    Notes: Abstract The development of differential and selective media for the isolation of fungi pathogenic for man is briefly reviewed. A new primary culture technique for medical mycology is presented. This involves the use of an appropriate paper carrier impregnated with suitable concentrations of cycloheximide and chloramphenicol. The carrier, when properly placed on the surface of an agar medium inoculated with a mixed-flora specimen, favors the isolation of a wide spectrum of pathogenic and opportunistic fungi. Evaluation systems for the technique included artificial mixtures of bacteria and fungi, pooled clinical materials artificially seeded with fungi, and routine clinical laboratory specimens. Test fungi were grouped according to a selection pattern based on the extent of their outgrowth within the bacterial inhibition zone (BIZ) established by the carrier on a battery of media. Two main advantages of the antibiotics-impregnated carrier system over the antibiotics-fortified medium system are: (1) the diffusion gradient of cycloheximide allows for the selective outgrowth throughout the BIZ of pathogenic fungi resistant to it, and allows the more susceptible fungal pathogens and “opportunists” to grow out up to their levels of tolerance to the antibiotic within the BIZ; (2) the system is economically advantageous.

    Notes: Summary Rice agar and corn meal agar, with and without Tween 80, were evaluated clinically as directly inoculable selective and differential media for the isolation ofC. albicans from vulvovaginal specimens taken from pregnant women. Chlamydospore formation on these media was investigated as a criterion for the identification ofC. albicans. Of 301 patients cultured, 118 (39.2 %) gave positive cultures for yeast-like fungi of the genusCandida. Of 118 strains for which fermentation patterns were determined, 69 (58.5 %) gave the pattern forC. albicans. Of these, 56 (81.1 %) formed chlamydospores. Tween 80 was found to exert a very stimulating influence on chlamydospore production. Rice agar with Tween 80 appeared to be the most efficient medium for eliciting chlamydospores. However, since strains of 4 out of 6 species ofCandida isolated were found to sporulate it was concluded that chlamydospore formation is not a reliable criterion for the speciation ofC. albicans. Each of the 4 media served satisfactorily as a directly inoculable selective medium for the isolation of yeast-like fungi of the genusCandida from vulvovaginal specimens. None of the media appeared to preferentially stimulate chlamydospore production inC. albicans.

    Notes: Abstract Absidia corymbifera is rarely cited as a cause of zygomycosis in the human host. Presented here is a case of absidiomycosis in a 29 year old construction worker hospitalized with a 6 year history of intravenous drug abuse, serological evidence for HIV infection, and the symptomatology of AIDS. ACT scan and gallium uptake imaging identified a nonfunctioning right kidney. Following a biopsy and a nephrectomy, histopathology revealed hyphal components consistent with zygomycosis. Culture of the biopsy and surgical specimens yielded A. corymbifera. Clinically the patient responded well to amphotericin B therapy.

    Notes: Abstract A 35 year old female patient with acute lymphocytic leukemia developed fusariosis in which dissemination appeared to be limited to cutaneous and vascular invasion. The first evidence of fungemia occurred nearly seven months after initial hospitalization. The fungus was identified asFusarium sp. and was considered a contaminant. Two weeks later blood cultures were again positive forFusarium sp. and the patient was placed on amphotericin B and 5-fluorocytosine therapy. The following day developing lesions were noted on her forearms and face; lesions ultimately spread to her trunk, abdomen, and lower extremities. Skin lesion biopsy sections revealed abundant septate and branching hyphae throughout the dermis and within capillaries. Twenty-six days after the initial isolation the patient died. Post-mortem blood cultures gave rise to the same fungus, which was identified asFusarium monoliforme. Postmortem cultures and stains of spleen, liver, lung, and brain specimens were all negative for fungi. The primary source and portal of entry of the organism remained undetermined.

    Notes: Abstract Presented is a case of widely disseminated systemic pseudallescheriasis in a 41 year old male with acute myelocytic leukemia. The immediate cause of death appeared to be due to an extensive invasion of the lungs which showed massive intra-alveolar hemorrhages, congestion, mycotic thrombi, and multiple fungal lesions in all lobes. Pseudallescheria boydii was diagnosed histopathologically by virtue of its characteristic conidia present in miliary lesions throughout a wide range of host's tissues, including the brain and the thyroid. Three antemortem blood specimens cultured during the patient's final hospital stay were positive for the fungus. It was concluded the fungemia was responsible for the rapid and widespread dispersion of P. boydii in this debilitated patient who was granulocytopenic and immunosuppressed.

    Notes: The N-monoethylated, N-monoethylated and N-mono-1-propylated homologues of mesoporphyrin IX dimethyl ester and the corresponding methylated and 1-propylated homologues of coproporphyrin III tetramethyl ester have been prepared. Substituted porphyrins in a series could be distinguished from each other and from the parent porphyrin by their dication absorption spectra. All compounds gave molecular ions of varying intensities during electron impact mass spectrometry but thermal N-dealkylation to mesoporphyrin IX or coproporphyrin III methyl esters occurred readily and it was difficult to obtain spectra not showing fragmentation ions derived from this process. The compounds serve as models for the ‘green pigments’ produced in vivo by the action of some drugs and illustrate the problem of dealkylation during their analysis by electron impact mass spectrometry.

    Notes: The electron impact mass spectrum (70 eV) of 26-trimethylsilyloxy-4-cholesten-3-one shows a base peak at m/e 196, attributable to combination of the trimethylsilyl group with a fragment of m/e 123 produced by cleavage across ring B. 18O labelling confirms that the fragment of m/e 196 contains the 3-oxygen atom, and 2H labelling indicates retention of the trimethylsilyl moiety. Similar ions are observed from other Δ4-3-ketones with sidechain trimethylsilyloxy groups: abundances depend on the site of substitution. The corresponding enol trimethylsilyl ethers are readily separable from the keto forms by gas-liquid chromatography, and afford mass spectra in which the molecular ions are abundant, while ions of m/e 196 are of only moderate intensity.

    Notes: Thirty-six Δ5- and 5α-3β-hydroxysteroids have been oxidized with cholesterol oxidase to give the corresponding Δ4- and 5α-3-ketosteroids, respectively. The mass spectral characteristics of the products (or their trimethylsilyl ether derivatives, in the case of 3-keto-hydroxysteroids) varied considerably, depending especially on the nature of the C-17 sidechain. The ion of m/e 124 (or its equivalent) from cleavage of ring B was frequently a major fragment from Δ4-3-ketosteroids, but in some instances was of insignificant abundance. Trimethylsilylation of the product of the oxidation of neoergosterol gave neoergosterone enol-trimethylsilyl ether. Fragmentations of the sidechain oredominated in the mass spectra of the 5α-3-ketosteroids.

    Notes: The electron impact mass spectra of prostaglandins F1α, F2α and F3α methyl ester cyclic 9,11-methane-, n-butane-, cyclohexane- and benzeneboronate 15-trimethylsilyl ethers show base peaks corresponding to [M - (C-16 - C-20)]+, i.e. [M - 71]+ for prostaglandins F1α and F2α but [M - 69]+ for prostaglandin F3α yielding identical ions in the latter two cases. Derivatives of prostaglandin F2α and F3α are difficult to separate by gas chromatography, so that the use of this ion type for single ion monitoring of either prostaglandin (as has been reported) is ambiguous. The chemical ionization mass spectra of these cyclic boronates, however, show distinctive base peaks for prostaglandins F1α, F2α and F3α at m/e 317, 315 and 313 respectively, corresponding to [M - RBO2H2 - TMSO]+. Fragmentation patterns have been investigated by the use of [3,3,4,4-2H4]prostaglandin F2α and by the formation of [2H9]trimethylsilyl ethers and a [2H3]methyl ester. Cleavage of the C-15 - C-16 bond is of minor importance under chemical ionization conditions. The possible value of the [M - RBO2H2 - TMSO]+ ions for single ion monitoring is explored: specificity is aided by the formation of the same ions from a series of boronates of characteristic retention indices.
