Sommaire Notre programme ‘en circuit’ fut développé afin d'identifier les contractions atriales et ventriculaires prématurées et les battements omis. Nous sommes spécialement attentifs á reconnaitre les artefacts. 14 patients avec des degrés, d'arythmie élevés furent étudiés et 108 000 battements furent analysés indépendamment par des cardiologues et corrélés avec les analyses d'ordinateur. Sur un total de 7458 contractions ventriculaires prématurées, 411 phénomènes furent détectés en tant que positifs ou négatifs faux. Ceci représente une erreur de 5%. Sur les 826 contractions atriales prématurées, 140 erreurs furent relevées (17%). Sur un groupe de 1772 battements omis (av block etc.), 159 furent identifiés incorrectement (9%). Sur 1410 ectopies ventriculaires non prématurées, 164 résultats étaient incorrects (12%). La grande majorité des potentiels et artefacts musculaires fut correctement classifiée, et détectée (erreur—11–15%). Grâce á l'ordinateur numérique, l'analyse certaine des arythmies les plus importantes chez les patients sous soins intenses fut réalisée. Une partie principale du programme devrait être orientée vers la détection des artefacts.
Abstract:
Zusammenfassung Unser schritthaltendes Programm wurde entwickelt, um ein vorzeitiges Zusammenziehen von Atrium und Ventrikel sowie fehlende Herzschläge festzustellen. Die Erkennung von Artefakten erfolgt mit besonderer Sorgfalt. 14 Patienten mit starker Arrhythmie wurden untersucht. 108.000 Herzschläge wurden unabhängig voneinander von Kardiologen untersucht und mit der Rechneranalyse in Beziehung gebracht. Von insgesamt 7458 verfrühten Ventrikelkontraktionen waren 411 falsche Positive oder Negative. Hierbei, handelt es sich um eine Fehlerrate von 5%. Bei 826 vorzeitigen Atriumkontraktionen wurden 140 Fehler festgestellt (17%). In einer Gruppe von 1772 fehlenden Schlägen (av block usw.) wurden 159 falsch identifiziert (9%). Bei 1410 ektopischen Ventrikelkontraktionen, die nicht vorzeitig erfolgten erschienen 164 falsche Ergebnisse (12%). Die meisten Muskelpotentiale und Artefakte wurden richtig klassiert und entdeckt (Fehlerrate 11–15%). Mit Hilfe eines digitalen Rechners wurde eine zuverlässige Analyse der wichtigsten Arrhythmien bei Patienten auf Intensivstationen erreicht. Einer der wichtigsten Teile des Programms sollte sich mit der Feststellung von Artefakten befassen.

Abstract Our online program was developed to identify premature atrial and ventricular contractions and missed beats. Special care is taken with the recognition of artefacts. 14 patients with a high rate of arrhythmias were studied and 108 000 beats analysed independently by cardiologists and correlated with the computer analysis. Of a total of 7458 premature ventricular contractions, 411 events were detected false positives or negatives. This is an error of 5%. Out of the 826 premature atrial contractions 140 mistakes were found (17%). In a group 1772 missed beats (av-block etc.) 159 were improperly identified (9%). Of 1410 ventricular ectopics without prematurity there were 164 incorrect results (12%). The vast majority of muscle potentials and artefacts was properly classified and detected (error 11–15%). The main reason for the errors is the bad signal-to-noise ratio experienced from time to time. With the help of a digital computer a reliable analysis of the most important arrhythmias in intensive care patients has been achieved. A main part of the program should be concerned with artefact detection.
