    Notes: Abstract Recently, the use of astemizole and terfenadine, both non-sedating H1-antihistamines, caused considerable concern. Several case reports suggested an association of both drugs with an increased risk of torsades de pointes, a special form of ventricular tachycardia. The increased risk of both H1-antihistamines was associated with exposure to supratherapeutic doses; for terfenadine the risk was also associated with concomitant exposure to the cytochrome P-450 inhibitors ketoconazole, erythromycin and cimetidine. To predict the size of the population that runs the risk of developing this potentially fatal adverse reaction in the Netherlands, the prevalence of prescribing supratherapeutic doses and the concomitant exposure to terfenadine and cytochrome P-450 inhibitors was studied. Data were obtained from the PHARMO data base in 1990, a pharmacy-based record linkage system encompassing a catchment population of 300,000 individuals. The results of the study showed that the prescribing of supratherapeutic doses and the concomitant exposure to terfenadine and cytochrome P-450 inhibitors was low. Furthermore, the results of a sensitivity analysis showed that the risk of fatal torsades de pointes has to be as high as 1 in 10,000 to cause one death in the Netherlands in one year.

    Notes: Abstract The use of bromocriptine for the suppression of postpartum lactation drew widespread attention to a potentially increased risk for cardiovascular and cerebrovascular adverse effects. To estimate the incidence of these events a follow-up study was performed among 2,130 women of 15–44 years of age who were treated with a course of bromocriptine in 1990–1992. None of these women were admitted to the hospital for cardiovascular or cerebrovascular events. However, the incidence of pregnancy hypertension and the use of cardiovascular drugs increased considerably in the last 2 months before delivery. Therefore, cardiovascular or cerebrovascular events can probably be explained by pre-existing morbidity rather than by the use of bromocriptine. We estimated, using a ‘worst-case’ analysis, that fewer than 2 Dutch women each year would develop serious cardiovascular or cerebrovascular events.

    Notes: Abstract In October 1990, a recall procedure was issued regarding the drug acitretin. The recommended post-therapy contraception period after acitretin therapy was extended from 2 months to 2 years. For a postmarketing surveillance study, we recruited a cohort from the source population of women aged 15–45 years who were exposed to acitretin. Recruitment occurred through dermatologists, and pharmacists plus dispensing general practitioners. We describe the speed of and the response to the recruitment procedures, and the representativeness of the recruited cohort. We also studied whether the individuals who gave informed consent would have preferred to be recruited by either dermatologists or pharmacists, and whether the information obtained from pharmacists and dispensing general practitioners was valid. This study revealed that pharmacists and dispensing general practitioners (drug dispensers) recruited their subjects rapidly, with no or little selection; they attained a 42% response. Dermatologists recruited their subjects slowly and selectively; they attained a 24% response. The majority of women (60%) recruited by dermatologists would have given their informed consent if they would have been recruited by their pharmacists. Drug dispensers are essential contributors to the recruitment of a study population. We do advise that such recruitment for a postmarketing surveillance study occurs by means of a collaboration between pharmacists and physicians.

    Notes: Es wird ein registrierendes spektrometrisches Verfahren zur Analyse von Gasgemischen beschrieben. Die Gase werden elektrodenlos in einem Hochfrequenzfeld angeregt. Während des Absaugens der Gase wird die Emission verschiedener Banden registriert. Die Intensitäten durchlaufen beim Absaugen ein Maximum, das ein Bild der Anregungsfunktion ist. Unter gleichen Anregungsbedingungen besteht bei Gasgemischen eine Proportionalität zwischen der im Maximum gemessenen Intensität und der Konzentration der Gase. Darauf beruht das entwickelte Analysenverfahren. Es fordert für Gase unterschiedlichen Drucks lediglich, daß der Druck vor der Messung einen Mindestwert überschreitet, bei der beschriebenen Anordnung 0,2 Torr. Es wurden Wasserstoff, Stickstoff und Kohlenoxyd bestimmt, die aus Stählen extrahiert wurden; gemessen wurde im sichtbaren Spektralbereich. Der Vorteil des Verfahrens wird in der hohen Nachweisempfindlichkeit und schnellen Ausführbarkeit gesehen. Die Bestimmung der drei Gase in Metallen ist in 2,5 bis 4 min möglich. Das Versuchsgerät bedarf für Reihenuntersuchungen einer konstruktiven Vervollkommnung. Das Verfahren läßt sich über die Bestimmung des Kohlenoxyds, Stickstoffs und Wasserstoffs hinaus auf andere Gasgemische ausdehnen und kann zur kontinuierlichen Überprüfung der Gaszusammensetzung bei Herstellungsprozessen dienen.

    Notes: Blends of poly(styrene)-block-poly(ethene-co-but-1-ene)-block-poly(styrene) (SEBS) with isotactic polypropylene (PP) and syndiotactic PP, respectively, were investigated. The morphology was observed by means of scanning electron microscopy (SEM) and transmission electron microscopy (TEM). The cryofracture surfaces studied by SEM did not show any particles that were pulled out, so that a good compatibility between SEBS and different PPs could be assumed. The multiphase character of the blends could be well detected by TEM of RuO4 stained samples. TEM micrographs of two-layer specimens revealed that SEBS tends to diffuse into the PP phase under formation of micelles. The block copolymer shows a reorientation phenomenon of large domains at the interface before the diffusion into the PP phase occurs. The interfacial strength as a function of annealing time was measured by a peel test of two-layer specimens. Mechanical properties are studied and related to the blend morphology. © 1996 John Wiley & Sons, Inc.

    Notes: Glass bead-reinforced isotactic polypropene hybrid composites containing 0-20 vol % thermoplastic elastomers were prepared to study both structure/property relationship and morphology development. Polystyrene-block-poly(ethene-co-but-1-ene)-block-polystyrene (SEBS) and the corresponding block copolymer grafted with maleic anhydride (SEBS-g-MA) were used as thermoplastic elastomers. Hybrid composites containing SEBS gave higher Young's moduli than did those containing SEBS-g-MA. The experimental Young's moduli were in good agreement with the theoretical predictions according to Lewis and Nielsen. The lower moduli of hybrid composites containing SEBS-g-MA were attributed to interlayer formation and in situ encapsulation of glass beads, resulting in core-shell particles. This elastomeric interlayer impaired the filler reinforcement. Analysis of tensile yield stress and results of lap-shear tests confirmed strong filler-polymer interactions in composites containing SEBS-g-MA. Only in excess of a critical volume fraction did SEBS-g-MA afford a significant improvement of the notched Izod impact strength. In contrast to stiffness, Izod impact strength was not influenced by the type of elastomer and morphology. Investigation of crystallization and scanning electron microscopic studies proved the in situ encapsulation of the glass beads with SEBS-g-MA, whereas SEBS addition results in separately dispersed glass beads and SEBS microphases. © 1996 John Wiley & Sons, Inc.
