    Notes: Overdesign is applied to chemical processes to account for expected variations in the design data or design conditions. These variables must be treated quantitatively in order to design a process that is certain to perform adequately. Process dependability is introduced as a means of quantifying variable process behavior. Overdesign using the dependability criterion is a stochastic optimization problem. An example problem with three stochastic variables and two design variables is presented to illustrate these procedures.

    Notes: The recently enacted Clean Air Act Amendments of 1990 [1] and the proposed OSHA Process Safety Management Standard [2] both require that hazard evaluations be completed on all facilities that handle hazardous chemicals. Reports of the hazard evaluations must be kept on file and will be subject to inspection during an OSHA compliance audit. This paper outlines those concepts that are important in the preparation of the documentation of a Hazards and Operability (HAZOP) study.

    Notes: This paper focuses on the new Guidelines for Chemical Process Quantitative Risk Analysis recently published by the AIChE Center for Chemical Process Safety (CCPS). CCPS was formed soon after the disasters in Mexico City and Bhopal. CCPS was established in 1985 by the AIChE to develop and disseminate technical information for use in the prevention of major chemical accidents. The center is supported by over 60 industrial sponsors in the chemical process industry (CPI), who provide the necessary funding and professional guidance to its technical committees.The major product of CCPS activities has been a series of guideline books. Table I presents a list of the currently available CCPS guidelines. The first book, Guidelines for Hazard Evaluation Procedures (HEP), presented various structured qualitative techniques (HAZOP, FMEA, etc.) that could be used to identify possible hazards in a chemical facility. The Technical Steering Committee of CCPS decided that a sequel to HEP should be developed which would present quantitative methods for acute process hazards. The Guidelines for Chemical Process Quantitative Risk Analysis (CPQRA) resulted from that decision.

    Notes: Many hazardous industrial chemicals are stored as liquidfied compressed gases. To evaluate the possible consequences of a pipeline rupture, hose break, or tank puncture, the safety or process engineer needs a means to estimate the two-phase (liquid and gas) flow rate. Recent technical advances by the American Institute of Chemical Engineers Design Institute of Emergency Relief Systems (DIERS) have produced methods that can be used to compute the two-phase flow rate. These methods are simple and can be completed using a personal computer and a standard spreadsheet program such as Excel (Macintosh) or Lotus (DOS based). This paper presents the two-phase flow rate calculation method and shows how spreadsheets can be used for these calculations.
