    Notizen: Summary To determine if osmotic minipumps can be used for the local delivery of parathyroid hormone (PTH), we examined the bone resorbing activity of PTH in minipumps, either removed and assayed in bone organ cultures or released over rat calvaria. Biological activity of PTH was maintained for up to 6 days when the hormone solution contained serum and the minipumps and tubing were siliconized and flushed with diluent prior to use. Addition of cysteine did not enhance activity.

    Notizen: Summary Twenty-eight premature infants of mean gestation 30.9±2.5 weeks and mean birth weight 1175±206 g had repeated serum calcitonin concentrations determined over the first 12 weeks of life. Serum calcitonin concentrations slowly fell but remained elevated even at 12 weeks of age [normal adult=71±48, 1 week (N=15)=327±167, 3 week (N=23)=270±129, 6 week (N=16)=249±154, 9 week (N=13)=214±108, 12 week (N=12)=174±11]. Throughout this period, serum total calcium was normal or low (8.4±.8–9.3±1.0). Serum phosphorus was normal or low (6.0±1.4–6.5±1.0), and serum magnesium was normal (1.7±0.24–1.8±0.34). The reason for the sustained elevation of serum calcitonin in these very small, sick, premature infants in unclear.

    Notizen: Summary This work was conducted to estimate the replacement dose of the synthetic bovine parathyroid hormone [PTH(1–34)] that is required for maintenance of serum calcium (Ca) in parathyroidectomized (PTX) rats. Male rats were PTX and used in this study only if serum Ca was reduced to at least 7 mg/dl. We found that a solution of 2% cysteine, 150 mM NaCl, and 1 mM HCl was superior to 20 mM acetic acid for maintenance of biological activity of PTH (1–34) in situ during the period of hormone infusion studied. The PTH dose—calcemic response relationship was investigated using PTH in doses of 0.6, 1, and 3 U/h. The infusion of 1 U PTH per hour raised Ca to the normal level, whereas rats infused with 0.6 U/h were hypocalcemic and 3 U/h resulted in marked hypercalcemia. To extend this observation we carried out an infusion of 1 U PTH per hour for 14 days. We found that this infusion rate of bovine PTH (1–34) provided a relatively stable level of serum calcium with modest fluctuation from normocalcemic to somewhat hypercalcemic levels for the entire 14-day period of PTH infusion. Serum calcitonin was also elevated during the infusion period and then returned to the initial level when PTH treatment was stopped. After the minipumps containing PTH were removed, the serum Ca dropped rapidly to 5 mg/dl, which was significantly lower than the control (vehicle-infused) or initial values of serum Ca (7 mg/dl). Infusion of PTH at 3 U/h for 4 days did not produce this rebound hypocalcemia after the pumps were removed. Serum Ca in those experiments returned to the initial level after hormone treatment was discontinued.

P.A.T. measurements were found to be very effective for this purpose. We propose that the observed effects are due to decoration of the void surface with nitrogen and vacancy loop decoration with hydrogen and nitrogen.

    Notizen: Abstract Positron annihilation measurements of the coincidence count rate at the peak of the angular correlation curve (CCR) have been performed as a function of temperature for the alloy systemFeTi in a concentration range up to about 1.3 at. %. The concentration dependence of the effective vacancy formation enthalpy (H 1V F )eff suggests the existence of an attractive interaction between vacancies and the impurity atoms. It will be shown that the description of the vacancy concentration in an alloy according to the Lomer model is not valid in this case, because it neglects binding of vacancies to solute atom pairs and to higher agglomerates. The application of a model proposed by Dorn and Mitchell gives evidence that beyond the binding of a vacancy to a single solute atom in the concentration range investigated also aggregates of a vacancy bound to two and three foreign atoms must be taken into account. The analysis of the measurements according to the method of Hehenkamp and Sander gives values for the respective vacancy-solute-atom(s) binding enthalpies H 1 B =0.25eV, H 2 B =0.53eV and H 3 B =0.91eV. The discussion of the temperature and solute-atoms concentration dependence of the vacancy concentration suggests a much more complicated behaviour than for pure metals even at low solute-atom concentrations.

    Notizen: Abstract The mixed ferrites Cu1+x Ti x Fe2−2x O4 with x=0, 0.1, 0.2, 0.3 and 0.4 were prepared by standard ceramic techniques. Both structural and IR investigations were carried out. The X-ray diffraction pattern showed that the prepared samples are pure spinels. It is also indicated that the system exists in two crystallographic forms, the tetragonal structure (for x=0, 0.1) and the cubic structure (for the rest of x). Some physical properties, such as density, porosity and shrinkage were also determined throughout this study. The IR spectra revealed two bands at 575 cm−1 and 400 cm−1. These bands were assigned as v 1 and v 2, respectively. v 1 and v 2 are associated with the vibrations of the tetrahedral and octahedral groups, respectively. The cation distribution (Cu t 2+ Fe 1−t 3+ ) [Cu 1+x−t 2+ Ti x 4+ −Fe 1−2x+t 3+ ]O4 is proposed and used to interpret and explain the obtained results.

    Notizen: Abstract Positron lifetime measurements were performed on amorphous Pd80Si20 and Cu50Ti50 alloys irradiated with 3MeV electrons at 20K. The irradiation was found to increase the mean positron lifetime in both specimens indicating the presence of vacancy-like radiation damage. Isochronal annealing between 77 K and 300 K resulted in a continuous reduction of the positron lifetime, which suggests a gradual recovery of the irradiation induced defects.

    Notizen: Abstract Numerical results are presented for a one-dimensional diffusion model for freely diffusing positrons in thin films with an initial exponential positron profile. The results are for two boundary conditions which are experimentally realizable. Since thermalized positrons generally obey this diffusion model, it should be possible to extract the bulk diffusion constant by fitting experimental measurements to the model's predictions. Recently, a scheme has been proposed for the brightness enhancement of slow positron beams. We discuss experimental methods for implementing this scheme and the various problems that may arise.

    Notizen: Abstract Positron annihilation and Hall effect inn-InP crystals as a function of electron irradiation up to 1 · 1019 cm−2 and post-irradiated isochronal annealing up to 550 °C have been studied. It is concluded that in irradiatedn-InP samples positrons interact with negatively charged acceptor-type defect with level atE c −0.33 eV, probablyV In (primary defect). In post-irradiated isochronal annealed (up to 330 °C) samples ofn-InP positron trapping occurs preferably in secondary defects-vacancy clusters, which are formed in the temperature range (150–300 °C). Inn-InP crystals containing radiation induced defects the trapping rate was found to decrease with temperature in the range (300–77) K.

    Notizen: Abstract Trapping of positrons at vacancy-type defects in magnesium was studied by positron lifetime and Doppler-broadening measurements. Vacancy defects were produced by quenching, electron irradiation and deformation at low temperatures as well as by thermal agitation at elevated temperatures. In the first three cases we observed trapping at multiple vacancies, which anneal out between 77...400 K. Thermal equilibrium measurements show S-shape behaviour originating from positron trapping at magnesium monovacancies. However, changes in the positron parameters were very small, which is due to the weakness of the positron-vacancy interaction. A detrapping analysis yielded a positron-vacancy binding energy of the order of 0.3...0.4 eV.

    Notizen: Abstract A simple model is developed for the trapping of positrons at grain boundaries. It is shown that there is a linear relationship between any linear annihilation parameter and the inverse grain size. An effective grain boundary width is defined, which depends on the positron diffusion length and on the strength of the grain boundary for positrons. The effect of detrapping on this effective width is also considered. The model is tested by using the experimental results available in the literature.

    Notizen: Summary Parathyroid hormone (PTH) metabolism in pregnancy has not been clearly defined. Studies have reported either increased or unchanged values of immunoreactive PTH (iPTH). However, iPTH levels do not necessarily correlate with hormonal bioactivity due to the presence of immunoreactive but nonbioactive PTH fragments. In this study we evaluated PTH metabolism in the third trimester of pregnancy by determining iPTH blood levels as well as the biological effects of PTH, assessed by tubular maximum phosphate reabsorption (TmP) and nephrogenous cAMP (ncAMP) excretion, in 10 young, healthy pregnant patients (mean gestational age 35 weeks) and 10 young, healthy age-matched female controls. Pregnancy was associated with a significant increase in creatinine clearance (146±13 vs 106±9 ml/min,P〈0.01), and a significant decrease in total fasting serum calcium (8.4±0.1 vs 9.0±0.1 mg/dl,P〈0.001) and serum albumin (3.6±0.1 vs 4.2±0.1 g/dl,P〈0.001). There was no significant difference in iPTH (3.7±0.4 vs 4.3±0.5 µlEq/ml), serum phosphorus (3.6±0.1 vs 3.8±0.2 mg/dl), TmP (3.61±0.13 vs 3.75±0.25 mg/100 ml GFR), or ncAMP (1.68±0.20 vs 1.88±0.23 nmoles/100 ml GFR) between the two groups. Pregnancy was attended by a significant increase in fasting urinary calcium to creatinine ratio (0.14±0.03 vs 0.06±0.01,P〈0.05), an index of bone resorption. The data suggest that the biological effects of PTH are unchanged in pregnancy, and that reported increments in 1,25(OH)2 vitamin D in pregnancy are not regulated by changes in either PTH, calcium, or phosphate.

    Notizen: Summary It has recently been demonstrated that calcium ionophore A23187 mimics certain of the effects of parathyroid hormone (PTH) on bone in vitro, including stimulation of45Ca release and cAMP formation. To further examine the relative effects of these two agents on bone cell metabolism, we compared the effects of synthetic PTH 1–34 (50 ng/ml) and calcium ionophore A23187 (0.5µg/ml) on45Ca release, DNA concentration, and nucleic acid synthesis in fetal rat forelimb rudiments cultured for periods up to 120 h. Both agents stimulated45Ca release; however, the effects of PTH were apparent after a shorter period of exposure. Bone DNA concentration (expressed asµg DNA/mg bone) was not affected by PTH but was significantly increased relative to control values by exposure to A23187 for 8–120 h of incubation. PTH increased the incorporation of3H-thymidine into DNA at 30 and 48 h, and increased the incorporation of14C-uridine into RNA at 48 h, time points which corresponded to a period of accelerated PTH stimulation of45Ca release. In contrast,3H-thymidine and14C-uridine incorporation were both uniformly suppressed by A23187 at all time points examined. Thus the increased DNA concentration observed in A23187-treated rudiments appeared to be the result of a decreased rate of bone maturation and mineralization. The markedly different patterns of nucleic acid synthesis in response to PTH and A23187 suggest that these agents differ significantly in their mechanisms of action on bone cell metabolism.

    Notizen: Abstract Both PGE2 and PTH (1-34) caused a time- and concentration-dependent stimulation of proliferation by embryonic chick periosteal cells. Cells were exposed to the agents for different periods of time, the medium was replaced with fresh medium, and 3H-TdR incorporation was measured after 16 hours. Challenge with 10-6 M prostaglandin E2 (PGE2) or 10-7 M parathyroid hormone (1-34) (PTH) for 5 minutes produced 4- and 5.5-fold increases in 3H-TdR incorporation, respectively. Longer exposures, however, produced diminishing responses and after 45 minutes, only minimal effects or slight inhibitions were seen. These timedependent effects were also seen with forskolin and dibutyryl-cAMP; TPA on the other hand stimulated DNA synthesis after both short- and long-term exposure. Both PGE2 and PTH (1-34) stimulated cAMP synthesis in periosteal cells but neither could be shown to stimulate protein kinase-C (PKC) at concentrations required for stimulation of proliferation, and dibutyryl-cyclic AMP (cAMP) effectively inhibited endogenous PKC activity. It is possible that the stimulation of proliferation by short-term exposure to PGE2 and PTH (1-34) is mediated by cAMP and that the time dependency possibly stems from the inhibition of endogenous PKC activity by increased intracellular cAMP levels.

    Notizen: Abstract Islet amyloid polypeptide (IAPP) is a member of the calcitonin/CGRP family and has been isolated from the β-cell of pancreatic islets. Recent evidence suggests that this peptide may be involved in calcium metabolism in that its administration resulted in lowering of serum calcium levels. To determine the mechanism of IAPP-induced hypocalcemia, the peptide was infused at 50 pmol/min/kg for 90 minutes in conscious male mongrel dogs. Infusion of the peptide resulted in a modest decline in the total serum calcium concentration (10.4±0.2 to 9.4±0.2 mg/dl; P〈0.05) and a concomitant increase in urinary calcium excretion (3.6±0.6 to 6.9±2.0 mg/dl; P〈0.01). Based on an extracellular volume of 7 liter in a 28 kg dog, the total decrement in calcium due to IAPP was 41.3±2.4 mg, whereas the total increase in urinary calcium was 3.2±0.7 mg. There were no detectable changes in calcitonin. We conclude that IAPP lowers serum calcium and increases the renal excretion of calcium independently of calcitonin. However, the calciuria can only account for a small component of the hypocalcemic effect and therefore, an additional calcium lowering effect of IAPP exits.

    Notizen: Abstract There is an interplay between the cells in the bone marrow and the surrounding bone tissue, but little is known about the effects of myeloablative treatment followed by bone marrow transplantation on bone metabolism. We have therefore investigated 24 patients undergoing bone marrow transplantation (14 autologous, 10 allogeneic) for hematological malignancies. Serum concentrations of parathyroid hormone (PTH), albumin-modified calcium, and biomarkers for bone turnover-osteocalcin, bone alkaline phosphatase (B-ALP), and carboxyterminal cross-linked telopeptide of type I collagen (ICTP)-were measured. The samples were collected before myeloablative treatment, on the day of bone marrow infusion and 1, 2, 3, and 12 weeks thereafter. A serum PTH peak was consistently seen the day after total body irradiation, but no long-term effects on PTH/calcium homeostasis were observed. Bone formation as reflected by serum osteocalcin and B-ALP decreased, with nadir levels 2 to 3 weeks after marrow infusion. A simultaneous increase in bone resorption (increased S-ICTP) occurred. Pretreatment values were not completely regained 12 weeks after transplantation. the findings indicate that bone tissue is affected by myeloablative treatment, and the changes in biomarkers imply a net loss of bone over the study period.

    Notizen: Summary The actions of divalent cation ionophores on bone resorption in vitro are complex; both enhancement of resorption and inhibition of stimulated resorption have been observed with the ionophore A23187. We have found in neonatal mouse calvaria, in which divalent ionophores were only inhibitory, that monovalent cation ionophores were even more potent inhibitors of stimulated bone resorption. Nigericin, monensin, and X206 each inhibited the release of calcium (Ca) from calvaria that were stimulated to resorb by 0.1 U/ml parathyroid hormone (PTH). Actions of the three ionophores were dose dependent and were maximal at 10−7M, 3×10−7M, and 1.2×10−7M, respectively, compared to A23187, which was maximally inhibitory at 2×10−6M. After pretreatment with nigericin alone or together with PTH for 24 h, inhibition of stimulated resorption was partially reversible. Prolonged (48 h) treatment, either with ionophore alone or together with PTH, caused irreversible inhibition of stimulated Ca release. However, the ionophore was only partially inhibitory if it was added to calvaria stimulated by pretreatment with PTH alone for 24 or 48 h. Resorption stimulated by prostaglandin E2, 1,25-dihydroxyvitamin D3, and epidermal growth factor was also inhibited by monovalent ionophores, indicating that the inhibition was not at the level of the PTH receptor. In addition, ionophores did not lower basal or PTH-stimulated production of cyclic AMP by calvaria. Submaximal doses of nigericin were synergistic with calcitonin or ouabain in inhibiting PTH-stimulated resorption. These results are consistent with the hypothesis that stimulated release of Ca from bone occurs by a Na/Ca exchange mechanism. Thus monovalent cation ionophores would increase intracellular Na+, thereby decreasing the Na gradient across bone cell membranes, leading to conditions unfavorable for Ca efflux coupled to further Na influx.

    Notizen: Summary The present study was undertaken to test the in vitro action of aluminum on bone phosphatase activities and the possible interaction of this metal with parathyroid hormone (bPTH) or vitamin D3 dihydroxymetabolites [1,25- and 24,25(OH)2D3). Three-day-old rat calvaria were incubated for 24 h with one of the following: bPTH at 5×10−8M, 1,25-or 24,25(OH)2D3 at 2.5×10−9M, Al at concentrations ranging from 3×10−11M to 6×10−6M, or their corresponding solvents. Al effects were also investigated when the medium phosphate or calcium concentrations were modified. In some experiments, Al was added simultaneously with bPTH or one of the vitamin D3 metabolites at the beginning of the 24 h incubation. At the end of all incubations, acid and alkaline phosphatase activities were measured in bone cytoplasmic extract. The results show that: (a) When compared to the value found in half calvaria incubated in a control medium, the bone acid and alkaline phosphatase content is significantly higher in paired halves incubated with Al (3×10−11M to 1.5×10−6M) as well as with bPTH, 1,25-, or 24,25(OH)2D3 and sharply decreased with higher Al concentrations (6×10−6M). (b) The Al effect on phosphatase activities is modified in a free phosphate or a free calcium medium. (c) The presence of Al at 1.5×10−6M or 6×10−6M significantly decreases the bPTH or 1,25(OH)2D3-induced stimulation of bone phosphatase activities. (d) A similar interaction could not be found between Al and 24,25-(OH)2D3.

    Notizen: Summary The effect of ovarian insufficiency on calcium metabolism has been thought to involve an increased bone resorptive effect of parathyroid hormone and possible impaired synthesis of 1,25-dihydroxyvitamin D3. In the present study a rat model allowing for controlled serum levels of parathyroid hormone and 1,25-dihydroxyvitamin D3 was used. Oophorectomy in this species is associated with increased serum levels of 1,25-dihydroxyvitamin D3 and decreased bone mass. Although thyroparathyroidectomy increased bone mass, an increased sensitivity of bone to parathyroid hormone in oophorectomized rats was not observed. Thus the development of the osteopenia did not seem to be related to increased parathyroid hormone sensitivity or to reduced levels of 1,25-dihydroxyvitamin D3. Exogenous 1,25-dihydroxyvitamin D3 increased bone mass in oophorectomized as well as intact rats. Intestinal calcium transport was increased by moderate doses of 1,25-dihydroxyvitamin D3. Intestinal calcium transport was also reduced by thyroparathyroidectomy and increased by the administration of parathyroid extract. A tendency for increased accumulation of 1,25-dihydroxyvitamin D3 in blood in oophorectomized rats has been noted. It is suggested that the tendency to hypercalcemia in ovarian-insufficient females given 1-hydroxylated vitamin D compounds may be related to a diminished metabolism of 1,25-dihydroxyvitamin D3.

    Notizen: Abstract The role of calcitonin and parathyroid hormone (PTH) in corticosteroid-induced osteoporosis is controversial. We therefore measured plasma calcitonin and PTH levels in 34 adults receiving chronic pharmacological corticosteroids for obstructive airways disease, and in controls matched for age, sex, menopause, and disease. In addition, the acute effect of a 7-day course of 15 mg prednisolone daily on fasting and calcium-stimulated calcitonin was studied in 10 normal male volunteers. There was no difference in calcitonin and PTH levels in the corticosteroid-treated patients when compared with controls. The corrected serum calcium was significantly higher in the steroid-treated patients (patients mean 2.40 (SEM 0.01) mmol/liter; controls mean 2.33 (SEM 0.01) mmol/liter; P〈0.001). The short course of corticosteroids in volunteers did not alter basal or stimulated calcitonin, PTH, or calcium levels. These results suggest that neither calcitonin deficiency nor PTH excess is a feature of corticosteroid-induced osteoporosis.

    Notizen: Abstract Both interleukin-1α (IL-1α) and interleukin-1β (IL-1β) are powerful stimulators of bone resorption in vivo and in vitro. Interleukin-1 receptor antagonist (IL-1ra) binds to many interleukin-1 receptors. It does not activate the receptor and effectively blocks the action of IL-1α and IL-1β. In this study, human recombinant IL-1ra, at 100-fold excess, was found to block bone resorption in cultured mouse calvaria due to IL-1β but not IL-1α. These observations may be explained by differential affinities of receptors for IL-1α, IL-1β and rhIL-1ra on target bone cells.

    Notizen: Summary The effect of parathyroid hormone (PTH 1–34 bovine) on alkaline phosphatase activity was investigated in an osteoblast-like clonal cell line derived from rat osteosarcoma (ROS 17/2). ROS 17/2 alkaline phosphatase resembled the bone enzyme in levamisole sensitivity and electrophoretic mobility but differed in heat stability. The specific activity of ROS 17/2 alkaline phosphatase increased with time in culture. This increase was inhibited by PTH (1–34) and (-)-isoproterenol in a dose-dependent manner starting at near-physiological hormone concentrations. The ID50 values were 0.02 nM for PTH (1–34) and 1.7 nM for isoproterenol. The two hormones stimulated ROS 17/2 adenylate cyclase, albeit at higher concentrations: Km values were 13 nM for PTH (1–34) and 16 nM for isoproterenol. The rise in alkaline phosphatase was also inhibited by dibutyryl cyclic AMP and 8-bromocyclic AMP (0.1 mM). These findings further document the osteoblastic properties of the ROS 17/2 osteosarcoma cell line, suggest that PTH inhibition of alkaline phosphatase represents a physiological response to the hormone in these cells, and implicate cyclic AMP as a mediator of this PTH effect.

    Notizen: Summary Calcitonin (CT) inhibits hormonally stimulated bone resorption only transiently in vitro. This phenomenon has been termed “escape,” but the mechanism for the effect is not understood. One possible explanation is that bone cell differentiation and recruitment of specific precursor cells, in response to stimulators of resorption, lead to the appearance of osteoclasts that are unresponsive to CT. To test this hypothesis, cell proliferation in neonatal mouse calvaria in organ culture was inhibited by irradiation from a cobalt-60 source. At a dose of 6000 R, [3H]thymidine incorporation into intact calvaria was inhibited approximately 90%. Irradiation had no effect on the resorptive response to 0.1 U/ml parathyroid hormone (PTH). However, irradiation induced a dose-dependent inhibition of the escape response which was maximal at 6000 R. A dose of 6000 R did not affect the binding of125I-salmon CT to calvaria and decreased PTH stimulation of cyclic AMP release from bone without affecting the cyclic AMP response to CT. Although irradiation caused a dose-dependent inhibition of DNA synthesis, the dose-response curves for that effect and inhibition of escape were not superimposable. A morphologic study of hormonally treated calvaria demonstrated that irradiation prevented the early increase in number of osteoclasts in PTH-treated calvaria that had been observed previously in unirradiated bones. Autoradiography showed that irradiation also prevented the PTH-stimulated recruitment of newly divided mononuclear cell precursors into osteoclasts. This may be correlated with the effect of irradiation to prevent the loss of responsiveness to CT in the presence of PTH.

    Notizen: Summary In 10 patients with Paget's disease of bone and 2 patients with osteoporosis, we studied the effects of hypocalcemia and hypophosphatemia induced by disodium-(3-amino-1-hydroxypropylidene)-1,-bisphosphonate (APD) treatment on the serum concentration of PTH and 1,25-dihydroxyvitamin D [1,25(OH)2D3] and on calcium absorption and balance. The fall in serum calcium and phosphate was associated with a rise in the serum concentration of PTH and 1,25(OH)2D3, coupled with increases in net calcium absorption and calcium balance. The concentration of 1,25(OH)2D3 was significantly related (P〈0.001) to the serum calcium (r=0.66), the serum phosphate (r=0.78), and the serum PTH (r=0.71), confirming the interrelated control of these parameters on 1,25(OH)2D3 production. Moreover, the rise in 1,25(OH)2D3 caused an appropriate rise in calcium absorption (r=0.74) and calcium balance (r=0.86), showing that this vitamin D metabolite contributes as a hormone to calcium homeostasis.

    Notizen: Summary The release of cyclic AMP from bone in response to stimulation with PTH 1–34 was examined in 20 dogs with long-term chronic renal failure (CRF) produced by unilateral nephrectomy and contralateral partial renal artery ligation. After 9 to 15 months of uremia, the tibiae were removed and perfused in vitro. Seven dogs with CRF served as controls, 7 dogs with CRF were treated with 24,25(OH)2D3 — 2.5 µg per day, and 6 CRF dogs underwent thyroparathyroidectomy (TPTX) 42 h before they were sacrificed. The release of cyclic AMP from bone in response to PTH 1–34 in the CRF dogs was severely reduced compared to the response observed in 7 dogs with normal renal function (net accumulation of cyclic AMP release 86±8.5 versus 426±59.0 pmol/30 min). Long-term treatment of uremic dogs with 24,25(OH)2D3 had no effect on the release of cyclic AMP by bone. However, the release of cyclic AMP was restored to normal levels in the CRF dogs that underwent thyroparathyroidectomy. All CRF dogs had secondary hyperparathyroidism and the fact that TPTX returned the cyclic AMP response to normal values suggests that desensitization to PTH of the adenylate cyclase system of bone exists in chronic uremia.

    Notizen: Abstract Urinary excretion of N-acetyl-β-D-glucosaminidase (NAG) transiently increases after PTH(1–34) infusion in idiopathic hypoparathyroidism (IHP) but this response is impaired in pseudohypoparathyroidism (PHP) type I. We investigated the effects of endogenous PTH, exogenous calcitonin (CT), and dibutyryl cAMP (DBcAMP) on urinary excretion of NAG. Urinary NAG excretion in 14 patients with primary hyperparathyroidism (1oHPT) was more than in normal subjects (P〈0.001) and decreased after parathyroidectomy (P〈0.01). Urinary NAG excretion increased after the infusion of 1.5 MRC/kg of eel CT in eight normal subjects (P〈0.001), two patients with IHP, and a patient with PHP type Ib but not in a patient with PHP type Ia. The increases of urinary NAG excretion by CT and by PTH(1–34) were positively correlated with the increases of urinary cAMP excretion (r=0.752; P〈0.001 and r=0.534; P〈0.002, respectively). Urinary NAG excretion increased after DBcAMP infusion in five normal subjects (P〈0.01), two patients with IHP, and two with PHP type I. The increase of urinary NAG by 6.0 mg/kg of DBcAMP was more than by 2.5 mg/kg of DBcAMP in normal subjects (P〈0.01). The increase of urinary NAG by 2.5 mg/kg of DBcAMP in PHP type I was comparable with that by 6.0 mg/kg in normal subjects, suggesting a hyperresponsiveness to DBcAMP in PHP type I. Urinary excretion of NAG is a useful indicator of renal tubular responsiveness to PTH and CT. Cyclic AMP-dependent mechanism is probably involved in PTH and CT-induced increase in urinary excretion of NAG.

    Notizen: Abstract We compared the effect of orally administered 100 mg of hydrochlorothiazide (HCTZ) among eight patients with pseudohypoparathyroidism (PHP) type I, 11 patients with idiopathic hypoparathyroidism (IHP), and 12 patients with primary hyperparathyroidism (1oHPT). Patients with PHP type I or with IHP were studied during the treatment with 1α-hydroxylated metabolites of vitamin D3. HCTZ raised serum levels of calcium (Ca) in 1oHPT (P〈0.001) and PHP type I (P〈0.01) but did not increase urinary excretion of Ca. Serum parathyroid hormone (PTH) in PHP type I decreased (P〈0.02) after HCTZ administration in response to the increase in serum Ca. HCTZ did not raise serum levels of Ca in IHP but increased urinary excretion of Ca in this group (P〈0.01). HCTZ suppressed tubular reabsorption of phosphate (P) in IHP (P〈0.01) and 1oHPT (P〈0.05) but not in PHP type I. Urinary excretion of cAMP did not change after HCTZ administration in PHP type I, IHP, or 1oHPT. Endogenous PTH modulated the effects of HCTZ on Ca mobilization from bone and renal reabsorption of Ca in PHP type I with normal or high serum levels of PTH and in 1oHPT with high serum levels of PTH. The inhibitory effect of HCTZ on renal tubular reabsorption of P (probably from proximal tubules) was independent of PTH. The resistance to this inhibitory effect of HCTZ on P reabsorption in PHP type I suggested a proximal tubular dysfunction in this disorder.

    Notizen: Abstract The cytokine interleukin-6 (IL-6) was produced by neonatal mouse parietal bones during a 6- or 48-hour culture period in response to prostaglandin E2 (PGE2) and bovine parathyroid hormone (PTH) 1-34 fragment but not 1,25-dihydroxyvitamin D3 [1,25(OH)2D3]. At the same time there was an increase in tartrate-resistant, acid phosphatasepositive osteoclasts (TRAP+OC) with all three osteotropic effectors over 6 hours, and an increase in 45Ca release over 48 hours. TRAP+OC numbers on PGE2-stimulated bones were positively correlated with IL-6 concentration. Our aim was to determine if IL-6 mediated this response. Recombinant human IL-6 (rhIL-6) was added to parietal bones in culture at concentrations within the range that PGE2 or PTH would produce during incubation. However, over 6 or 48 hours, rhIL-6 did not stimulate TRAP+OC to increase in number nor did it cause an increase in calcium release over 48 hours. Adding an antibody against mouse IL-6 to bone cultures stimulated with PTH or PGE2 neutralized the resulting IL-6 bioactivity by up to 92% but did not inhibit TRAP+OC formation. We conclude that although IL-6 is produced in response to two important stimulators of bone resorption, it does not mediate osteoclast differentiation or bone resorption in this model.
