    Notes: Summary The pharmacokinetics of Factor VIII was evaluated by mathematical modeling in a large-scale study in 62 haemophilia-A subjects, in whom 137 plasma Factor VIII-time curves were measured during single dose (n=87) and repeated-dose (n=47) treatments for prophylaxis or minor bleeding episodes. The pharmacokinetic parameters [mean (SD)] estimated from single-dose curves were: clearance 3.85 ml·h−1·kg−1, volume of distribution 58.2 ml·kg−1, mean residence time 15.9 h. Parameters calculated from repeated-dose curves were: clearance 3.93 ml·h−1·kg−1, volume of distribution 61.8 ml·kg−1, and half-life 12.2 h. In patients with mild haemophilia, pharmaco-statistical analysis revealed that the endogenous synthesis of Factor VIII was constant and was not influenced by the administration of exogenous Factor VIII. The coefficient of variation for intra-individual variability of Factor VIII kinetics (estimated according to the Standard Two-Stage method) was 20.7% in single-dose curves and 23.2% in repeated-dose curves.

    Notes: In this paper, we describe a reappraisal of an earlier cost-effectiveness study [7] by application of this extrapolated Q-TWIST method [12]. Our cost-effectiveness study [7] evaluated the pharmacoeconomic profile of adjuvant cyclophosphamide, methotrexate and fluorouracil (CMF) in patients with node-positive breast cancer [13] and utilised a Gompertz analysis to estimate lifetime overall survival (OS), which was 862 and 756 discounted years per 100 patients in the CMF and the control groups, respectively. In applying the extrapolated Q-TWIST method to this data set, a second Gompertz analysis is carried out on the disease-free survival (DFS) curve of the two patient groups. Lifetime DFS of Bonadonna's patients can thus be estimated as 741 years in the CMF group and 572 years in the controls (discounted values normalised to 100 patients). Figure 1 shows the two curves of OS and DFS for the CMF group and the partition of the area under the OS curve into its three components. To introduce an assessment of quality of life into these data, the Q-TWIST method partitions the value of OS into the three components called TWIST (absence of symptoms and toxicity), TOX (time spent with toxicity) and REL (survival after relapse). Hence, OS = TWIST + REL + TOX, where REL = OS − DFS. In the control group, TOX = 0 and TWIST = DFS because no treatment-related toxicity is present.

    Notes: Abstract Objective: Lamotrigine as add-on treatment (500 mg per day) is effective in patients with refractory epilepsy, but its high cost requires a pharmacoeconomic analysis. We conducted a retrospective lifetime cost–utility study in which clinical data were derived from a recent placebo-controlled clinical trial, cost-of-illness data were drawn from a previous ad-hoc study, and quality-of-life values were obtained by prospectively interviewing a separate group of 81 patients referred to our institution with epilepsy. Results: Our analysis showed that chronic lamotrigine treatment implies an incremental lifetime cost of about $1 600 000 for every 100 patients. Incremental lifetime utility was around 40 quality-adjusted life-years (QALYs) for every 100 patients. On the basis of these data, adjunctive lamotrigine was estimated to cost approximately $41 000 per QALY gained. Sensitivity testing suggested a range of $25 000–$85 000 per QALY gained. Conclusion: Adjunctive lamotrigine (500 mg per day) in refractory epilepsy seems to have a worse pharmacoeconomic profile than many pharmacological treatments commonly used in areas other than epilepsy. Further data are needed to determine if lamotrigine can be equally effective at lower (and less costly) daily doses which could markedly improve its pharmacoeconomic characteristics.

    Notes: Abstract Background: The analysis of published survival curves can be used as the basis for incremental cost-effectiveness analyses in which two treatments are compared with each other in terms of cost per life year saved. In patients with node-positive breast cancer adjuvant chemotherapy with cyclophosphamide + methotrexate + fluorouracil has been reported to improve survival in comparison with patients who are not given this treatment. Methods: To assess the pharmacoeconomic profile of this adjuvant chemotherapeutic regimen in terms of cost per life-year gained, we conducted an incremental cost-effectiveness analysis in which the Gompertz model was used to calculate the lifetime estimate of the patient-years gained by treated subjects compared to controls. Results: Using data from a published, controlled long-term trial involving 207 patients treated with cyclophosphamide + methotrexate + fluorouracil and 179 controls, we estimated that this adjuvant chemotherapy improved life expectancy by 357 patient-years per 100 subjects. Direct costs, which were almost exclusively related to the administration of chemotherapy, were estimated to be US $159,516 per 100 patients. On the basis of these data, adjuvant chemotherapy was found to imply an incremental cost of US $447 per life-year saved. Conclusions: The cost-effectiveness ratio of adjuvant chemotherapy with cyclophosphamide + methotrexate + fluorouracil in patients with node-positive breast cancer seems to be particularly favourable in comparison with estimates of cost per life-year saved previously calculated for other types of pharmacological intervention.

    Description: The present study analyses large values (or pulses) of local and zonally integrated meridional atmospheric heat transport due to transient eddies. The data used is the European Centre for Medium-Range Weather Forecasts ERA-Interim reanalysis with daily, 0.7° latitude and longitude resolution. The domain of interest is the extra-tropics. First, the circulation associated with local pulses of heat transport is described. This is found to match many of the features found in warm conveyor belts, although important regional differences exist. The large values of heat transport are seen to be associated with co-varying meridional velocity and moist static energy anomalies. Next, it is shown that there exist strong pulses of meridional heat transport when a zonal integral around a given latitude circle is considered. These zonal pulses are only partly driven by the synchronised occurrence of a large number of local pulses. The existence of such pronounced variability in zonally integrated meridional heat transport can have important consequences for the energy balance of the high latitudes.

    Description: The present study analyses extreme events in meridional atmospheric heat transport, due to transient eddies, in the time-frequency domain. The data used is the European Centre for Medium-Range Weather Forecasts ERA-Interim reanalysis data, at the 850 mb pressure level and with daily, 0.7◦ latitude and longitude resolution. Fast growing atmospheric modes are associated with a large heat transport, which would suggest a link between transport extremes and growing baroclinic systems (defined here as motions in the 2.5-6 day band). However, by analysing wavelet power spectra of transport extremes, and of the corresponding meridional velocity and moist static energy temporal anomalies, this is found not to be the case. In fact, baroclinic systems provide only a modest contribution to the integrated power of the extreme heat transport event spectrum. The transport extremes are driven by very precise phase and coherence relationships between the velocity and moist static energy anomalies, acting over a broad range of frequencies (2.5 – 30 days). Planetary scale motions (k = 0–4) with periods beyond 6 days play a key role in this framework.
