    Description: The diagnosis of gastrointestinal graft versus host disease (GI-GVHD) is based on clinical symptoms and histological findings. In clinical practice, it is often difficult to decide whether abdominal symptoms in an allogeneic transplant recipient are caused by GVHD or other disorders. Endoscopic biopsies are helpful in establishing the diagnosis, but endoscopy is not always possible to perform due to poor general condition of the patients. No biomarkers are routinely used to predict GVHD. The aim of fecal calprotectin and alpha-1 antitrypsin testing in our study was to find out whether determination of the concentrations of these proteins may be used as a screening method for enteric GVHD. We studied prospectively 51 patients, 8 of whom developed GI-GVHD. Our data demonstrate that elevated fecal calprotectin levels were significantly associated with presence of GI-GVHD. We found a positive association between high F-calprotectin and severe gastrointestinal GVHD. In bivariate analysis, only calprotectin but not alpha-1 antitrypsin was independently associated with GI-GVHD. Testing for fecal calprotectin after allogeneic stem cell transplantation may be a useful screening tool. Scientific Reports 5 doi: 10.1038/srep07920

    Description: Background: Iron overload is common after allogeneic hematopoietic stem cell transplantation and may cause liver damage. High pre-transplantation serum ferritin levels have been reported to be associated with impaired survival post transplantation in patients with acute myeloid leukemia or myelodysplastic syndrome. Design and Methods: We performed a retrospective study of 309 adult patients with haematological malignancies who underwent allogeneic hematopoietic stem cell transplantation after myeloablative (51%) or non-myeloablative conditioning (49%). The aim was to determine the incidence of hyperferritinemia and iron overload and the effects of these conditions on graft versus host disease, relapse and survival in patients with blood disorders who receive allogeneic stem cell grafts. Results: For overall survival and for relapse-free survival, only high pretransplantation ferritin (〉400ug/l) and advanced stage of disease were significant in multivariable analysis. High pretransplantation ferritin and advanced stage of disease were also significant for relapse risk at 3 years, but ferritin was not significant for non-relapse mortality. In multivariate analysis, ferritin, age, type of donor, and previous transplantation were significant factors for cGVHD at one year. Conclusions: Hyperferritinemia was associated with lower risk of cGVHD and increased risk of relapse and death, but not with increased non-relapse mortality. The lower rate of cGVHD among patients with high ferritin levels can be explained by an immunosuppressive effect of ferritin, reducing graft versus host disease and graft versus leukaemia effect, resulting in increased risk of relapse and impaired chance of survival. Therefore, these conditions might be best explained by an immunosuppressive effect of ferritin and not by a direct toxic effect.

    Description: An object-based method for automatic iceberg detection from Advanced Synthetic Aperture Radar (ASAR) images has been developed and applied in the Amundsen Sea, Antarctica. The automatic identification is based on brightness and spatial parameters of the ASAR images at five scale levels, and was verified with manual classification in four areas chosen to represent varying environmental conditions. The presented algorithm works comparatively well with images of the ocean in freezing temperatures and strong wind conditions, common in the Amundsen Sea. The detection rate was 96.2% which corresponds to 93.2% of the icebergs area, for all seasons. The algorithm generated 3.8% errors in the form of 'misses' and 7.0% of 'false alarms', mainly caused by the presence of ice floes. The method was applied on 432 radar images acquired in 2011 under different meteorological, oceanographic and sea ice conditions. As an output a map showing the probability of finding icebergs has been created. It shows that high probability coincides with depth contours and indicates a westward drift of the bergs throughout the whole region.

    Notes: Summary The influence of food intake on the biovailability of dicoumarol from a non-micronized formulation was examined in ten healthy volunteers, by examination of its single-dose kinetics after ingestion of dicoumarol 250 mg with a standardized breakfast and on an empty stomach. Blood samples were collected at regular intervals from 0 to 72 h, and the serum concentration of unmetabolized dicoumarol was determinded by spectrophotometry. Postprandial AUC (area under the curve) values were significantly (p〈0.01) greater than the preprandial figures, the mean increase being 85 per cent. The results suggest that dicoumarol should always be taken with food.

    Notes: Summary The influence of food intake on the absorption of phenytoin was examined in eight healthy volunteers, by study of single-dose kinetics following ingestion of phenytoin 300 mg either with a standardized breakfast or on an empty stomach. Blood samples were collected at regular intervals from 0 to 48 h, and serum concentrations of unmetabolized phenytoin were determined by gas chromatography. Serum concentrations of the major metabolite of phenytoin, 4-hydroxyphenytoin, were measured by mass fragmentography. Concurrent intake of food and phenytoin appeared to accelerate absorption of the drug from the formulation used, and the peak concentrations were significantly higher (mean increase 40%) in the postprandial than in the preprandial state. As reflected by the AUC (area under the curve), the amount of drug absorbed was increased during postprandial conditions, although the difference only reached borderline significance. It is suggested that phenytoin should always be taken in a defined relation to meals.

    Notes: Summary In an attempt to assess whether intake of glibenclamide before meals would improve its therapeutic capacity, the present investigation compared the effect of glibenclamide 2.5mg t.i.d. given before and together with meals. In addition, these effects were compared with that of glibenclamide given as a single morning dose of 7.5mg. The subjects studied were six Type 2 diabetics not previously exposed to sulphonylurea drugs. Irrespective of dosage and mode of administration, addition of glibenclamide to a standardized breakfast, lunch and dinner enhanced plasma IRI concentrations and reduced blood glucose concentrations as compared to administration of meals without the drug. The different modes of glibenclamide administration did not differ significantly with respect to IRI responses. However, the blood glucose reduction after breakfast was significantly greater when glibenclaimde 2.5mg had been given before the meal than when 2.5 or 7.5mg were given with the meal; a similar, but non-significant tendency was observed after lunch; no consistent difference was seen after dinner. Food intake did not affect glibenclamide kinetics. It appears that administration of glibenclamide 2.5mg before breakfast improved glucose utilization following the breakfast load, due to earlier attainment of an effective concentration of glibenclamide.

    Notes: Summary The concentrations of “real” and “apparent” (= “real” hydralazine + acid-labile hydrazones) hydralazine in maternal and umbilical plasma obtained at delivery of 6 women treated with hydralazine and atenolol for pregnancy hypertension were measured by gas chromatography. In one of the patients, the concentrations of the same substances were subsequently measured in breast milk. “Apparent” hydralazine reached higher levels in umbilical than in maternal blood. The concentration of “real” hydralazine seemed to be at least as high in the fetus as in the mother. On the other hand, even though the fraction of “real” (i.e. presumably active) hydralazine was greater in milk than in plasma, the total concentration was smaller, and the estimated dose per milk feed of 75ml would not exceed 0.013mg. Thus, hydralazine treatment of the pregnant woman would expose her fetus to effective concentrations of the drug, but breast feeding would not result in a clinically relevant concentration in the infant.
