    Notes: Summary Some physicians regard patients of Geriatric Units as a homogeneous population with respect to digoxin dosage requirements. Others advocate the use of pharmacokinetic models in prescribing digoxin for the elderly. Sixty in-patients of Geriatric Units were studied and the results compared with those previously obtained from 129 patients of other adult Units; all were receiving maintenance digoxin. For each patient the dose required to achieve a mean steady-state serum digoxin concentration of 1.6 nmol·l−1, the standardized dose, was calculated, assuming proportionality between the dose given and the concentration achieved. A mean of four estimates of standardized dose for each individual was used in the analysis. Threefold ranges of standardized dose covered the requirement of approximately 85% of patients both of Geriatric Units (62.5 to 187.5 μg per day) and of other adult Units (125 to 375 μg per day). The variables, serum creatinine concentration, sex, age, and body weight were of relatively little value in predicting the standardized dose for the patients in Geriatric Units. There was a sub-group of these in-patients for whom the standardized dose was extremely large.

    Notes: Summary A major role of therapeutic drug monitoring services is to advise on the dose of a drug which would be required to bring the concentration in the blood to within specific ‘therapeutic’ limits. Monitoring digoxin usage constitutes a substantial part of the work load. We have examined the potential variability in recommendations for digoxin dosages based on a series of serum digoxin measurements in each of 80 out-patients. In over a third of cases a dose, which seemed to be optimal on the basis of the first assay result, would have produced concentrations outside the conventional therapeutic range when the measurement was repeated. This was despite careful supervision of medication and the timing of the blood sample in relation to its administration. In routine practice the apparent variability in dose requirements would be far greater. Objectives of therapeutic drug monitoring services remain the same, the way forward would seem to lie in their taking on a heavy commitment to counsel and supervise patients before repeated blood sampling. However, effort and resources might be better channelled into producing simple prescribing aids relating the risk of cardiotoxicity directly to the size of the maintenance dose and the individual's renal function.

    Notes: Summary We compared our ability to predict the dose of medigoxin and of digoxin required to achieve a fixed serum concentration (the dose requirement) in 33 outpatients. Preliminary work supported the assumptions that the steady state glycoside concentration achieved was proportional to the daily dose given to an individual, and that the bioavailability of the different tablet presentations was similar for either glycoside. We were not able to predict the dose requirement from patient characteristics with any more certainty for medigoxin than for digoxin. Not only the between-patient variability in dose requirement, but also the within-patient variability, was similar for the two glycosides. However the digoxin used had a dissolution rate of over 90% in 1 h. When comparing medigoxin with digoxin of lower, or more variable dissolution rate, medigoxin may be preferable.

    Description: The electroencephalographic and behavioral responses during sleep of four subjects, aged 46 to 58 years, to three types of noises were tested over 14 consecutive nights. The stimuli were two DC-8 jet landing noises (each 30 seconds in duration and coming from DC-8 aircraft with and without acoustical treatment on the engine nacelles) and a 4-second burst of pink noise. Each of the noises was tested at nominal intensities of 61 and 79 dBA. Other physical descriptors of the noises were measured or computed. The results indicate that for an equivalent degree of sleep disruption, noise form the jet aircraft with untreated nacelles must be about 6 dBA less intense than the jet with acoustically treated nacelles. Predictions of the effects of noise on sleep appear, tentatively, to attain the highest accuracy when the physical descriptor of noise intensity includes information about the impulsive characteristics of that noise as well as its long-term spectral content.

    Description: We reconstruct the gravitational lensing convergence signal from Cosmic Microwave Background (CMB) polarization data taken by the Polarbear experiment and cross-correlate it with Cosmic Infrared Background (CIB) maps from the Herschel satellite. From the cross-spectra, we obtain evidence for gravitational lensing of the CMB polarization at a statistical significance of 4.0sigma and evidence for the presence of a lensing B-mode signal at a significance of 2.3sigma. We demonstrate that our results are not biased by instrumental and astrophysical systematic errors by performing null-tests, checks with simulated and real data, and analytical calculations. This measurement of polarization lensing, made via the robust cross-correlation channel, not only reinforces Polarbear auto-correlation measurements, but also represents one of the early steps towards establishing CMB polarization lensing as a powerful new probe of cosmology and astrophysics.

    Description: In the cores of some galaxy clusters the hot intracluster plasma is dense enough that it should cool radiatively in the cluster s lifetime, leading to continuous "cooling flows" of gas sinking towards the cluster center, yet no such cooling flow has been observed. The low observed star formation rates and cool gas masses for these "cool core" clusters suggest that much of the cooling must be offset by astrophysical feedback to prevent the formation of a runaway cooling flow. Here we report X-ray, optical, and infrared observations of the galaxy cluster SPT-CLJ2344-4243 at z = 0.596. These observations reveal an exceptionally luminous (L(sub 2-10 keV) = 8.2 10(exp 45) erg/s) galaxy cluster which hosts an extremely strong cooling flow (M(sub cool) = 3820 +/- 530 Stellar Mass/yr). Further, the central galaxy in this cluster appears to be experiencing a massive starburst (740 +/- 160 Stellar Mass/ yr), which suggests that the feedback source responsible for preventing runaway cooling in nearby cool core clusters may not yet be fully established in SPT-CLJ2344-4243. This large star formation rate implies that a significant fraction of the stars in the central galaxy of this cluster may form via accretion of the intracluster medium, rather than the current picture of central galaxies assembling entirely via mergers.

    Description: The LiteBIRD mission will map polarized fluctuations in the cosmic microwave background (CMB) to search for the signature of gravitational waves from inflation, potentially opening a window on the Universe a fraction of a second after the Big Bang. CMB measurements from space give access to the largest angular scales and the full frequency range to constrain Galactic foregrounds, and LiteBIRD has been designed to take best advantage of the unique window of space. LiteBIRD will have a powerful ability to separate Galactic foreground emission from the CMB due to its 15 frequency bands spaced between 40 and 402 GHz and sensitive 100-mK bolometers. LiteBIRD will provide stringent control of systematic errors due to the benign thermal environment at the second Lagrange point, L2, 20-K rapidly rotating half-wave plates on each telescope, and the ability to crosscheck its results by measuring both the reionization and recombination peaks in the B-mode power spectrum. LiteBIRD would be the next step in the series of CMB space missions, COBE, WMAP, and Planck, each of which has given landmark scientific discoveries.

    Description: We study the long term orbital evolution of a terrestrial planet under the gravitational perturbations of a giant planet. In particular, we are interested in situations where the two planets are in the same plane and are relatively close. We examine both possible configurations: the giant planet orbit being either outside or inside the orbit of the smaller planet. The perturbing potential is expanded to high orders and an analytical solution of the terrestrial planetary orbit is derived. The analytical estimates are then compared against results from the numerical integration of the full equations of motion and we find that the analytical solution works reasonably well. An interesting finding is that the new analytical estimates improve greatly the predictions for the timescales of the orbital evolution of the terrestrial planet compared to an octupole order expansion. Finally, we briefly discuss possible applications of the analytical estimates in astrophysical problems.
