    Description: Background: Measurement of HbA1c has been widely used for long-term monitoring and management of diabetes control. There is increasing use of point-of-care (POC) devices for measuring HbA1c where quicker results would allow immediate clinical management decisions to be made. Therefore, it is important to evaluate and compare the performance of such devices to the reference laboratory method.FindingsA total of 274 venous blood was collected from normal healthy adults during the community screening programmes. The performance of POC devices, Afinion and Quo-test were compared to central laboratory HPLC method; Adams A1c HA 8160. Both POC devices showed good correlation to HA 8160 with r = 0.94 (p 〈 0.001) and r = 0.95 (p 〈 0.001) for Afinion and Quo-test respectively. The means difference were statistically higher between POC and HA 8160 with 0.23% (95% CI 0.19-0.26, p 〈 0.001) and 0.29% (95% CI 0.24-0.34, p 〈 0.001) for Afinion and Quo-test respectively. Conclusion: Both POC devices could be considered in health clinics for diabetes management but not to be used for the diagnostic purposes.

    Description: IODP Exp. 383 recovered two Pleistocene sedimentary sequences from the upper continental slope along the southernmost Chilean margin that are well positioned to monitor changes in the Antarctic Circumpolar Current (ACC) upstream of the Drake Passage and the history of Patagonian glaciation. These sites are characterized by high sedimentation rates and a complex distribution of siliciclastic sediments with infrequent decimeter-scale beds of calcareous biogenic sediments. Unravelling ocean circulation and climate history from these sites requires a primary understanding of sedimentary provenance and transport mechanisms derived from a complete lithological characterization of the sequence. Here, we integrate downcore shipboard physical properties with sedimentological observations to fully characterize the sequences, evaluate potential for correlation and constrain regional depositional processes. Site U1542 (52°S; 1101 m water depth) consists of a 249 m spliced sedimentary sequence containing Middle Pleistocene to Holocene sediments. It mainly consists of clayey silt that is often interbedded with thin (~75 cm) beds of calcareous sand-bearing clayey to sandy silt with foraminifera and nannofossils or foraminifera-rich nannofossil ooze. Site U1544 (55°S; 2090 m water depth) consists of a 98 m sedimentary sequence obtained from a single hole. Sediments are also dominated by silty clay, but exhibit slightly thicker beds of calcareous ooze and a significantly higher proportion of cm- to dm-scale sand beds that are interpreted as turbidites. Based on the lithology of the recovered sediments and proximity to a glaciated continental margin, terrigenous sediment is likely delivered to these locations by a combination of ice rafting, glacial meltwater plumes, episodic downslope transport from the outer continental shelf and fine-grained sediments transported by the Cape Horn Current entering the Drake Passage as the northern branch of the ACC.

    Description: Mean, clear-sky surface temperature of the Greenland Ice Sheet was measured for each melt season from 2000 to 2005 using Moderate-Resolution Imaging Spectroradiometer (MODIS)–derived land-surface temperature (LST) data-product maps. During the period of most-active melt, the mean, clear-sky surface temperature of the ice sheet was highest in 2002 (−8.29 ± 5.29°C) and 2005 (−8.29 ± 5.43°C), compared to a 6-year mean of −9.04 ± 5.59°C, in agreement with recent work by other investigators showing unusually extensive melt in 2002 and 2005. Surface-temperature variability shows a correspondence with the dry-snow facies of the ice sheet; a reduction in area of the dry-snow facies would indicate a more-negative mass balance. Surface-temperature variability generally increased during the study period and is most pronounced in the 2005 melt season; this is consistent with surface instability caused by air-temperature fluctuations.

    Description: The North Atlantic deep water (NADW), according to the classic ocean circulation theory, moves southward as a deep western boundary current (DWBC) even though it may veer into interior and then rejoin DWBC when encountering regional circulation features, such as eddy-driven recirculation. In potential vorticity dynamics, the eastern side of the Mid-Atlantic Ridge (MAR) may provide a similar topographic support as the continental slope off the western boundary for a southward transport of NADW. In this article, we quantify the mean meridional NADW transports on both sides of the MAR using a data-assimilated product and find that the flow in the eastern basin contributes about 38 ± 14% of the net southward transport of NADW from 50° to 35°N. Our study points to the importance of observing NADW transport variations on the eastern side of the MAR in order to monitor the transport strength of Atlantic Meridional Overturning Circulation.

    Description: The western boundary current in the equatorial Atlantic Ocean is a main conduit for water-mass exchanges across the equator and thus a major pathway for the interhemispheric transports in the Atlantic Meridional Overturning Circulation (AMOC) system. In this study we quantify and examine the mean and seasonal variability of the equatorial western boundary current (EWBC) in the upper ocean layer using two data-assimilated products, the Estimating the Circulation and Climate of the Ocean (ECCO4r3) and the Simple Ocean Data Assimilation (SODA3). It is found that the EWBC between 10°S and 10°N exhibits two pronounced features in its seasonal variability: (1) the transport varies anti-symmetrically across the equator, that is, the northward EWBC strengthens to the north of the equator when it weakens to the south of the equator, and vice versa; and (2) the amplitude of seasonal variations is much greater in the northern hemisphere than in the south. We hypothesize that the cross-equatorial anti-symmetry in EWBC transport variability is attributable to the impingement of equatorial Rossby waves at the western boundary and the shape of the western boundary is the main cause for the amplified seasonal variability in the northern hemisphere. A simple 1 and 1/2-layer model is used to test and validate this hypothesis and to elucidate the role of wind forcing and topography plays in the seasonal variability in the EWBC transport.

    Description: The southernmost Mariana margin lacks a mature island arc and thus differs significantly from the central‐north Mariana and Izu‐Bonin margins. This paper presents a new P wave velocity model of the crust and uppermost mantle structure based on a 349‐km‐long profile of wide‐angle reflection/refraction data. The active source seismic experiment was conducted from the subducting Pacific plate to the overriding Philippine plate, passing through the Challenger Deep. The subducting plate has an average crustal thickness of ~6.0 km with Vp of 7.0 ± 0.2 km/s at the base of the crust and low values of only 5.5–6.9 km/s near the trench axis. The uppermost mantle of the subducting plate is characterized by low velocities of 7.0–7.3 km/s. The overriding plate has a maximum crustal thickness of ~18 km beneath the forearc with Vp of ~7.4 km/s at the crustal bottom and 7.5–7.8 km/s in the uppermost mantle. A zone of slight velocity reduction is imaged beneath the Southwest Mariana Rift that is undergoing active rifting. The observed significant velocity reduction in a near‐trench crustal zone of ~20–30 km in the subducting plate is interpreted as a result of faulting‐induced porosity changes and fracture‐filling fluids. The velocity reduction in the uppermost mantle of both subducting and overriding plates is interpreted as mantle serpentinization with fluid sources from dehydration of the subducting plate and/or fluid penetration along faults.

    Description: Background: The Malaysian Node of the Human Variome Project (MyHVP) is one of the eighteen official Human Variome Project (HVP) country-specific nodes. Since its inception in 9th October 2010, MyHVP has attracted the significant number of Malaysian clinicians and researchers to participate and contribute their data to this project. MyHVP also act as the center of coordination for genotypic and phenotypic variation studies of the Malaysian population. A specialized database was developed to store and manage the data based on genetic variations which also associated with health and disease of Malaysian ethnic groups. This ethnic-specific database is called the Malaysian Node of the Human Variome Project database (MyHVPDb).FindingsCurrently, MyHVPDb provides only information about the genetic variations and mutations found in the Malays. In the near future, it will expand for the other Malaysian ethnics as well. The data sets are specified based on diseases or genetic mutation types which have three main subcategories: Single Nucleotide Polymorphism (SNP), Copy Number Variation (CNV) followed by the mutations which code for the common diseases among Malaysians. MyHVPDb has been open to the local researchers, academicians and students through the registration at the portal of MyHVP (http://hvpmalaysia.kk.usm.my/mhgvc/index.php?id=register). Conclusions: This database would be useful for clinicians and researchers who are interested in doing a study on genomics population and genetic diseases in order to obtain up-to-date and accurate information regarding the population-specific variations and also useful for those in countries with similar ethnic background.

    Description: Background: Tetrapleura tetraptera, a seasoning and nutritive spice is also used in western African folk medicine in the management of wide variety of diseases including diabetes, inflammation and hypertension. Flavonoids and saponins are some abundant secondary metabolic constituents in the fruits of this plant. This study aimed at evaluating the potential therapeutic action of the polyphenol-rich hydroethanolic extract (HET) of this fruit in experimentally induced obese and type 2 diabetic rats (T2DM) with characteristic metabolic syndrome (MetS). Methods: MetS was induced in rats by high-carbohydrate, high-fat diet and administration of low-dose streptozotocin. Then different oral doses of HET (200 and 400 mg/kg) were administered to T2DM rats for 28 days. A standard antidiabetic drug, metformin (300 mg/kg), was used for comparison. The body weight, systolic blood pressure, oxidative stress and metabolic parameters were then assessed to evaluate the effect of HET on MetS. Results: HET reduced weight gain, fasting blood glucose and plasma insulin levels as well as homeostasis model assessment of insulin resistance (HOMA-IR) and alleviated obesity and T2DM associated oxidative stress and hypertension in rats. Moreover, a significantly hypolipidemic property and an attenuation of liver injury and tissue steatosis was observed after HET administration. HET further demonstrated its anti-inflammation effect via down regulation of tumor necrosis factor-alpha (TNF-α), interleukin-6 (IL-6), C-reactive protein (CRP), leptin and an increase in adiponectin. The HET exhibited dose-dependent effects which were comparable to that of metformin. Conclusions: The present study thereby demonstrates the anti-insulin resistance, antilipidemic, anti-obesity, hypotensive and anti-inflammatory properties of HET; hence it has the potential to be further developed for the management of MetS such as obesity, T2DM and hypertension.
