    Notes: Various x-ray contrast materials, barium sulphate (BaSO4) and zirconium oxide (ZrO2), were tested for their radioactivity by means of a gamma spectrometer. While BaSO4 showed no detectable radiation emission, the ZrO2 materials of various makes showed slight to partially high radioactive emission. From all zirconium oxides, only the pharmaceutic agent OPTIPUR® (MERCK) showed no detectable radioactivity. Taking into account that these x-ray contrast materials remain in the body for decades as components of the bone cement, it would seem prudent to urge cement manufacturers to substitute high purity radiation-free zirconium in place of radioactive zirconium oxide.

    Notes: An aqueous humor draining device, with size comparable to that of the Krupin tube, was constructed by using poly-HEMA material. Deposits were found on the surface of poly-HEMA when contacted in vitro with the aqueous humor of the rabbit's eye. A fibrous structure, probably composed of proteins and other macromolecules, developed on poly-HEMA surface in 15 days after the draining device was implanted into the rabbit's eye. The draining device was still in function 250 days after its implantation. SEM analysis of the retrieved poly-HEMA draining device indicated that the poly-HEMA tube opening was not blocked by any substance. These results suggest that poly-HEMA could be used as a biomaterial for construction of the aqueous humor draining device to relieve the intraocular pressure of glaucoma patients. Its long-term application awaits further investigation.

    Notes: The patent literature can be a morass of confusion. It is complicated by legalese, often uncommunicative titles, and difficult, as well as cryptic, categorization. To keep current with the broad arena of patents on biomaterials and medical devices is much more difficult than keeping current with the scientific literature of the international journals.It is the goal of my column to provide readers with techiques and tools to better utilize this rich source of scientific information. Also, I intend to provide focused surveys of recent patents in specific areas relating to biomaterials and medical devices. Finally, I will try to include discussion of what constitutes patentable material and educate those of us who are new to patent literature and new to the requirements of applying for patents for our discoveries.I appeal to the readership to submit request for specific areas to be addressed in this column.     Notes: Numerous carrier systems have been developed for the controlled delivery of biologically active molecules such as drugs and diagnostic agents. The biophysical interactions between the biologically active molecules and their carriers, however, may denature the former and lead to reduced biological activity. In this study, a model nondenaturing carrier comprised of a nanocrystalline (10-7 m) tin oxide core and a surface-charge-reducing organic bonding layer (GF292) was synthesized. A subsequently bound protein (human transferrin) showed significant retained conformation by immunoelectron microscopy. In the synthesis of targeted drug systems and vaccines, nanocrystalline cores treated with appropriate surface-modifying agents may be suitable carriers.

    Notes: Porous coated Ti-6Al-4V has a high-cycle fatigue strength that is approximately one-third the strength of the uncoated material. Stress concentrations at the interface between the porous coating and the substrate contribute to this fatigue strength reduction. Modification of the interfacial geometry may reduce the severity of the stress concentrations. The interface between the porous coating and substrate was modeled using two-dimensional finite element analysis. This analysis identified geometric parameters that affect the value of the stress concentration factor, Kt. The effect of five geometric parameters onKt was determined: contact area between porous coating and substrate (CA), sinterneck radius (r), porous coating particle radius (R), interparticle distance (d), and interparticle sintering. Increased contact areas, decreased sinterneck radii, and decreased interparticle distances increased Kt, while porous coating particle size had a small effect on Kt. Sintering between neighboring porous coating particles increased Kt at the outermost sinternecks, but decreased Kt at inner sinternecks. The results can help predict how varying these coating parameters can reduce stress concentrations in a porous coated material.

    Notes: Calcium phosphate materials are being extensively investigated for orthopaedic and dental applications. Bone grafts and bone substitutes are being developed using many forms of these materials. Numerous medical devices are being coated with them. Because of the many forms that calcium phosphate materials take, characterization of the material under investigation is essential. Aworkshop sponsored by the Society For Biomaterials on characterization of calcium phosphate materials was held on June 14 and 15, 1989. Sessions discussing the historical development, in vitro characterization, in vitro coating characterization, in vivo coating characterization, the FDA perspective, and the manufacturers perspective were held. A summary of the workshop with references is presented.

    Notes: The mechanical properties of biodegradable polymers and composites proposed for use in internal fixation (in place of stainless steel) are crucial to the performance of devices made from them for support of healing bone. To assess the reported range of properties and degradation rates. we searched and reviewed papers and abstracts published in English from 1980 through 1988. Mechanical property data were found for poly(lactic acid), poly (glycolic acid), poly(∊-caprolactone), polydioxanone, poly(ortho ester), poly(ethylene oxide), and/or their copolymers. Reports of composites based on several of these materials, reinforced with nondegradable and degradable fibers, were also found. The largest group of studies involved poly(lactic acid). Mechanical test methods varied widely, and studies of the degradation of mechanical properties were performed under a variety of conditions, mostly in vitro rather than in vivo.Compared to annealed stainless steel, unreinforced biodegradable polymers were initially up to 36% as strong in tension and 54% in bending, but only about 3% as stiff in either test mode. With fiber reinforcement, reported highest initial strengths exceeded that of stainless steel. Stiffness reached 62% of stainless steel wiht nondegradable carbon fibers, 15% with degradable inorganic fibers, but only 5% with degradable polymeric fibers.The slowest-degrading unreinforced biodegradable polymers were poly(L-lactic acid) and poly(ortho ester). Biodegradable composites with carbon or inorganic fibers generally lost strength rapidly, with a slower loss of stiffness, suggesting the difficulty of fiber-matrix coupling in these system. The strength of composites reinforced wiht (lower modulus) degradable polymeric fibers decreased more slowly.Low implant stiffness might be expected to allow too much bone motion for satisfactory healing. However, unreinforced or degradable polymeric fiber reinforced materials have been used successfully clinically. The key has been careful selection of applications, plus use of designs and fixation methods distinctly different from those appropriate for stainless steel devices.

    Notes: Traditional water permeability evaluation procedures developed for measurement of gross leakage rates of textile prostheses are not adequate for testing the permeability of low-porosity bio-impregnated vascular prostheses. Obtaining reliable water permeability data for these low porosity grafts requires a technique that can accurately assess minor water leakage under the applied pressure. This study proposes an automated method for graft permeability assessment which measures the pressure change inside the graft sample as a function of water loss. The new unit of water permeability defines the pressure gradient at the given initial conditions. The proposed method for evaluating the water permeability of bio-impregnated vascular prostheses provides highly reliable data, is accurate, and excludes human error.

    Notes: Knot slippage continues to be a serious concern in surgery. This study explored the possibility of improving knot performance by exposing it to radiation from a CO2 laser. The specimens were mounted in a special device and exposed in a predetermined position for precisely controlled lengths of time. In the first set of experiments, the welding of ends was examined in nylon and polyester sutures. Although the welding was easily achieved, the joint was found brittle and its strength lower than that of a surgical knot. In the next set of experiments, the work was performed at a more fundamental level using oriented, high density, polyester films. The results showed that up to a certain level of exposure the tensile properties did not change, but beyond it a rapid decay took place. Shear tests on polyester sutures twisted together and then exposed to various levels of energy indicated that a bond of maximum strength developed at this exposure. Mersilene sutures of size 3/0 containing a two throw square knot when exposed to energy of this level produced optimal results. The knot strength increased by about 16% (from 13.4 N unexposed to 15.6 N exposed) and the slippage often known to occur in a two-throw knot was completely absent. This work performed under standard atmospheric conditions provided a direction for greatly enhancing the performance of a two-throw knot in terms of both the security and the strength. The investigation, however, must now be extended to other suture materials and sizes and to in vivo environments to realize the gain in actual surgery.

    Notes: Two types of hydroxylapatite (HA) surface coatings were tested as a means for bone attachment to polysulfone implants. The first coating was a roughened surface that was created by imbedding particulate HA in a polysulfone (PS) rod. The second surface was a smooth surface where HA particles were imbedded in the PS rod and the surface of the composite machined smooth. The smooth surface permitted only bonding of bone to exposed HA: whereas, the rough surface permittted mechanical interdigitation and bone bonding to HA. Specimens were implanted transcortically in the distal femurs of 12 rabbits, and groups of 4 were sacrificed at 4, 12, and 26 weeks. The attachment strength between bone and the HA/PS implants was determined via a push-out test. The results showed a statistically significant increase in push-out force for the rough surfaced implant when compared to the smooth surfaced implant at each time period. Histological analysis of samples from the in vivo study showed what appeared to be direct bone attachment on both the rough and smooth HA/PS implants.

    Notes: Anodic polarization measurements of several orthopaedic implant alloys - AISI 316L stainless steel, Co—Cr—Mo alloy (ASTM F-75), nickel-titanium (NiTi) alloy, commercial pure titanium, and Ti—6Al—4V - were carried out in Ringer's solution containing 1% bovine serum albumin (BSA) and in simple Ringer's solution. Constituent metals in the medium were analyzed by inductively coupled plasma emission spectrophotometry after measurements. Pitting potentials and total anodic current of former two alloys were constant in the two mediums and not affected by the addition of bovine serum albumin. However, the amount of dissolved metals from pure nickel and AISI 316L stainless steel were increased by the addition of BSA; dissolved metals from Co—Cr—Mo alloy were decreased by it.

    Notes: Nylon monofilament sutures were tested in a straight pull as well as a conventional knot pull tensile test. In each test, sutures were evaluated following storage under prevailing atmospheric conditions or saturation in whole human blood. Blood saturation decreased the ultimate tensile strength by as much as 20%. The present investigation of sutures that were stored under prevailing atmospheric conditions substantiated the proposal previously made for polypropylene monofilaments-that 60% of the ultimate tensile strength could be established as a fundamental USP criterion for Class I monofilament sutures.

    Notes: The capability of ALCAP ceramic drug-delivery implantable devices to release testosterone for 12 months was investigated. A total of 120 Sprague-Dawley male albino rats were distributed equally into three groups. Two ALCAP capsules, one nonimpregnated and the other impregnated with polylactic acid (PLA) were implanted into each rat in Groups I and II. Capsules implanted into Group I rats were loaded with 40 mg testosterone (T) each. Group II rats were implanted with two empty capsules (sham group), and Group III animals served as unimplanted controls. Eight rats from each group were euthanized at the end of 1, 3, 6, 9, and 12 months following the implantation of the ceramics. No significant changes in the weights of vital organs of rats were observed between any of the three different groups. Vas deferens and epididymal fluid were devoid of normal spermatozoa within 3 months of implanting the steroid-containing ceramics. Testes and epididymis weights decreased significantly in the rats implanted with ALCAP ceramics containing steroid, and the seminiferous tubules became oligospermic after 1 month and azoospermic after 3 months. Circulating levels of both LH and FSH hormones were suppressed in experimental rats. Serum T level was lower than the control but showed no significant difference in comparison to control rats. The data collected in this study suggest that: (a) ALCAP ceramic capsules are capable of delivering T in a sustained manner for 12 months; (b) T delivered by ALCAP capsules can be used effectively to regulate spermatogenesis in rats.

    Notes: Long-term in vivo strain sensing would provide information about deformation changes adjacent to implants during bone remodeling. Biodegradation of the cyanoacrylate adhesive commonly used to attach strain gauges to bone has generally limited in vivo strain sensing to time periods less than one month.Hydroxyapatite (HA) which has been used to attach implants to bone in vivo, was attached to strain gauges using a solvent-thinned polysulfone solution. Three HA-polysulfone surface morphologies were tested in a preliminary bench-top test. The single layer pressed surface morphology, which responded most accurately during bench-top testing, was modified slightly and applied to two gauges which were implanted on the femur of a greyhound.Strain measurements from the HA-backed gauges in place for four months in vivo were compared to strains measured from the contralateral femur. Comparison of the results indicated these gauges were well-bonded and that they were sensing strain accurately. After embedding in PMMA, the femur having the HA-backed gauge and the control femur were sectioned at the level of one of the HA-backed gauges, Microradiographs of these sections indicated no adverse tissue response to the HA-backed gauge on the endosteal or periosteal surface.

    Notes: Repeated needle trauma to grafts used for chronic hemodialysis may result in loss of graft integrity and aneurysmal formation. The gross and scanning electron microscope observations of the virgin expanded polytetrafluorethylene graft (ePTFE) material following repeated needle punctures are presented. Understanding how the normal structure of the ePTFE graft can be affected, particularly by larger diameter needles, underlines the effect and importance of needle puncture technique. In assuming that aneurysm related to needle punctures is the only evolutive complication of the vascular access, this analysis showed that the half life of an ePTFE prosthesis which was 6 mm in diameter and 25 cm in length was reached after about 1512 punctures using the 1.8-mm diameter hemodialysis needle, 1111 punctures with the 2.1-mm diameter needle or 784 punctures with the 2.5-mm diameter needle.

    Notes: Adhesion formation associated with tendon surgery is a widespread problem in which a healing tendon becomes adherent via scar tissue to surrounding structures such as bone, muscle, skin, tendon sheath, or other tendons. A model is described in which adhesions were generated reproducibly between the plantaris and Achilles tendons of the rabbit using a partial tenotomy, a Bunnel suture, and immobilization. Using this model, the effect of an absorbable barrier, INTERCEED (TC7),INTERCEED (TC7) Absorbable Adhesion Barrier is a trademark of Johnson & Johnson Patient Care Inc. on adhesion formation was investigated. This material, which is a fabric comprised of oxidized regenerated cellulose, was found to diminish significantly the extent and severity of intertendinous adhesions, assessed both mechanically and histologically. No evidence of a foreign body reaction was observed.

    Notes: Fatigue crack propagation in a notched specimen under cyclic loading was investigated for a series of experimental composite resins and six commercial composites. The fatigue resistance was measured as a function of filler content, water absorption, and post-curing temperature. Constants A and m of the Paris law have been obtained by linear regression analysis. The plot of fatigue resistance against volumetric filler content, V shows a sigmoidal relationship wiht a rapid increase in the range of V = 40-50% then tapering off at higher V. A substantial increase in the constant m of the Paris law was found for water-saturated commercial composite resins. Although increasing the post-curing temperature from 37°C to 70°C results in a 10% increase in fracture resistance, a high post-curing temperature is not favorable for highly filled composites (V 〉 60%) where more strained areas can be induced unless polymerization shrinkage is minimal. The ranking of fracture resistance for the commercial materials is: Occlusin 〉 P30 ≃ Adaptic II ≃ Ful-Fil 〉 Isomolar 〉 LC33. Based on the assumption that microcrack growth in the subsurface damage layer is the precursor of wear, fatigue resistance combined with fracture toughness satisfactorily predicted the clinical wear of commercial composites. Surface microfracture which amplifies the subsurface flaw size and creates undesirable stress concentrators can be subdued by high fracture toughness. In addition, high fatigue resistance retards the formation of wear particles, thereby reducing loss of material from the composite surface.

    Notes: Distortion of ceramometal restorations during porcelain application is often attributed to mismatch of thermal expansion coefficients. Little attention has been paid to treatment the metal receives after casting. Oxide formation, lattice parameter changes, and macroscopic distortion were studied for flat specimens of two ceramometal alloys, a high Pd alloy and a high Au alloy. SnO2 and In2O3 formed on the surface of the high Au alloy after the oxidation firing, while SnO2 and CuGa2O4 formed on the high Pd alloy. Lattice parameter changes reflected the loss of base metal elements in the formation of surface oxides. The edges of specimens distorted toward the ground side. Such distortion can be explained by the induction of residual stress in the metal surface during grinding and subsequent surface stress annealing accompanied with subsurface elastic recovery during firings. Oxide formation was not related to specimen distortion. This study suggests a relationship between surface preparation and specimen distortion during porcelain application procedures.

    Notes: Pyrolytic carbon (Pyrolite® carbon) valve occluders from 18 clinical Bjork-Shiley prostheses recovered at necropsy were examined for wear. In situ durations extended to 12 years. Results were combined with data from 17 recovered valves in the literature to produce a data base of 35 Bjork-Shiley valves. Cumulative in vivo results were compared to in vitro, Shiley type, accelerated life-cycle tester results for Bjork-Shiley prostheses. Comparative results were then interpreted using Hertzian impact wear theory. The theory provided a rational basis for the difference in wear observed in terms of applied dynamic load levels. Furthermore, Hertzian impact wear theory yielded an estimate of in vivo total dynamic loads on the order of 1 to 22 Newtons (0.2-5 lbf.) acting on the occluder of the heart valve prosthesis.

    Notes: Liquid extrusion pore volume analysis was used to characterize the pore structure of the collagen-based artificial skin material developed by Yannas and Burke.1 This technique is based on following the liquid loss from a saturated porous material as the pressure gradient across the material is increased. From the measured increments of volume of liquid drained at successively increasing pressures, each of which corresponds to a given pore radius, one obtains a cumulative pore volume curve as a function of pore radius from which pore volume distributions can be calculated. Using a nonswelling liquid (hexadecane) and a swelling liquid (aqueous surfactant solution), the effects of crosslinking and the method of drying were examined. In hexadecane, the median pore size of the uncrosslinked material is 25 μm, while for the crosslinked material, it is 115 μm. In water, the free volume of the crosslinked material is 36 mm3/mg, while for the uncrosslinked material it is an order of magnitude less. Sublimative drying provides a material with a total free volume of 55 mm3/mg, while evaporative drying collapses the porous structure to give a material with a total free volume of 8 mm3/mg. The liquid extrusion technique is relatively simple and allows pores to be measured in swollen materials in terms of their accessibility to skin cells. This method of pore characterization is considered more relevant for artificial skin than is a purely geometric characterization of pore dimensions.

    Notes: Since modern vascular surgery started with the discovery of heparin and the development of modern vascular grafts including autogenous saphenous vein, the speciality has been technologically driven. At the outset, vascular surgery depended almost entirely on the development of specialized clamps, instruments, and tools to permit decisive attack on the problems of occlusion.No less important was the development of insight into the basis of atherosclerosis, the discovery that atherogenesis with thrombosis is electrochemically identical to corrosion in pipes and therefore subject to chemical forces, which have not yet been delineated, as well as mechanical forces which have been delineated and permit one to attack the atheroma directly.Thus, classic replumbing techniques including bypass and endarterectomy have long been a part of the fundamental firmament of vascular surgery.Most recently, modern techniques in cleaning out blood vessels, removing occlusive processes, and modern thrombus-dissolving enzymes have all come to the forefront. Frequently multiple techniques are used simultaneously. This article is an attempt to summarize various aspects of this activity and describe several of the patents which have been seminal in the ultimate application of the techniques in both experimental animals and man. This should be the first of a series of efforts to summarize this area. The process of change has never been more kinetic than now.

    Notes: After the first approach to patent literature in a scientific library (Part I), we have completed the patent retrieval on the mentioned subject with the facilities of patent collections. This work describes the various methods: the direct access to original documents with classification codes, the consultation of the secondary literature of official gazettes and private services, and the use of indexes. The application of all these methods concerning calcium phosphate-containing biomaterials is presented and discussed. The retrieval resulted in 363 applications during the period 1975-1985 and 134 in 1986. Forty-nine percent and 63%, respectively, of these applications were basic patents, the rest were equivalents. The technical information which 80% of all these documents contain has already been indexed in Chemical Abstracts. The remaining documents are only available in patent collections. They deal with more technical problems and concentrate in specific IPC groups. Because of the language barrier, the access to Japanese patents is often only possible by means of equivalent patents applied in the United States, Great Britain, the Federal Republik of Germany or international authorities (EPO, WIPO). These are filed up in patent collections.

    Notes: The statistics and contents of patent and non-patent literature on calcium phosphate-containing biomaterials have been compared with each other. This was done by means of patent documents and scientific articles which have been published by the same author respectively on the same specific topic: prostheses coated with calcium phosphate-containing biomaterials. It was found that 765 articles in periodicals and 624 patents were brought out between 1975 and 1987. Both figures refer to calcium phosphate-containing biomaterials; the volume concerning the selected coating topic amounts to 40 articles and 83 patents. About 20% of the inventors publish also in periodicals but rarely about the subjects of their patents. The time lag between the disclosure of a patent and the publication of the corresponding article in a periodical comes to about one year but it is also possible that the article appears earlier than the patent. Patents preferably report on technical facts in detail (sintering processes, coating methods, components), periodicals much more on biological aspects (animal tests, histological investigations). The contents of patent and non-patent and non-patent literature do not overlap, but complete each other, so that studying patents should not be neglected to get an extensive survey on the progress of research. Volume, style, and formulation still provide an obstacle to patent documents for many research workers. New ways of excerpting patents are proposed for an efficient use.

    Notes: This article describes a method for analyzing the cell distribution occurring in the membrane encapsulating cylinders of specific polymers, metals, or ceramics when implanted in the paraspinal muscles in rats for periods between 2 weeks and 1 year. Using a computer-assisted cell-counting method, it is shown that the cell distribution exhibits a non-Gaussian spectrum, which can be modeled by a three-parameter statistical function. This observation was made possible because of the use of specific histological techniques, adapted to each particular material class, (polymer, metal, ceramic), keeping intact the implant-tissue interface. The parameters arising from the histomorphometric and statistical analyses allow comprehensive and quantitative comparisons of the tissue response induced by the various implanted materials. This pattern of cell distribution was observed with all of the implanted materials to one year. This behavior has also been observed in rabbit muscle and reported by others in subcutaneous tissue in mice. This method provides both a way to evaluate the tissue response to a surgically implanted material on a quantitative basis and a research tool for the study of tissue-implant compatibility.

    Notes: Bioactive glass powder (AW-G) was made into a rigid compound by mixing with ammonium hydrogen phosphate and was evaluated as a bone-defect filler. The proximal metaphysis of the rat tibia was drilled and packed with (a) polymethyl-methacrylate (PMMA) bone cement, (b) AW-G powder, (c) AW-G powder with ammonium hydrogen phosphate (AW-G)-(A-P), or (d) nothing, as a control. The animals, with different implantation periods up to 24 weeks, were sacrificed and the defective sites were histologically analyzed. The results revealed direct bonding between the bone tissue and the (AW-G)-(A-P). The general inflammatory reaction of (AW-G)-(A-P) was less than that of PMMA bone cement. The compressive strength of (AW-G)-(A-P) implanted subcutaneously into rats was measured chronologically and deterioration did not occur during a period of 24 weeks. The rigidity increased to 1.6 times 6 months after implantation as compared with the initial value.This compound can be used as paste and is transformed into a rigid compound in about 4 min without noticeable elevation of the temperature. Thus, this (AW-G)-(A-P) composite can be used as a bone defect filler, and there is a possibility that it can even be used as a bone cement if higher rigidity can be attained.

    Notes: Use of collagenous substrates for growth of attachment-dependent and attachment-independent cells have been reported in the literature. Growth of fibroblasts and epithelial cells on collagen sponges and fibers has been previously studied in our laboratory. This article reports the result of studies of neurite outgrowth on etched collagen fibers in cell culture. Collagen fibers with a mean diameter of about 134 μm were etched on glass coverslips using a collagenase solution until individual fibrils of 1 μm in diameter were observed. The kinetics of etching were optimized at a calcium chloride concentration of 5 mM and a collagenase activity of 300 unit/mL. At room temperature an etching time of 10 h maximized the number of fibrils exposed per fiber. Cell culture studies on etched collagen fibers indicate that neurites from rat fetal cortical neurons elongate along the longitudinal axes of collagen fibrils. In some instances the cell body observed was not on top of the collagen fibrils but situated between two parallel fibrils. Results of these experiments indicate that growth of neurites along collagen fibrils in cell culture may be a useful model to evaluate neurite extension in the presence of neurotrophic factors as well as to study optimization of substrates for nerve regeneration.

Standards are descriptive documents given official status by their producers, and in the case of biomaterials, they are generally intended to provide formalized descriptions of (a) materials used in the construction of medical devices, (b) materials used directly in surgical repairs, (c) test methods to evaluate materials for such applications and (d) methods for handling or processing such materials.Please consider this column not only a place to obtain biomaterials standards information but also a place to submit news of biomaterials standards developments in order to get the information before the public. This invitation extends to members of all professional organizations and government agencies in the United States and abroad who are active in standards development and would like specific activities or standards publicized. I will present the information in this column to the extent that allotted space allows.And last, a caveat - since this is a column, it contains both factual information and opinions.     Notes: Effective integration of synthetic fiber scaffold materials into connective tissues depends to a great degree on tissue attachment and ingrowth into the material. The authors have developed an experimental model that utilizes rat tendon explants to test the rates of cell colony formation on different types of synthetic fibers that are used as tendon and ligament scaffolds and for other medical uses. It was found that cell colony growth rates are linear on all 12 types of fibers tested. This agrees with the results of previous studies involving cell growth on synthetic fiber matrices. While most synthetic fibers showed similar colony growth rates, an overall trend was observed toward increased colony growth rates on fibers with relatively hydrophilic surfaces. One type of fiber that showed significantly increased colony growth rates was covalently coated with type I collagen. It was concluded that whereas most types of synthetic fibers had similar cell growth, a collagen coating can significantly enhance cell growth.

    Notes: To evaluate 2-cyanoacrylates as surgical adhesives, the bond strength in vivo as well as the tissue reaction was investigated using methyl-, ethyl-, isobutyl-, and ethoxyethyl-2-cyanoacrylate. In addition, their set time and spreading on blood were studied. When the 2-cyanoacrylates were applied to an incised site of rabbit skin, they could maintain the skin closure without suturing during the first week and the bond strength increased during the second week. Significant inflammatory response was observed around the subcutaneous tissue glued with methyl- and ethoxyethyl-2-cyanoacrylate and persisted for approximately one week. All the 2-cyanoacrylate polymers were absorbed and the tissues treated were healed two weeks after the operation. There was a mild inflammatory reaction in the tissue treated with ethyl- and isobutyl-2-cyanoacrylate, and their polymers still remained at the wound site at the second week postoperatively. The disappearance rate of the 2-cyanoacrylate polymers was roughly in proportion to the inflammatory tissue response. Ethoxyethyl-2-cyanoacrylate spread more broadly on tissues than the other 2-cyanoacrylates, while its set time was shorter than that of methyl- and ethyl-2-cyanoacrylate.

    Notes: Commercially pure 5-mm-diameter titanium (cpTi) discs received droplet inoculations of cells derived from rat bone marrow and were maintained in supplemented culture medium for 2-3 weeks. The cells and extracellular matrix (ECM) were processed for observation by light (LM), scanning (SEM), and transmission electron (TEM) microscopy. The latter was achieved by freeze-fracturing the solid metal from the resin-embedded tissue using a method which preserved the interface. Surface staining of whole discs revealed cells separated from the metal substratum by areas of ECM which stained positively using von Kossa's method to identify mineralization. At SEM, the ECM comprised dense interwoven collagen fiber networks which were partially obscured by globular masses (GMs). Individual GMs were associated with collagen fibers, especially at fiber intersections. EDAX line scan analysis confirmed the presence of Ca and P in these areas which were assumed to be spheritic foci of calcification since the Ca and P peaks diminished in areas which demonstrated only collagen fibers or the underlying cpTi. TEM examination confirmed the presence of globular mineralization and also revealed the presence of an interfacial zone between the metal substratum and the mineralized ECM elaborated by osteoblasts during the culture period. The interfacial zone comprised two layers, a bonding zone containing few collagen fragments and a ruthenium red positive layer containing more densely packed collagen fibers. We believe that this is the first report of both the formation of bonelike tissue on solid titanium substrata in vitro and demonstration of an interface which bears close morphological similarities to that known to develop in vivo.

    Notes: In order to increase mechanical properties of surgical cements, an apatitic filler has been linked to the polymethylmethacrylate (PMMA) matrix. We report here the study of the linkage between the mineral and organic components by a dielectric spectroscopy: Thermally Stimulated Current. First, the mineral - organic interface between apatitic - octocalcic phosphate and hydroxyethylmethacrylate (HEMA) has been investigated. Second, the filler - matrix interface between grafted apatite and PMMA has been examined. It has been shown that the filler is chemically linked to the matrix and stiffens the PMMA bone cement.

    Notes: To clarify involvement of complement activation in thrombus formation on polymer surfaces, in vitro complement activation was evaluated for polyethylene (PE) tubes radiation-graft copolymerized with acrylamide (AAm), acrylic acid (AC), 2-hydroxyethyl methacrylate (HEMA), Nvinylpyrrolidone (NVP), and vinyl alcohol (VOH), and compared to their in vivo antithrombogenicity and cell adherence in canine peripheral veins. The complement-activating surfaces (NVP and VOH) cause preferential adhesion of leukocytes and were more thrombogenic than the low complement-activating surfaces (AAm, PE, and HEMA). Infusion of naja haje cobra venom factor depressed leukocyte adhesion, followed by a marked decrease in thrombogenesis, for the strong classical-pathway-activating surface (NVP). Although estimation of in vitro activation for AC was inconclusive because of a large effect of adsorption, AC behaved like VOH in vivo. These results suggest that C5a(des Arg) mediated activation of leukocytes may play a role in thrombus formation by complement activation on polymer surfaces.

    Notes: Poly(anhydrides) proposed for use as vehicles for controlled drug delivery were administered subcutaneously in Sprague-Dawley rats at two dosage levels (800 mg/kg rat and 2400 mg/kg rat) for a period of eight weeks. Biocompatibility was assessed using a number of methods. Thirtysix clinical chemistry and hematology parameters were monitored throughout the study. Blood values were statistically analyzed for any possible effects due to the implanted polymer. After 8 weeks, rats were sacrificed and complete necropsies were performed. Histological evaluations of 33 organ sites including heart, lung, liver, kidney, and brain were performed. In addition, subcutaneous implant sites were excised and examined both grossly and microscopically. Results from evaluations of blood chemistry and hematology data, organ analyses and local implant site analyses overall demonstrated that the poly(anhydride) biomaterial possessed excellent in vivo biocompatibility.

    Notes: A hydrophilic polymer composite film (approx. 420 nm thick), with potential application as an interface for biomaterials has been prepared on nonorganic substrates, which include glass, silicon, and aluminum foil, using a glow discharge plasma polymerization technique. A thin film (110 nm thick) polymerized from hexane provided an adherent protective coating for the substrate material, and covalent bonding sites for the outer layer polymerized from N-vinyl-2-pyrrolidone. This outer layer provided the hydrophilic surface or interface. The two layers were copolymerized for a short period during transition between monomers to provide an intimate covalently bonded diffuse interphase. Preliminary in vitro and in vivo biocompatibility studies indicate that the hydrophilic film is non-cytotoxic, and does not increase the inflammatory response when compared with negative controls.

    Notes: Biomer, a segmented polyether polyurethane, has been analyzed via hydrolysis/gas chromatography to determine its composition. In addition to the previously reported 4,4′-methylene bis(phenyl isocyanate) (MDI), polytetramethylene glycol (PTMO), and ethylenediamine, we now report the presence of diethylamine, 1,3-diaminocyclohexane and poly(diisopropylaminoethyl methacrylate-co-decyl methacrylate), Biomer's cloudy insoluble phase. In addition, a method is presented to characterize the methacrylate additive by molecular weight based on GPC. Also found by chromatography were the antioxidants Santowhite Powder and BHT. XPS shows no Si (silicone) on the Biomer surface, and a total chloride analysis reports no chloride (〈0.03%). Time-of-flight SIMS data suggest evidence for the methacrylate additive at the surface, and mass spectroscopy can be interpreted as evidence for a diaminocyclohexane.

    Notes: The impact resistance of highly crosslinked plastic denture teeth materials such as SR-Orthosit (R), Crystal-ND (R), and Mitel-OM (R) was investigated and compared to that of conventional denture teeth materials. Dentures with upper second molars were placed on a metal model. The thicknesses of the denture teeth were 1.4, 1.0, and 0.4 mm. An impact force was repeatedly applied to the central fossae of the denture teeth with an impact test machine. Impact resistance was defined as the number of impacts sustained by the tooth material prior to fracture. The results showed that the impact resistance of highly crosslinked plastic teeth decreased with decreasing thickness, but was comparable to that of conventional plastic teeth when the teeth were 1.4 mm thick and supported by denture base resin. It is hypothesized that the layer of PMMA denture base resin beneath the denture teeth functions as a shock absorber during impact.

    Notes: To investigate the bone formation ability of porous hydroxyapatite (HA) and tricalcium phosphate (TCP), ceramic discs were implanted with or without rat marrow cells into subcutaneous sites in syngeneic rats. The discs of HA and TCP had identical microstructures: pore size was 190-230 μm, porosity was 50-60%, and they were fully interconnected. Implants without marrow cells (discs themselves) did not show bone formation, whereas implants with marrow cells showed bone formation in the pores of the ceramics. The bone formation of both HA and TCP occurred initially on the surface of the ceramic and progressed towards the center of the pore. The de novo bone was quantitated from decalcified serial sections of the implants. One month after implantation with marrow cells, the percentage fractions of the pore area filled with bone for implanted HA and TCP were 16.9 and 15.1, respectively. At 2 months after implantation with marrow cells, the fractions of bone were 34.3 and 30.9, respectively. These results indicate that both HA and TCP ceramics can show comparable osteogenic ability in the presence of marrow cells.

    Notes: In this article we studied the adsorption of serum albumin to substrata with a broad range of wettabilities from solutions with protein concentrations between 0.03 and 3.00 mg · mL-1 in a parallel-plate flow cell. Wall shear rates were varied between 20 and 2000 s-1. The amount of albumin adsorbed in a stationary state was always highest on PTFE, the most hydrophobic material employed and decreased with increasing wettability of the substrata. Increasing stationary amounts of adsorbed albumin were observed with increasing wall shear rates at the lowest protein concentration. Inverse observations were made at the highest protein concentration. Transmission electron micrographs of replicas from the albumin-coated substrata showed that proteins were mostly adsorbed in islandlike structures on the hydrophobic substrata. The tendency to form islandlike structures was shear rate- and concentration-dependent and disappeared gradually going to more hydrophilic substrata. On glass, the most hydrophilic material employed, a homogeneous, well distributed, fine knotted, reticulated structure was found. In conclusion, this study demonstrates that both the amount of adsorbed albumin as well as the surface structure of the adsorbed proteins are regulated by the substratum wettability. This observation may well account for the fact that substratum properties can be transferred by an adsorbed protein film to the interface with adhering cells or microorganisms.

    Notes: Polystyrene and poly(ethylene terephthalate) substrates were modified by radiofrequency plasma deposition with organic vapors comprised of carbon, oxygen, and hydrogen (acetone, methanol, glutaraldehyde, formic acid, allyl alcohol, and ethylene oxide). The treatments resulted in the deposition of a film at least 100 Å thick containing up to 26% atomic oxygen at the surface. A high oxygen incorporation was observed for vapors with a large oxygen-to-carbon ratio. Bovine aortic endothelial cell growth measured on acetone, methanol, and glutaraldehyde films was linearly correlated with the oxygen content of the treated surfaces. Nitrogen was incorporated in the surface by blending nitrogen gas into the organic vapors used for plasma deposition. The resulting nitrogen-containing substrates exhibited a high affinity for serum fibronectin but a moderate cell growth.

    Notes: Physicochemical properties of glass ceramics in weakly buffered, supersaturated, metastable calcium and phosphate solution were studied. Formations of calcium phosphate precipitates were also analyzed thermodynamically. All biomaterials examined here showed primarily dissolution process and subsequent nucleation. Dissolution was enhanced in those which contained Na2O and/or SiC. The dissolution of calcium and phosphate ions from biomaterials and subsequent nucleation of calcium phosphate salts after induction time resulted in a stable equilibrium having solubility characteristics similar to octacalcium phosphate (OCP). The most soluble material, CPSN-SiC, showed an initial transient equilibrium with amorphous calcium phosphate (ACP). From above results, calcium phosphate ceramics had certain driving force of its own which kept the equilibrium of solution to OCP thermodynamically for 7 days. Despite the favorable solubility equilibrium attained, there still remain problems for clinical application.

    Notes: The purpose of this study was to investigate bone formation after implantation of a mixture of apatite - wollastonite containing glass ceramic granules (A-W.GC, 220-355 μm) and fibrin glue with a low A-W.GC granular density. The fibrinogen was prepared from rat blood plasma. A mixture of A-W.GC granules and fibrin glue 1:4 in volume ratio (1:4 mixture) was implanted in the proximal metaphysis of the rat tibia, and bone formation was evaluated quantitatively by histomorphometry in undecalcified specimens. Microangiography was performed in animals sacrificed 5 days after the operation. The results were compared with those of a previous study in which a 1:1 mixture of A-W.GC granules and fibrin glue was used. With the 1:4 mixture, the A-W.GC density was 39.7% because of concentration of fibrin at packing. Good early vascularization comparable to that after implantation of the 1:1 mixture was observed. Bone formation proceeded at the same rate as after implantation of 1:1 mixture, but the level of the plateau was higher than that for the 1:1 mixture. The osteoconductive potential of the 1:4 A-W.GC-fibrin mixture was similar to that of the 1:1 mixture, and was higher than that of A-W.GC without fibrin.

    Notes: The contact of flowing blood with an artificial surface leads to adsorption of plasma proteins, followed by platelet adhesion and aggregation and thrombus formation. This phenomenon is enhanced by turbulent flow at joints, bifurcations, and constrictions. In therapeutic plasmapheresis using an IBM blood cell separator, blockage of the extracorporeal circulation system by platelet-fibrin thrombi imposed a halt in treatment for manual clearance of the circuit for 66 in 149 cases (44%). Thus it was decided to passivate the surface of the extracorporeal circuit by filling the tubing with 4% human serum albumin 15-20 min before the treatment session and then displacing the albumin solution with the patient's blood without creating an air-liquid interface. After introduction of this technique, a blockage was observed for only 11 in 239 cases (5%). In vitro measurements of platelet accumulation on the internal surface of the circulation system were carried out using washed human platelets labeled with 111In-oxine in the presence of a 40% hematocrit. Preadsorption of the surface with albumin reduced platelet deposition to 4-5% that observed for an equivalent pretreatment with physiological saline.

    Notes: Well characterized, laboratory-synthesized polymeric materials which have been extensively tested for biocompatibility via initial platelet and protein deposition in an acute ex vivo canine model were placed as interpositional series shunts in canines with chronically implanted iliac arteriovenous shunts ex vivo. Platelet deposition was measured on a base polyurethane block copolymer, a sulfonated ionic derivative, an alkyl grafted (C18) derivative, Biomer, polyethylene, and polydimethylsiloxane for 24 h using radiolabeled platelets. Platelet survival and in vitro aggregation were determined to investigate the effects of the shunting pprocedure on experimental animals. The viability of adopting a chronic arteriovenous (iliac) shunted canine model for use with series shunts to evaluate polyurethanes having applications as materials in vascular graft construction was investigated and the results compared with acute model data.

    Notes: A polyetherurethane (PEU) wound covering with non-interconnected micropores up to approximately 5 μm has been prepared by means of a phase inversion process. This highly elastic, very thin (15-20 μm), pliable wound covering showed good, immediate adherence to wet wound surfaces and high water vapor permeability, but was impermeable to bacteria. In guinea pigs epidermal wound healing of partial-thickness wounds under PEU wound coverings was accelerated compared with uncovered controls and an occlusive wound covering, OpSite. Water in liquid form or wound exudate could not leak through the PEU covering, but its high water vapor permeability induced concentration of the wound exudate into a jellylike clot layer, which apparently accelerated reepithelialization. The main conclusion from a clinical study on 20 donor sites was that the use of the PEU covering reduced pain, besides prevention of fluid retention. No differences in epithelialization were seen in comparison to tulle gras-treated wounds.

    Notes: Short carbon fiber reinforced composites could potentially replace some of the metal alloys used in orthopedic implants. In particular, polysulfone and, more recently, polyetheretherketone have been considered as the matrix material for carbon fiber reinforced composite implant materials. ASTM standards F813 and F619 for direct contact cell culture evaluation and extraction were employed to determine the in vitro biocompatibility of a carbon fiber composite of polyetheretherketone, PEEK, in comparison to a carbon fiber reinforced polysulfone composite. The cell cultures were assessed qualitatively by microscopy and quantitatively using an enzyme assay to determine cytotoxicity. Overall, the cellular response to the PEEK and polysulfone composites were negligible indicating that further in vivo studies with these materials are appropriate.

    Notes: A method has been designed for making spherical microencapsulation of activatedcarbon particles for use in the removal of metabolic wastes and/or toxins from blood. This bench-scale method was able to make better uniformity in particle size, and at a higher particle production rate. The material properties such as particle size, particle porosity, electromicroscopy of surface morphology, surface hardness, and pore-size distribution were characterized in order to conclude with their adsorption capacity for specific toxin solutes. The performance characteristics of these self-prepared particle encapsulation was also evaluated against the commercially available hemoperfusion particles, e.g., Kuraray's DHP (Japan). The encapsulation particles were also coated with polyhydroxyethyl methacrylate (PHEMA) for blood compatibility purpose.

    Notes: The purpose of this study is to examine the formation of hydroxyapatite on the surface of glass-ceramics (chemical composition: SiO2, 34.2; P2O5, 16.3; CaO, 44.9; MgO, 4.6; CaF2, 0.5 in weight ratio). Two experiments were performed. In the first experiment, plates (2 × 25 × 25 mm) of glass-ceramics containing apatite-wollastonite (designated A · W-GC) were used. In the second experiment, plates (15 × 10 × 2 mm) of A · W-GC and its parent glass (designated G) were used. In each experiment, two paired specimens of identical material, one tied with silk thread, the other not tied, were implanted subcutaneously into rats. In both experiments, bonding to each other of both tied and untied specimens was observed one month after implantation. A pattern resembling hydroxyapatite was identified on the detached surface of the bonded A · W-GC by micro-beam x-ray diffraction. The weak crystalline pattern was also observed on the detached surface of bonded G samples. Analysis of the interface by SEM-EPMA showed that a calcium-phosphorus rich layer formed between the two bonded surfaces in both experiments. It is suggested that the bonding between the two materials was formed by the calcium-phosphorus rich layer, and that the calcium-phosphorus rich layer is virtually identical to hydroxyapatite.

    Notes: The tissue/biomaterial interface reactions of four elastomers - selected as candidates for scaffolding for tympanic membrane tissue in a total alloplastic middle ear prosthesis - were studied at the electron microscopical level after implantation in the rat middle ear. Time-dependent changes in the phagocyte/polymer interface suggested degradation of porous implants made of Estane polyether urethane, polypropylene oxide, and a poly(ethylene oxide hydantoin) and poly(tetramethylene terephthalate) segmented polyether polyester copolymer (HPOE/PBT copolymer), but not of dense Silastic silicone rubber implants. Silastic was always encapsulated in fibrous tissue. Contact between fibrous tissue and HPOE/PBT copolymer or Estane was established in the third month, but fibrous tissue was never seen close to polypropylene oxide. Bone made contact only with Estane and HPOE/PBT copolymer implants. The bone/copolymer interface showed an electron-dense layer morphologically similar to that seen between bone and hydroxyapatite ceramic, suggesting that with respect to bone HPOE/PBT copolymer behaves like a bioactive implant material. The electron-dense layer was absent at the bone/Estane interface. Estane and especially HPOE/PBT copolymer seem to be suitable as alloplastic tympanic membrane because of their interface behavior with respect to fibrous tissue and bone.

    Notes: Amphiphilic block copolymers containing poly(dimethylsiloxane), poly(ethylene oxide), as well as heparin-coated glass beads and tubes were evaluated for the amounts and activities of surface-immobilized heparin. Because the amphiphilic copolymer system is thermodynanmcally predicted to demonstrate low-energy phase enrichment on the surfaces of aircast films, studies were also undertaken to understand the in vitro results. Solvent-cast copolymer films have a heterogeneous microphase-separated structure according to transmission electron micrographs. Wilhelmy plate contact angle analysis indicates significant surface restructuring occurs upon hydration. Attenuated total reflectance infrared spectroscopy studies of the desiccated and hydrated films at two different sampling depths show compositional heterogeneity as a function of depth, as well as near surface restructuring allowing surface enrichment of the high-energy segments following contact with water. Significant concentrations of heparin are detected on the surface of these coatings by toluidine blue assays. In addition, a portion of the surface-bound heparin maintains its original bioactivity as determined by recalification times, thrombin times, and Factor Xa assays. These substrates were also tested for platelet adhesion and activation reactions in vitro using polymer-coated beads in rabbit platelet-rich plasma. Heparinized polymers promoted low levels of platelet adhesion and serotonin release. Surface concentrations of heparin from bioactivity assays were then correlated with platelet adhesion and the extent of platelet release to assess the efficacy of this heparin-immobilized copolymer as a blood-compatible material or coating.

    Notes: This study provides information on the behavior of macrophages on the surface of the subcutaneously implanted hydrogel strips. Hydrogel containing —OH,—CO—NH—, and (CH3)2N—groups induced a spreading of macrophages on the implants. The materials containing —SO3H groups slightly, and materials containing—COOH groups more intensively, inhibited spreading of the macrophages. The fusion of macrophages into multinucleate cells was inhibited on the surface of materials containing acidic groups (—SO3H, —COOH) and increased on the hydrogels containing 30 mol% of alkaline (CH3)2N— groups in comparison with hydrogels containing —OH or —CO—NH— groups. The differences of the behavior of macrophages on the surface of individual types of hydrogels are probably independent on the adsorption of plasma fibrnectin onto the hydrogels. The correlation between the macrophages spreading and fusion and surface charge of the hydrogel implant can be hypothetically explained by electrostatic interaction between macrophages cell membrane and implant.

    Notes: This study examines the histological effects of different sizes of polyethylene particles implanted into the rabbit tibia. Seventeen mature New Zealand white female rabbits were allocated into three groups. Group 1 (5 rabbits) received polyethylene particles averaging approximately 16 μm in diameter, implanted into the right proximal tibia through a drill hole. Group 2 (5 animals) received particles averaging 26 μm, and Group 3 (7 rabbits) received particles averaging 67 μm. The left tibia was drilled but not implanted. Animals were sacrificed after 16 weeks. Histological analysis disclosed decreased hematopoietic activity within the left tibial drill hole. In all groups, the right tibia demonstrated positively birefringent polyethylene particles surrounded by, and within (smaller particles), histiocytes and giant cells in a fibrous tissue stroma. Statistical analysis disclosed more fibrocytes and less marrow cells at the interface of Group 3 (largest particles) compared to Group 1 and 2. Larger polyethylene particles, being less readily phagocytosed, appear to produce more fibrous encapsulation, compared to particles of a smaller size. The histological reaction stimulated by the different sizes of polyethylene particles resembled the membrane surrounding loose joint arthroplasties in humans.

    Notes: Highly porous poly(2-hydroxyethyl methacrylate) (PHEMA) microspheres prepared by suspension polymerization of 2-hydroxyethyl methacrylate (HEMA) in presence of polymeric diluents such as poly(methyl methacrylate) (PMMA) in toluene and poly(tetramethylene glycol) (PTMG) were made radiopaque by esterification of the reactive hydroxyl groups with iothalamic acid and iopanoic acid, two radiopaque substances clinically used. Of the various solvents and catalysts examined, tetrahydrofuran (THF) and N,N′-dimethyl paratoluidine (DMPT) were found to be best for obtaining a high degree of conversion. More than 30 wt% iodine could be bound to the microspheres which made them sufficiently radiopaque to be imaged radiographically. Microspheres retained their porosity, swelling ability, hydrophilicity, and surface morphology to a significant extent after iodination. Preliminary implantation studies of such microspheres subcutaneously in rats have shown no adverse tissue reactions over a 6-month period. It is suggested that these microspheres would prove to be useful as particulate emboli in endovascular embolization.

    Notes: In view of reports on the involvement of high-molecular-weight (HMW) kininogen and high-density lipoprotein (HDL) in the Vroman effect, we studied the adsorption of fibrinogen, HMW kininogen, HDL and several other proteins from pooled human plasma and congenitally HMW kininogen-deficient plasma onto glass and low-density polyethylene, both as a function of the plasma concentration and the contact time. Mixtures of purified (lipo)proteins were also included in the study. Protein adsorption was determined by means of a two-step enzyme-immunoassay. Our results support the hypothesis that HMW kininogen is involved in the displacement of fibrinogen, which is almost instantly adsorbed from normal plasma onto glass. On hydrophobic polymers like polyethylene, the low amounts of adsorbed fibrinogen and HMW kininogen from plasma and concentrated plasma solutions may be due to a preferential adsorption of HDL.

    Notes: Reconstituted, 100-μm-thick collagen sheets were crosslinked with either UV light, chromium, or cysteine for use as a burn covering. The sheets were also exposed to a “surface agent” (hydroxyproline, fibronectin, or soluble basement membrane matrix containing Type IV collagen) as a preliminary step in planned adherence studies. Since some chemicals render the collagen toxic, the modified sheets were tested for cytotoxicity using human keratinocytes and fibroblasts. Autoradiography and 3H-thymidine incorporation were used to quantitate the proliferative rate of these cells in vitro. There was a universal depression of keratinocyte incorporation of 3H-thymidine following a 1-day exposure to any collagen sheet when compared to cells not exposed to any collagen. This effect had lessened by 5 days' exposure to the collagen. Conversely, the fibroblasts showed an enhancement in rate of incorporation after 1-day exposure, especially for cells exposed to collagen sheets crosslinked by UV light. This effect had also lessened by 5 days' exposure. Autoradiography showed few significant variations for any of the cells exposed for either time period. Chromium leaching was determined, with no values greater than 30% of the allowable maximum set by both the British and American Pharmacopeia.

    Notes: A new canine model utilizing an implantable chamber with multiple bone in-growth channels has been used to study the response of intramedullary bone to various implant materials and surfaces.The first group of dogs received implants containing channels lined by smooth-surfaced coupons of titanium, titanium alloy, sputter-hydroxyapatite-coated (HA-coated) titanium alloy, and polyethylene. A pattern of early initial bone ingrowth by 2 weeks, becoming maximal at 6 to 12 weeks with remodeling to a more mature lamellar bone, and later resorption by 24 weeks was seen for all test groups, with fibrous tissue interfaces covering the smooth test coupons at all time points. Significantly increased bone ingrowth in the sputter-HA coated group was found only at 6 weeks.The second group of dogs received implants with channels lined by surfaceroughened coupons of either titanium or plasma-HA-coated titanium, half of which were also packed with a crystalline-HA grouting at the time of surgery. At both 6 and 12 weeks, bone ingrowth was greatly enhanced by the presence of the plasma-HA coating or the crystalline-HA grouting as compared to the uncoated titanium channels. Histologically, bone was seen to bond directly to the plasma-HA coating and the crystalline-HA grouting. A thin fibrous tissue layer was noted between bone and the titanium in most areas, but evidence of direct bone contact to the metal surface was seen. Mechanical testing in tension of intact coupon--bone--coupon units revealed significant strength of the bone--plasma--HA bond, with failure initiating at the metal-HA interface with forces of 15.3 N at 6 weeks, increasing to 44.8 N at 12 weeks. Plasma-HA-lined channels with crystalline-HA packing required similar forces for failure. No significant adhesion strength was noted for the titanium channels at 6 weeks, and only the crystalline-HA-filled channels displayed measurable strength of the bone--titanium interface at 12 weeks, with a force of 9 N needed for failure.

    Notes: Changes in the fibrinogen molecule after its adsorption to biomaterials may be important in determining blood compatibility. Previously, postadsorptive transitions were detected by measuring the elutability of adsorbed proteins with surfactant solutions. The elutability decreased with increased residence time, suggesting that protein - surface interactions increased with residence time. In this study, we have examined the effects of polymer structure and composition, chain mobility, and hydrophobicity on the postadsorptive transitions of fibrinogen. Glassy, rigid polymers showed high fibrinogen adsorption, regardless of whether the polymer was hydrophilic or hydrophobic. However, the binding strength (as measured by elutability) was much lower on hydrophilic polymers and oxygen-containing hydrophobic polymers. Short-term transitions, requiring 2 h or less after adsorption, were observed only on hydrophobic polymers that contained no oxygen. More gradual transitions were observed on hydrophobic polymers containing oxygen, but only after a lag time of 1-4 h.

    Notes: Nitrogen-ion-implanted Ti-6Al-4V against UHMW polythylene was tested on a joint simulator to evaluate the wear behavior under different conditions. It was concluded that the polymer wear rate was not affected by ion implantation. The metallic wear, on the other hand, was substantially reduced by this treatment. It was also found that ion implantation resulted in good protection of the metal surface against third-body wear by PMMA, while the introduction of C.P. titanium particles in the joint initiated tremendious wear. Although ion implantation improved the wear of titanium alloy against UHMW polyethylene, the durability of the implanted layer needs further study.

    Notes: Hyperemia associated with an inflammatory response has been investigated in rats, by using four different experimental models, i.e., “positive” and “negative” polymer implants from the pharmacopea, operative control, and abscess induced by turpentine oil. 133Xenon clearance, infrared thermography and Laser Doppler Flowmetry (LDF) were used to monitor the subcutaneous local hemodynamic changes from 1 to 40 postoperative days. LDF proved to be a sensitive, reproducible method, able to discriminate positive from negative implants already at the 3rd postoperative day and up to 40 days. This increased local blood flow was also visualized at the site of positive implants at the 14th and 21st postoperative days by means of 133Xe Clearance. Additional information obtained by infrared thermography allowed discrimination between positive implants and control sites but only at the very early stage (1 to 3 days). The significance of the different data collected by the three techniques was correlated with histological events occurring at the different implant sites. LDF may therefore represent a useful technique for noninvasive semiquantitative assessment of tissue response to biomaterials.

    Notes: The level of mercury vapors in the oral cavity was determined by analyzing 242 samples of intra-oral air before and after removing, setting, and polishing dental amalgam. The analysis was effected using atomic absorption spectrometry in cold vapors with the detection limit of 6 μg/m3. A score of 1 to 3 was assigned to each restoration to adjust the differences of the amalgam sizes. The present study showed that the vapor was released during any procedure: removing, setting, and polishing amalgam. The mean levels were between 85 and 326 μg/m3. However, before the restorative procedures, no mercury vapors had been detected in the oral cavity. This investigation has also demonstrated a significant direct correlation between Hg vapor concentrations in intra-oral air and the sizes of amalgam restorations during each procedure: removing, setting, and polishing. The statistic analysis showed no significant difference of mercury vapors in the oral cavity between the use or nonuse of water coolant during the amalgam polishing.

    Notes: Twenty-eight mature New Zealand white female rabbits were allocated into 4 groups of 7 rabbits. Group 1 received a coiled wire of cobalt chrome alloy (Vitallium) (16 gauge × 1 cm). Group 2 received an equal weight of cobalt chrome particles averaging 15.4 μm in diameter. Group 3 received a coiled wire implant of commercially pure (C.P.) titanium (16 gauge × 1 cm). Group 4 received the same weight of C.P. titanium particles averaging 3.8 μm. The implants were placed through a drill hole in the proximal right tibia; the left tibia served as a prepared but nonimplanted control. The animals were killed after 16 weeks and quantitative histology was performed on undecalcified sections of the implant area. Bulk cobalt chrome and titanium implants were surrounded by a thin, incomplete, fibrous tissue layer with decreased numbers of cells. Trabeculae of bone were present within this connective tissue envelope. Fingerlike projections of bone enveloped the implant where it abutted endosteal bone. Clumped and loosely scattered cobalt chrome and titanium particles were surrounded by a minimal amount of fibrous connective tissue. Smaller particles were present within cells. Hematopoietic cells abutted the bulk or particulate implants directly. There was no evidence of acute or chronic inflammation or foreign body reaction. These results should be contrasted with those of Howie et al. in which intraarticular cobalt chrome particles stimulated a rapid proliferation of macrophages and synovial degeneration after 1 week. This may be due to a direct toxic effect of metals in an intra-articular environment, the smaller particle size used in that study, or to abrasive injury to the hyaline cartilage and subsequent synovitis. Our results underscore the general inert properties of these metals in the short term, when implanted into bone in the sizes and physical forms chosen.

    Notes: Type, size and distribution of extracellular matrix vesicles (MV), known mediators of primary calcification, were studied around bone-bonding and metal-oxide containing, nonbonding, glass-ceramic implants. This was performed in order to further understand the different effects of implants on bone healing. At 14 days after implantation in adult rat tibial bone the effects of different implants on MV were studied by transmission electron microscopy and computerized morphometry. A total number of 4607 MV in 245 electron micrographs were counted and grouped according to diameter, distance from the calcifying front, and classified as four types: “empty,” “amorphous,” “crystal,” and “rupture.” The sequence of types according to diameter and distance was recorded as follows around both implants tested: “rupture” MV were the closest to the front with the largest diameter, followed by “crystal,” “amorphous,” and “empty,” MV with the largest distance from the front and the smallest diameter. Most vesicles were concentrated in a distance of less than 2.4 μm from the front and between diameters of 0.06 μm and 0.22 μm. The noncalcified extracellular matrix around bone-bonding implants contained more MV than the matrix around the nonbonding type (26.24 MV/10 μm2 and 18.76 MV/10μm2). MV distribution according to type showed that around bonding implants ther was a higher percentage of “crystal” and a lower percent age of “rupture” when compared to the nonbonding type. These results indicate that bonding implansts affect osteoblastic function by increasing tht vesicular number and retardation of intravesicular crystal fromation. It might be suggested that bonding implants induce an increase in the process of primary calcification and a decreased rate of crystal formation resulting with the highest organization of the healing bone.

    Notes: The ability of bacteria to adhere to surfaces is a major cause of concern in the use of biomaterial substrates. The adhesion of Staphylococcus epidermidis strain 1938 was examined using image analysis and was found not to correlate with polymer surface tension, unlike that of Lactobacillus acidophilus, which adhered to more hydrophobic polymers. A fimbriated uropathogenic E. coli strain showed very low levels of adherence to the biomaterials. Precoating the polymers with lactobacilli significantly reduced the staphylococcal and E. coli adhesion, a result which could have clinical significance. An additional finding was that the interaction of staphylococci and E. coli with lactobacilli coated polymers altered the adhesion profile of the latter. Lactobacilli appeared to detach from polymers of low surface tension and reattach to polymers with high surface tensions. This resulted in the highest levels of exclusion of uropathogens being found for lactobacilli-coated glass and sulfonated polystyrene, both of which are hydrophilic (with high surface tensions). These results demonstrate that lactobacilli can be used to coat biomaterial surfaces leading to a reduced adhesion of uropathogens.

    Notes: β-Galactosidase has been immobilized within thermally reversible hydrogel beads and has been studied in batch and packed bed reactor systems. The enzyme was entrapped in a copolymer hydrogel of N-isopropylacrylamide (NIPAAm) and acrylamide (AAm) as beads were formed in an inverse suspension polymerization. A reversible deswelling and reswelling of the hydrogel matrix was induced by first warming and then cooling through 37-40°C, which is the lower critical solution temperature, LCST, of the backbone copolymer. The optimum temperature for maximum activity of the immobilized enzyme-gel bead system was found to be 30-357deg;C in a batch mode and 40°C in a packed bed reactor, which were both below the 50°C optimum for the free enzyme. These differences are understandable, since the mass transfer rates of substrate and product within the pores of the gel matrix are controlled mainly by the temperature, so therefore it is the temperature which governs the overall activity of the immobilized enzyme system. It was also found that when the operational temperature in the packed bed reactor was cycled between temperatures below (35°C) and above (45°C) the copolymer gel LCST, the activity of the immobilized enzyme almost fully recovered after each cycle. In fact, the enzyme-gel system exhibited a complete “shut-off” in activity at 50°C which was the temperature where the free enzyme showed its maximum activity. The thermal cycling operation of LCST enzyme-gel beads can be used to enhance overall activity and productivity of a packed bed reactor, when compared to isothermal operation of this reactor. This is due to the thermally induced “pumping” which enhances mass transfer rates of substrate in and product out of the gel beads.

    Notes: Novel polyetherurethaneureas which have been synthesized by the present authors were chosen for the substrate polymers, on which adhesion of platelets was investigated. The number of adhered platelets and the amount of serotonin released from platelets adhered on the polymers and the protein-coated polymers were determined by radioisotope method. Both of them were ehanced with increasing content of urea linkages in the polyetherurethaneureas. The platelet adhesion was discussed in terms of the denaturation of plasma proteins upon adsorption, which was determined by Fourier-transform infrared spectroscopy. With increasing degree of protein denaturation, the platelet adhesion and the serotonin release were enhanced. This relationship was particularly evident in the case of albumin adsorption. It was shown that the surface properties of substrate polymers affect the protein adsorption, which in turn influences the adhesion of platelets.

    Notes: Our previous studies reported the performance of Macroporous Biphasic Calcium Phosphate (MBCP) in spine fusion. In the present study, this material was used in block forms in selected patients with tumoral resection in long bone. Two cases were chosen with large benign bone tumors. Clinical and radiographic assessments, CT scans, and NMR were performed after 16 months, and in one case control biopsies were taken. In order to understand the kinetic process of biodegradation of the MBCP blocks and bone formation at the expense of the ceramics, an experimental study in surgicaly created bond defects in canine femoral cortices was made. The MBCP blocks recovered after implantation period from 2 to 18 weeks were analyzed using histological, stereological, ultrastructural, electron microprobe, and IR spectroscopy analyses. This study demonstrated the efficiency of MBCP blocks for filling pathological defects in human long bone. The biointegration process of the MBCP blocks was due to a partial dissolution of the ceramics crystals (b-TCP content) by multinucleated cells. Simultaneously, bone ingrowth at the expense of the ceramic is observed. The new bone formation inside the MBCP macropores and in the spaces between the blocks, involved the formation of a new cortical bone on the outer part, and a trabecularlike bone with bone marrow in the inner part of the implant. The biological resorption of the MBCP ceramic decreased after 1 month implantation in dog, due to the protective role of the newly formed lamellar bone on the surface and in the core of the ceramics.

    Notes: The surface reactions of calcium phosphate ceramics have been thought to play an important role in bonding with living bone. Hydroxyapatite (HA), tricalcium phosphate (TCP), and two kinds of apatite-containing glass ceramics were immersed in three types of solutions with different chemical constituents. The first solution was a physiological saline, the second contained phosphate (PO4), and the third was a balanced salt solution consisting of calcium (Ca), magnesium (Mg), and PO4. After serial incubation periods, changes in the solutions were assessed by measurement of total Ca, Mg, and PO4. The ceramic surfaces were studied using scanning electron spectroscopy, infrared reflection spectroscopy, and thin-film x-ray diffraction. The surface reactions of the ceramics were greatly affected by the chemical compositions of the surrounding media. In the complete solution with both Ca and PO4, a carbonated apatite layer was formed on the surfaces of HA, TCP, and the glass ceramics. In comparison to HA and TCP, the glass ceramics were characterized as Ca-releasing materials, the dissolved Ca creating an apatite layer on the surfaces in a few days, in conjunction with PO4 stock in the surrounding media. The immersion test with various solutions proved to be a simple and effective method of assessing surface conditions of ceramic materials.

    Notes: Tissue reactions toward biodegradable poly(L-lactic acid) implants were monitored by studying the activity pattern of seven enzymes as a function of time: alkaline phosphatase, acid phosphatase, α-naphthyl acetyl esterase, β-glucuronidase, ATP-ase, NADH-reductase, and lactate dehydrogenase. Cell types were identified by their specific enzyme patterns, their morphology and location. Special attention was paid to the enzyme patterns of macrophages, fibroblasts and polymorphonuclear granulocytes (PMNs), being involved in foreign body reactions or inflammatory responses. One day after implantation, an influx of neutrophilic and eosinophilic granulocytes was observed, coinciding with activity of alkaline phosphatase (PMN's) and β-glucuronidase (eosinophils). From day 3 on, macrophages containing ATP-ase, acid phosphatase and esterase could be observed. From day 7 on, lactate dehydrogenase, the enzyme normally involved in the conversion of lactic acid, and its coenzyme NADH-reductase were observed in macrophages and fibroblasts. These two enzymes demonstrated more activity than expected on basis of wound-healing reactions upon implantation of a nonbiodegradable, inert biomaterial (as, e.g., Teflon). It is concluded that the biodegradable poly (L-lactic acid) used in these implantation studies is tissue compatible, and evokes a foreign body reaction with minor macrophage and giant cell activity, as observed during this 3-week implantation period. Most enzyme patterns were simply due to a wound-healing reaction. The slightly increased levels of LDH and NADH suggest the release of lactic acid from the implant, and thus confirms the biodegradable nature of this polymer.

    Notes: High-strength bioactive glass-ceramic A-W was soaked in various acellular aqueous solutions different in ion concentrations and pH. After soaking for 7 and 30 days, surface structural changes of the glassceramic were investigated by means of Fourier transform infrared reflection spectroscopy, thin-film x-ray diffraction, and scanning electronmicroscopic observation, in comparison with in vivo surface structural changes. So-called Tris buffer solution, pure water buffered with trishydroxymethyl-aminomethane, which had been used by various workers as a “simulated body fluid,” did not reproduce the in vivo surface structural changes, i.e., apatite formation on the surface. A solution, ion concentrations and pH of which are almost equal to those of the human blood plasma - i.e., Na+ 142.0, K+ 5.0, Mg2+ 1.5, Ca2+ 2.5, Cl- 148.8, HCO3- 4.2 and PO42- 1.0 mM and buffered at pH 7.25 with the trishydroxymethylaminomethane-most precisely reproduced in vivo surface structure change. This shows that careful selection of simulated body fluid is required for in vitro experiments. The results also support the concept that the apatite phase on the surface of glass-ceramic A-W is formed by a chemical reaction of the glass-ceramic with the Ca2+, HPO42-, and OH- ions in the body fluid.
