance, particularly under impact.A porous composite was manufactured by molding and sintering short metal fibers, which fulfilled all of these requirements. The composite was manufactured both with titanium and Vitallium fibers.Experimental implants were performed in dogs and rabbits indicating peripheral bone formation around the implant at 2 weeks and penetration from 3 weeks on. Shear strength values at the prosthesis-bone interface were in the range of 20 kg/cm2 3 weeks after implantation.Femoral prostheses implanted in dogs revealed fixation by bone formation and maintenance by bone ingrowth up to 1 yr following implantation.Massive incorporation with bone of skeletal segmental replacements for the baboon's femur were seen up to 3 months following implantation.Practical problems that can be encountered with the use of porous fiber metal composites for prosthesis fixation relate to: (1) the possibility of long-term corrosion occurring in an implant with a very large surface area; (2) technical problems with implantation requiring intimate contact between the fiber metal surface and the bone and (3) the difficulties in removing an implant which has failed for mechanical reasons requiring that a device manufactured using fiber metal composites for fixation be absolutely failure free from a mechanical viewpoint.

    Notizen: Surgical repair of injuries to the flexor tendons of the hand does not give satisfactory clinical results. In an attempt to solve this clinical problem, basic studies have been done aimed at developing an artificial tendon. In this paper, the anatomy of the flexor tendons and the physiology of their wound healing is related to the clinical problems encountered in this type surgery. It is proposed that a satisfactory way to solve this clinical problem is development of a permanently implantable functional flexor tendon prosthesis. Dacron polyester fabric of four various weave configurations were implanted into the musculotendinous junction of rabbits and into the femurs of dogs for periods varying from 2 to 8 weeks. The strength of the ingrowth into these fabrics was mechanically measured and related to surface area and porosity of the implant. Histological studies of the ingrowth was made. It would appear, at the musculotendinous junction, the strength of the ingrowth into the implant is primarily dependent on the internal fabric porosity and not the surface morphology. The strength of the ingrowth from the bone into the implant exceeded the strength of the materials in all specimens.

    Notizen: Osseous reconstruction of the surgically resected mandible has presented, for some time, a considerable surgical problem. This paper presents the results of a series of experimental investigations evaluating the various morphologic and immunologic types of autogenous bone grafts in the reconstruction of the surgically resected mandible. The osteogenic inductive effect of hemopoietic marrow in combination with autogenous cancellous bone is reviewed and examples of this inductive effect are cited. The application of the particulate graft of cancellous bone and marrow to the restoration of large areas of lost mandibular bone is described. In this application, the particulate graft is used with a compatible metal implant designed to offer an optimal grafting system for the reconstruction of large areas of the mandible. The metallic implant, in addition to containing the bone graft, serves to control and fix the host bone fragments and lessens the period of intermaxillary immobilization necessary for healing. Normal aesthetic mandibular contour is restored and routine dental prosthesis may be constructed over the reconstructed mandible. It is felt that this system offers the optimal approach, through the combination of implant metals and highly osteogenic graft tissues, to the reconstruction of the mandible following surgery.

    Notizen: Results of a preliminary study of stress in bone around dental implants are presented. Stress is calculated using the method of finite elements. Two blade type and one pin type implant are subjected to horizontal and to vertical forces of 19 lb. Differences in blade implant design do not alter the stress distribution significantly. The pin implant shows that most of the load is carried near the entrance into the bone, with a rapid fall in stress.

    Notizen: A study of metallic implants consecutively removed from 133 patients at the Hospital for Special Surgery during a 2-year period was conducted to evaluate their performance. They had been implanted for 2 weeks to over fifteen years. The implants studied consisted of prostheses and internal fixation devices used for open reduction of fractures or immobilization of osteotomies. The implants had been fabricated from either stainless steel or cobalt-chromium alloys.Case histories and roentgenograms were reviewed. A detailed inspection of each implant was performed including failure analysis when required. Crevice or fretting corrosion was noted on all but one stainless steel implant of the plate and screw type, and to a lesser extent with the cobalt-chromium alloy implants. Extreme wear was noted on contact areas of articulating prosthetic components made of cobalt chromium alloy. Complete mechanical failures of internal fixation devices were attributable to fatigue, design deficiencies or misuse of the implant. These findings are discussed in light of the clinical results.

    Notizen: A group of 12 materials was evaluated for tissue compatibility via a sequential histopathologic evaluation of the rabit muscle response to the implanted materials. Tissue samples were obtained daily for 7 days and weekly for 12 weeks to follow the time course of the response. Some cytotoxic samples revealed a rapid and marked response of relatively short duration, whereas others tended to produce a sustained or a long-term, intermittent inflammatory and/or necrotic response. These effects also demonstrated the suitability of 7 days as a satisfactory time period for routine evaluation of rabbit muscle reaction to implanted materials. The histopathologic evaluation of the response was compared with a gross evaluation of the implant site and an independent test of the material by tissue culture (agar-overlay method).

    Notizen: The local reaction to polyurethane (Thurane foam-rigid type) varies significantly in the subcutaneous tissue of the mouse, rat, and rabbit. Polymorphonuclear leucocytes concentrated at the periphery of the particles of polyurethane in the mouse in conspicuous numbers for as long as 550 days. In the rat leucocytes were infrequent and in the rabbit they were absent. The mechanism by which this variation occurs is unknown. It is suggested that it may be attributed to an electrical phenomena with variations in different hosts. There was an accumulation of pigment in the local inflammatory reaction apparently resulting from a degeneration of the polyurethane as well as degeneration of erythrocytes. The amount of this pigment varied significantly in the mouse, rat, and rabbit. Proliferation of large fibroblast-like cells, apparently originating from adjacent striated muscle, occurred in the mouse but was absent in the rat and rabbit, another significant, variation in the reaction of the mouse to polyurethane when compared to that in the rat and rabbit.

    Notizen: The biological tolerance of poly(N-substituted acrylamides) implanted subcutaneously to rats and pigs was investigated. Besides a macro- and microscopical evaluation of the implant and its surroundings, the surface changes of the implants were determined by means of a Stereoscan electron-optical microscope;the results obtained confirmed the very good in vivo stability of the polymers used.The histological analysis showed that all the polymers under investigation are very well tolerated by the organism and do not provoke any unfavorable reaction. The implants become surrounded with a granulation tissue which is gradually replaced by fibrocytes and collagen fibers situated in the basic substance staining positively with the alcian blue and PAS. In contrast with some authors we did not find a thicker capsule surrounding the implant on the sharp edges but on the flat planes. No calcification could be observed within the intervals investigated. The surface changes, which were studied electronoptically, were insignificant and connected with the way in which the implant was treated. No indications of malignant growth were observed.

    Notizen: A combined histological, metallurgical, and clinical study has been made in 19 instances where a surgical operation had been performed to remove a titanium implant used in the treatment of disease or trauma in man. All the implants were static devices such as pins, plates and rods. Histological examination of nonosseous tissue excised from adjacent to the implant was supplemented by estimation of its titanium content by neutron activation analysis.The results indicate that in some patients titanium can be liberated into tissue adjacent to titanium implants. The phenomenon can occur in the absence of corrosion or fracture of the implant, and is unrelated to the possible liberation of wear particles from an artificial articular surface. There is variation from individual to individual in the amount, if any, of titanium released. In the case of those implants showing titanium release, the results suggest that local release of the metal, or its subsequent accumulation in the tissue, is a patchy process at the implant site. The histological features of the local reaction of nonosseous tissue to titanium release are described. No conclusions have been drawn concerning possible effects on the osteogenic response to the insertion of the implants and on the overall state of the bone at the time of their removal. From study of the clinical records it is apparent that considerable caution is needed before concluding that release of titanium can cause clinically significant local effects. Although such effects may have occurred in a minority of patients, this possibility is by no means proven. A distinction must be drawn between a “metal reaction” in the histological sense and a “metal reaction” in the clinical sense.

    Notizen: Bioelectric charge transfer between blood and the vascular wall appears to play a significant role in maintaining vascular homeostasis. Since the surface charge characteristics of the vessel wall are a function of the dielectric properties of the material, a study was made of the dielectric characteristics of various ion permeable materials and their relationship to electrokinetic phenomena and thrombogenicity. The dielectric properties of the ion permeable materials and of animal tissue were determined and found to be strikingly similar. Several models of electrodiffusion phenomena are reviewed with respect to dielectric properties. The electro-osmotic characteristics of animal tissue and ion permeable materials were investigated, and it was found that animal tissue exhibited electro-osmotic characteristics to a greater degree than did any other man-made materials. The propensity of a material to electro-osmose could not be predicted from the dielectric properties. In addition, no correlation could be found between thrombogenicity and dielectric properties. Dielectric relaxation phenomena is reviewed, and its possible contribution to nonthrombogenicity through the generation of a uniform, constant wall surface charge is presented. It is concluded that to mimic the ion content and mobility characteristics of animal tissue, as determined by dielectric properties, is insufficient to preclude thrombus formation.

    Notizen: The apparent biocompatibility of many synthetic and natural aqueous gel materials has encouraged their study and testing for a wide variety of biomedical device applications. Many of the physical and in particular the interfacial properties of such gels are highly dependent on the organization of water within and on the surface of the hydrogel. Water is an important component of such gels, varying from about 30 to nearly 100 wt-%, yet the role of water in the gels has been virtually ignored. This paper briefly reviews the nature of water structure in pure bulk water, in solutions, and at interfaces. Polywater or anomalous water is also briefly reviewed. Evidence is presented that the water in many hydrogel systems can exist in at least three different, structurally distinct forms. A hypothesis is presented which can be used to evaluate and study the nature of water in bulk hydrogels. Consideration is also given to the role of organized water at the hydrogel surface on the interfacial properties of such systems.

    Notizen: A systematic series of chemically well defined, segmented polyurethane-polyether copolymers and their mixtures are shown to develop predictable and controlled morphologies. Changes in compositions and casting histories allow the preparation of polymer films with various types of morphological structures including uniform nonporous, coalesced spherulitic nonporous, spherulitic porous, and “Swiss cheese-like” structures with uniform (but alterable) pore sizes.The desirability of producing biocompatible materials with controlled morphologies provides incentive for further investigation of these and similar urethane copolymers.

    Notizen: The purpose of this investigation is to clarify the role of the component phases of dental amalgam in a corrosive environment and to analyze the corrosion products at various stages of formation predominantly by means of x-ray diffraction methods and energy dispersive x-ray spectroscopy.     Notizen: The aging reaction of a 90 wt-% gold, 10 wt-% platinum alloy with nominal iron concentrations of 0.04, 0.08, and 0.16 wt-% iron has been studied using electrical resistance measurements. The electrical resistance was measured as a function of temperature from 300 to 750°C. A decrease in electrical resistance was observed occurring at 350 to 450°C, when heating the solution treated and water quenched specimens containing iron. Subsequent heating and cooling curves follow the gold-platinum standard. Isothermal electrical resistance measurements were made as a function of time. The data from the specimen containing a nominal iron concentration of 0.16 wt-% iron was analyzed using the Johnson-Mehl equation. The shape parameter n was found to vary as a function of aging temperature from n = 0.6 to n = 1. These results have been rationalized in terms of a model involving precipitation of FePt3 at dislocations (400°C) and FePt3 in the shape of a rod at 500°C.

    Notizen: This experiment describes resorption rate, light and electron microscopic appearance of the implant site, and tissue distribution and route of elimination of radioactivity from carbon-labelled polylactic acid implants. Fifty-six rats received abdominal implants and were serially sacrificed at 28, 56, 84, 112, 140, 154, and 168 days. Urine and fecal output was measured for radioactivity, along with implant site and several tissues at the time of sacrifice. Tissue distribution of activity was negligible. Resorption rate of the implant was linear with 63.2% remaining after 168 days. Elimination of radioactivity was via respiration, approximately 29.4% by this route. Histologically, the implant was invaded rapidly along septal defects or slowly on broad fronts. Invading cells were characterized either by large double-walled vacuoles or by large accumulations of mitochondria and microvesicles adjacent to the implant surface. The long duration of this type of implant may be an important consideration in surgical applications.

    Notizen: Topographical changes which occur during the degradation of suture materials were studied by scanning electron microscopy. Potentially absorbable sutures were compared with those known to be absorbable. Loops of plain catgut, chromic catgut, polylactic acid, polyglycolic acid, polyester “A” and polyester “B” were implanted subcutaneously in rats for periods ranging from 2 to 14 weeks. They were removed and examined without further treatment. Specific and localized structural defects were observed to develop in the implant materials. In some cases the time required for development of these defects indicated that although they can be assumed to play a role in the ultimate dissolution of the material, these flaws are not necessarily responsible for the early loss of mechanical properties which have been observed by others. The implications of these observations for the development and evaluation of synthetic surgical repair materials are discussed.

    Notizen: The development of materials and technology suitable for the fabrication of intra-aortic balloons (IABs) is described; a brief discussion of materials and a fabrication process for flexible ventricular assist devices (heart massage cups) is also given. A series of block copolyether-urethanes was developed, the mechanical properties of which can be varied over a wide range. One member of the series was “tailored” specifically for nondistensible IABs of a particular design. Several of the block copolyether-urethanes were subjected to selected in vitro and in vivo tests for blood compatibility and compared favorably with other materials. A two-stage process was developed for the fabrication of high-quality IABs from block copolyether-urethanes. This process involves dip-forming over expendable wax mandrels, followed by removal of the wax and solvent-welding the balloon to the tip and catheter.
