    Notes: Abstract Objective: The present study compares the effects of yohimbine, an alpha2 adrenoceptor antagonist, and anetholtrithione, a reference drug in the treatment of dry mouth, in patients treated with psychotropic drugs (tricyclic antidepressants or neuroleptics) and suffering from xerostomia. Methods: Ten patients were included in a randomized, double-blind, cross-over study, and receiving after yohimbine (3 × 6 mg per day) or anetholtrithione (3 × 25 mg per day) orally for 5 days. Salivary secretion was estimated under resting conditions, before any drug, and then on day 6, 1 h after the ingestion of yohimbine or anetholtrithione. Results: Compared with basal secretion, the increase in salivary flow was significantly more marked after yohimbine than after anetholtrithione. Conclusion: This study demonstrates the sialogenic effect of yohimbine in drug-induced dry mouth.

    Notes: Summary Sympathetic efferent pathways and alpha-adrenergic receptivity were investigated in one patient with spinal cord transection (D1 level) and orthostatic hypotension. The lack of increase in catecholamine plasma levels during orthostasis and the paradoxical pressor effect of clonidine (2 μg/kg orally) suggested complete interruption of efferent sympathetic pathways. The pressor response to exogenous noradrenaline was significantly higher in the patient than in 6 normal controls (0.09 vs 0.72 μg·kg−1), indicating supersensitivity of vascular α-adrenoceptors. The platelet α2-adrenergic receptor number, measured with [3H]yohimbine, was 507 in the patient vs 178 fmol·mg−1 protein in controls. The increase in systolic blood pressure induced by 10 mg midodrine, a specific α1-agonist, was significantly higher in the patient (δ=56 mm Hg) than in controls (δ=15 mm Hg). The results indicate that in the patient there was α-adrenergic supersensitivity both of α1- and α2-adrenoceptors. This led to successfully oral treatment of the orthostatic hypotension with clonidine 150 μg bd and midodrine 10 mg bd.

    Notes: Abstract Objective: While data on drug abuse liability have been determined for selected drugs in experimental studies, there is limited information available about drug abuse in real life. This study presents the results of a simple experimental epidemiologic survey of prescription forgeries in community pharmacies in the Midi-Pyrénées area (southwest France). During four periods (September–October 1991, January–June 1992, September–October 1992 and January–June 1993), resident pharmacy students in several volunteer pharmacies were asked to complete a specific report form for each suspect prescription request. The main criteria used to identify forgeries were: inadequate dosage, multiple use of the prescription form, drafting not in accordance with the rules of prescription or false prescription forms (stolen prescription forms, photocopies). Results: A total of 165 falsified prescriptions were collected. The 305 drugs involved in these forged prescriptions were opiate analgesics, benzodiazepines, amphetamines and minor opiate analgesics. Medications were essentially buprenorphine, flunitrazepam (in 2 mg dosage), phenobarbitone+amphetamine and chlorazepate. Conclusion: These results suggest that forged prescriptions can be used as an indicator of potential abuse liability of marketed psychoactive drugs. Although this survey cannot describe the real prevalence of the misuse or abuse of drugs, it constitutes a useful warning epidemiologic system to elicit early observations regarding new misuses of drugs as they are requested at the pharmacy.

    Notes: Summary The effects of 14 days of treatment with the alpha2-adrenoceptor antagonist yohimbine on cardiovascular parameters (blood pressure and heart rate), plasma catecholamines, insulin and non-esterified fatty acids (NEFA), and the number of alpha2-adrenoceptors on platelets have been evaluated. The treatment increased plasma noradrenaline but not adrenaline and did not change the other cardiovascular, metabolic or routine blood parameters. It caused a decrease in the velocity of adrenaline-induced platelet aggregation, although [3H]-yohimbine binding to platelets remained unchanged. Acute administration of yohimbine before the experiment led to a significant increase in plasma noradrenaline and NEFA levels. After the 15 day treatment, acute administration of the same dose of yohimbine had less effect on plasma NEFA and there was no significant change in plasma noradrenaline. The acute lipid-mobilizing effect of yohimbine in healthy volunteers persisted after the 14 days of treatment with no change in other parameters. The prolonged alpha2-adrenoceptor blockade with yohimbine did not induce any change in platelet alpha2-adrenoceptors.

    Notes: Summary We have studied the sympathetic response to blockade of presynaptic α2-adrenoceptors in essential hypertension by measuring plasma concentrations of noradrenaline after a single oral dose of yohimbine, an α2-adrenoceptor antagonist. Mean baseline plasma noradrenaline and adrenaline concentrations were similar in the hypertensive and normotensive groups. Yohimbine (0.2 mg×kg−1 orally) caused a lesser increase in the plasma concentrations of noradrenaline in hypertensive patients (+67%) than in normotensive subjects (+178%) and a pressor response in hypertensive (but not in normotensive) patients. These results are consistent with an alteration in the balance of α-adrenoceptors (for example presynaptic α2-adrenoceptor desensitization and post-synaptic α1-adrenoceptor hyper-responsiveness) which would help to develop and/or maintain arterial hypertension.

    Notes: Summary A one-year case-control study was performed by occupational physicians in 328 injured people and 662 controls in order to investigate a possible relationship between the consumption of benzodiazepines and the occurrence of industrial injuries. Benzodiazepine consumption was found to be related to sex (females) and age but not to a higher frequency of industrial injuries. It is suggested that knowledge of being at risk might be a factor reducing the consumption of benzodiazepines.

    Notes: Abstract Objectives: The incidence and the cost of adverse drug reactions (ADR) in patients treated by cancer chemotherapy were assessed using hospital database records from 1993 in a French regional cancer institute. Methods: Patients with ADRs were identified using a list of ICD-9 codes describing potential adverse events. Direct medical costs for treating these ADRs were assessed according to the hospital system of claims data. Results: Among the 3429 in-patients hospitalized in 1993, we found 171 patients (5% of the population) who presented at least one ADR (3.5% of the total number of hospital stays). A total of 313 ADRs occurred in 256 hospital stays (3.5% of the hospital stays in 1993). Of the patients with ADRs 60.2% were female and their mean age was 51.5 years; 106 patients presented with at least one “serious” ADR according to the WHO definition. These ADRs occurred during 130 hospitalizations. In 7 cases, ADRs led to death. There was no relationship between age or sex and the seriousness of the ADR. Of the ADRs 91% was type “A” (predictable). We estimated that the cost of “serious” ADRs was 1.8% of the global budget of the hospital. The average cost of ADRs leading to hospitalization was 33 037 French Francs at the current rate in 1993. This cost represented an additional cost of 32% of the overall cumulative yearly cost per patient in the institution. Conclusion: This study emphasizes the medical and economic impact of the management of ADR in anticancer treatments.
