Sommaire On a mené une étude sur l'usure des cavités cotyloîdes en P.T.F.E. (téflon) provoquées par des têtes de fémur en acier inoxydable de différentes tailles dans le cas d'un remplacement total de la hanche sur des sujets humains. Le défaut le plus important du Téflon était l'apparition d'une réaction tissulaire aux débris d'usure. Les articulations artificielles utilisant des cotyles en plastique doivent donc être étudiées de façon à produire un minimum de débris d'usure sur une période donnée. Plus le diamètre de la tête métallique est faible, moins il y a de débris d'usure, mais en-dessous d'une certaine dimension optimum, le taux de pénétration de la tête dans la cavité devient trop rapide. Les conclusions basées sur l'étude du comportement du téflon chez l'homme montrent que pour obtenir une durée de vie maximum de la cavité, la dimension optimum de la tête de fémur doit être égale à la moitié du diamètre extérieur de la cavité. On pense que ces observations peuvent être appliquées à d'autres matières plastiques que le téflon. Une observation clinique inattendue a révélé que l'usure d'un cotyle en téflon dépendait davantage de l'activité du sujet que de son poids.
Abstract:
Zusammenfassung Die Abnützung von Hüftgelenkspfannen aus PTFE bei verschiedenen Größen von Femurköpfen aus rostfreiem Stahl wurde für den vollständigen Hüftersatz beim Menschen untersucht. Das schwierigste Problem bei Mißerfolg mit PTFE war die Erzeugung einer Gewebsreaktion durch Abnützungsmaterial. Daher sollten künstliche Hüftgelenke mit Plastikgelenkpfannen so angelegt sein, daß in einer gegebenen Zeit ein minimales Volumen an Abnützungsmaterial entsteht. Je geringer der Durchmesser des Metallkopfes, um so kleiner ist das Volumen an Abnützungsmaterial. Unterhalb einer optimalen Größe wäre jedoch die Durchstoßung der Pfanne durch den kleinen Kopf zu schnell zu erwarten. Aus dem beobachteten Verhalten von PTFE im menschlichen Körper ist zu schließen, daß für größte Langlebigkeit der Pfanne die Optimalgröße des Femurkopfes die Hälfte des Außendurchmessers der Pfanne beträgt. Diese Beobachtungen können eventuell auch auf andere Plastikstoffe angewandt werden, welche geringere Abnützung zeigen als PTFE. Die klinische Praxis ergab die unerwartete Beobachtung, daß die Abnützung einer PTFE-Gelenkpfanne mehr von der Bewegungsaktivität des Patienten als von dem Körpergewicht abzuhängen schien.

Abtract A study has been made of the wear of hip sockets of P.T.F.E. against various sizes of stainless steel femoral head in the course of total prosthetic replacement of the hip in the human subject. The most serious aspect of the failure of P.T.F.E. was the production of a tissue reaction by wear debris. Artificial hip joints using plastic sockets should, therefore, be designed to produce the minimum volume of wear debris over a given period. The smaller the diameter of the metal head the smaller the volume of wear debris, but below a certain optimal size the rate of penetration of the socket by the small head would be too rapid. Conclusions based on the observed behaviour of P.T.F.E. in the human body indicate that for the longest life of a socket the optimum size of femoral head will be half that of the external diameter of the socket. It is considered that these observations can be applied to other plastics more resistant to wearing than P.T.F.E. An unexpected observation from clinical practice was that the wear of a P.T.F.E. socket seemed to be more dependent on the activity of the patient than the weight of the patient.
