    Description: OBJECTIVES: To develop and validate an automated noninvasive method to quantify mitral regurgitation. BACKGROUND: Automated cardiac output measurement (ACM), which integrates digital color Doppler velocities in space and in time, has been validated for the left ventricular (LV) outflow tract but has not been tested for the LV inflow tract or to assess mitral regurgitation (MR). METHODS: First, to validate ACM against a gold standard (ultrasonic flow meter), 8 dogs were studied at 40 different stages of cardiac output (CO). Second, to compare ACM to the LV outflow (ACMa) and inflow (ACMm) tracts, 50 normal volunteers without MR or aortic regurgitation (44+/-5 years, 31 male) were studied. Third, to compare ACM with the standard pulsed Doppler-two-dimensional echocardiographic (PD-2D) method for quantification of MR, 51 patients (61+/-14 years, 30 male) with MR were studied. RESULTS: In the canine studies, CO by ACM (1.32+/-0.3 liter/min, y) and flow meter (1.35+/-0.3 liter/min, x) showed good correlation (r=0.95, y=0.89x+0.11) and agreement (deltaCO(y-x)=0.03+/-0.08 [mean+/-SD] liter/min). In the normal subjects, CO measured by ACMm agreed with CO by ACMa (r=0.90, p 〈 0.0001, deltaCO=-0.09+/-0.42 liter/min), PD (r=0.87, p 〈 0.0001, deltaCO=0.12+/-0.49 liter/min) and 2D (r=0.84, p 〈 0.0001, deltaCO=-0.16+/-0.48 liter/min). In the patients, mitral regurgitant volume (MRV) by ACMm-ACMa agreed with PD-2D (r= 0.88, y=0.88x+6.6, p 〈 0.0001, deltaMRV=2.68+/-9.7 ml). CONCLUSIONS: We determined that ACM is a feasible new method for quantifying LV outflow and inflow volume to measure MRV and that ACM automatically performs calculations that are equivalent to more time-consuming Doppler and 2D measurements. Additionally, ACM should improve MR quantification in routine clinical practice.

    Description: BACKGROUND: Cardiac papillary fibroelastoma (CPF) is a primary cardiac neoplasm that is increasingly detected by echocardiography. The clinical manifestations of this entity are not well described. METHODS AND RESULTS: In a 16-year period, we identified patients with CPF from our pathology and echocardiography databases. A total of 162 patients had pathologically confirmed CPF. Echocardiography was performed in 141 patients with 158 CPFs, and 48 patients had CPFs that were not visible by echocardiography (〈0.2 cm), leaving an echocardiographic subgroup of 93 patients with 110 CPFs. An additional 45 patients with a presumed diagnosis of CPF were identified. The mean age of the patients was 60+/-16 years of age, and 46.1% were male. Echocardiographically, the mean size of the CPFs was 9+/-4.6 mm; 82.7% occurred on valves (aortic more than mitral), 43.6% were mobile, and 91.4% were single. During a follow-up period of 11+/-22 months, 23 of 26 patients with a prospective diagnosis of CPF that was confirmed by pathological examination had symptoms that could be attributable to embolization. In the group of 45 patients with a presumed diagnosis of CPF, 3 patients had symptoms that were likely due to embolization (incidence, 6.6%) during a follow-up period of 552+/-706 days. CONCLUSIONS: CPFs are generally small and single, occur most often on valvular surfaces, and may be mobile, resulting in embolization. Because of the potential for embolic events, symptomatic patients, patients undergoing cardiac surgery for other lesions, and those with highly mobile and large CPFs should be considered for surgical excision.

    Description: Recent developments in Precise Orbit Determinations (POD) due to in particular to revisions to the terrestrial reference frame realization and the time variable gravity (TVG) continues to provide improvements to the accuracy and stability of the PO directly affecting mean sea level (MSL) estimates. Long-term credible MSL estimates require the development and continued maintenance of a stable reference frame, along with vigilant monitoring of the performance of the independent tracking systems used to calculate the orbits for altimeter spacecrafts. The stringent MSL accuracy requirements of a few tenths of an mm/yr are particularly essential for mass budget closure analysis over the relative short time period of Jason-l &2, GRACE, and Argo coincident measurements. In an effort to adhere to cross mission consistency, we have generated a full time series of experimental orbits (GSFC stdlllO) for TOPEX/Poseidon (TP), Jason-I, and OSTM based on an improved terrestrial reference frame (TRF) realization (ITRF2008), revised static (GGM03s), and time variable gravity field (Eigen6s). In this presentation we assess the impact of the revised precision orbits on inter-mission bias estimates, and resultant global and regional MSL trends. Tide gauge verification results are shown to assess the current stability of the Jason-2 sea surface height time series that suggests a possible discontinuity initiated in early 2010. Although the Jason-2 time series is relatively short (approximately 3 years), a thorough review of the entire suite of geophysical and environmental range corrections is warranted and is underway to maintain the fidelity of the record.

    Description: We report on X-ray follow-up observations of the March 2018 nuclear transient event AT2018zf (ASASSN-18el) from an ongoing campaign that includes XMM-Newton, NuSTAR, Swift, and high cadence (daily-to-weekly) NICER observations. The event was associated with the Seyfert galaxy 1ES 1927+654, a "True Type-2" that, nevertheless, showed an X-ray spectrum typical of an unobscured Type 1 AGN. Optical monitoring revealed the emergence of broad Balmer emission lines following the outburst, suggesting a transition from a Type 2 to a Type 1 AGN on timescales consistent with the light-travel time between the central black hole and the broad line region. The optical outburst was followed by a dramatic shift in the X-ray spectrum as the hard X-ray luminosity of the corona plunged by 〉2 orders of magnitude and the spectrum became dominated by a 10^6 K thermal component. The subsequent X-ray evolution of 1ES 1927+654 includes an additional two-order of magnitude decline in overall luminosity, followed by an extended period of quiescence, and re-brightening to levels that make it the brightest AGN currently in the X-ray sky. Throughout its evolution, the source has displayed X-ray flux variability of factors of several on timescales less than hour, and of ~100 on timescales less than a day. The X-ray spectrum varies along a consistent luminosity-dependent track, wherein increases in flux are accompanied by a spectral hardening. We seem to be witnessing the onset of an instability in the pre-existing AGN disc and corona, followed by a still-continuing re-emergence of the corona. If the initial transient is identified as the tidal disruption of a star, it is possible that this was the instigator of this unique behavior.

    Description: The stability behavior of a circular cross-section cylinder's wake at Reynolds number values of up to 45 is presently investigated by means of local linear stability theory. The steady-wake profiles computed are Navier-Stokes solutions of a uniform, incompressible viscous flow around a cylinder obtained by a spectral method. An absolutely unstable region is found to begin to form at a Reynolds number of about 20, and grows with incresing Reynolds number. The onset of global instability response must be characterized by a critical length of an absolutely unstable region; a critical Reynolds number criterion and preferred frequency are proposed based on linear stability analysis.

    Description: We present a new model for the global tropospheric chemistry of inorganic bromine (Bry) coupled to oxidant-aerosol chemistry in the GEOS-Chem chemical transport model (CTM). Sources of tropospheric Bry include debromination of sea-salt aerosol, photolysis and oxidation of short-lived bromocarbons, and transport from the stratosphere. Comparison to a GOME-2 satellite climatology of tropospheric BrO columns shows that the model can reproduce the observed increase of BrO with latitude, the northern mid-latitudes maximum in winter, and the Arctic maximum in spring. This successful simulation is contingent on the HOBr + HBr reaction taking place in aqueous aerosols and ice clouds. Bromine chemistry in the model decreases tropospheric ozone mixing ratios by 〈1-8 nmol/mol (6.5% globally), with the largest effects in the northern extratropics in spring. The global mean tropospheric OH concentration decreases by 4 %. Inclusion of bromine chemistry improves the ability of global models (GEOS-Chem and p-TOMCAT) to simulate observed 19th-century ozone and its seasonality. Bromine effects on tropospheric ozone are comparable in the present-day and pre-industrial atmospheres so that estimates of anthropogenic radiative forcing are minimally affected. Br atom concentrations are 40% higher in the pre-industrial atmosphere due to lower ozone, which would decrease by a factor of 2 the atmospheric lifetime of elemental mercury against oxidation by Br. This suggests that historical anthropogenic mercury emissions may have mostly deposited to northern mid-latitudes, enriching the corresponding surface reservoirs. The persistent rise in background surface ozone at northern mid-latitudes during the past decades could possibly contribute to the observations of elevated mercury in subsurface waters of the North Atlantic.

    Description: eXTP is a science mission designed to study the state of matter under extreme conditions of density, gravity and magnetism. Primary goals are the determination of the equation of state of matter at supra-nuclear density, the measurement of QED effects in highly magnetized star, and the study of accretion in the strong-field regime of gravity. Primary targets include isolated and binary neutron stars, strong magnetic field systems like magnetars, and stellar-mass and supermassive black holes. The mission carries a unique and unprecedented suite of state-of-the-art scientific instruments enabling for the first time ever the simultaneous spectral-timing-polarimetry studies of cosmic sources in the energy range from 0.5-30 keV (and beyond). Key elements of the payload are: the Spectroscopic Focusing Array (SFA) - a set of 11 X-ray optics for a total effective area of approx. 0.9 m(exp. 2) and 0.6 m(exp. 2) at 2 keV and 6 keV respectively, equipped with Silicon Drift Detectors offering less than 180 eV spectral resolution; the Large Area Detector (LAD) - a deployable set of 640 Silicon Drift Detectors, for a total effective area of approx. 3.4 m(exp. 2), between 6 and 10 keV, and spectral resolution better than 250 eV; the Polarimetry Focusing Array (PFA) - a set of 2 X-ray telescope, for a total effective area of 250 cm(exp. 2) at 2 keV, equipped with imaging gas pixel photoelectric polarimeters; the Wide Field Monitor (WFM) - a set of 3 coded mask wide field units, equipped with position-sensitive Silicon Drift Detectors, each covering a 90 degrees x 90 degrees field of view. The eXTP international consortium includes major institutions of the Chinese Academy of Sciences and Universities in China, as well as major institutions in several European countries and the United States. The predecessor of eXTP, the XTP mission concept, has been selected and funded as one of the so-called background missions in the Strategic Priority Space Science Program of the Chinese Academy of Sciences since 2011. The strong European participation has significantly enhanced the scientific capabilities of eXTP. The planned launch date of the mission is earlier than 2025.

    Description: At present, anthropogenic halogens and oceanic emissions of Very Short-Lived Substances (VSLS) are responsible for stratospheric ozone destruction. Emissions of the, mostly long-lived, anthropogenic halogens have been reduced, and as a consequence, their atmospheric abundance has started to decline since the beginning of the 21st century. Emissions of VSLS are, on the other hand, expected to increase in the future. VSLS are known to have large natural sources; however increasing evidence arises that their oceanic production and emission is enhanced by anthropogenic activities. Here, we introduce a new approach of assessing the overall impact of all oceanic halogen emissions on stratospheric ozone by calculating Ozone Depletion Potential (ODP)-weighted emissions of VSLS. Seasonally and spatially dependent, global distributions are derived exemplary for CHBr3 for the period 1999–2006. At present, ODP-weighted emissions of CHBr3 amount up to 50% of ODP-weighted anthropogenic emissions of CFC-11 and to 9% of all long-lived ozone depleting substances. The ODP-weighted emissions are large where strong oceanic emissions coincide with high-reaching convective activity and show pronounced peaks at the equator and the coasts with largest contributions from the Maritime Continent and West Pacific. Variations of tropical convective activity lead to seasonal shifts in the spatial distribution of the ODP with the updraught mass flux explaining 71% of the variance of the ODP distribution. Future climate projections based on RCP8.5 scenario suggest a 31% increase of the ODP-weighted CHBr3 emissions until 2100 compared to present values. This increase is related to larger convective activity and increasing emissions in a future climate; however, is reduced at the same time by less effective bromine-related ozone depletion. The comparison of the ODP-weighted emissions of short and long-lived halocarbons provides a new concept for assessing the overall impact of oceanic bromine emissions on stratospheric ozone depletion for current conditions and future projections.
