    Notes: Summary The pharmacokinetics of a new, potent H2-receptor antagonist, famotidine, 20 mg i.v. was studied in 7 subjects with normal renal function and in 24 patients with varying degrees of renal impairment. The volume of distribution at steady state was 1.14 l/kg in normal subjects and was not altered in renal failure. The half-life of elimination was 2.59 h in normal subjects and was unchanged in mild renal failure (creatinine clearance, CLCR 90–60 ml/min/1.48 m2) but was increased to 4.72 h in moderate renal failure (CLCR 60–30 ml/min/1.48 m2), and to 12.07 h in severe renal failure (CLCR below 30 ml/min/1.48 m2). The cumulative urinary excretion and renal clearance of famotidine were correspondingly reduced in patients with impaired kidney function. In normal subjects and in patients with mild to moderate renal failure, about 70% of famotidine was excreted through the kidney, mainly by tubular secretion. In patients with a CLCR above 60 ml/min/1.48 m2 the normal daily dose of famotidine can be employed, but in those with a CLCR between 60 and 30 ml/min/1.48 m2 the dose should be reduced by half, and in patients with a CLCR below 30 ml/min/1.48 m2 a reduction by three quarters of the normal dose is recommended.

    Notes: Summary The immediate haemodynamic effects of the calcium antagonist nilvadipine have been studied in ten patients with established mild essential hypertension. Nilvadipine 4 mg p.o. reduced both the systolic and diastolic blood pressures within 60 min, associated with a fall in total peripheral resistance and an increase in heart rate and cardiac index. The peak of blood pressure and total peripheral resistance reached during a cold pressor test were reduced by nilvadipine, but it did not affect the haemodynamic responsiveness to cold stimulation. Plasma renin activity was unaltered and the plasma noradrenaline concentration was increased only slightly. Thus, nilvadipine lowered blood pressure at rest and during cold stimulation as a result of arteriolar dilatation. The hypotensive effect at rest was associated with a reflex increase in heart rate and cardiac index.

    Notes: Summary We have studied pharmacokinetics of a new H2-receptor antagonist, TZU-0460, in patients with varying degrees of renal impairment. The apparent volume of distribution at steady-state was 1.70 l/kg, and the plasma protein binding of TZU-0460 or its active metabolite, desacetyl TZU-0460 was less than 10% in normal subjects. These variables were not altered with renal impairment. Sixty percent of TZU-0460 given orally was excreted via the kidney, mainly by tubular secretion. The half-time of elimination was 3.94 h in normal subjects, and was prolonged to 12.13 h in severe renal failure (creatinine clearance below 30 ml/min/1.48 m2). Dosage adjustment of TZU-0460 is necessary in renal failure.

    Notes: Abstract Styrene/2-hydroxyethyl methacrylate polymer microspheres consisting of various polymer compositions were produced by emulsifier-free seeded emulsion polymerization technique. Using these microspheres, which should have hydrophilic/hydrophobic heterogeneous surface, the effects of surface hydrophilicity on the main, fundamental requirements for an immunomicrosphere — high colloidal stability, sensitive immunologic agglutinability and insensitive non-specific agglutinability — were studied in detail. There was a region of the surface hydrophilicity that satisfied the three requirements simultaneously.

    Notes: Abstract Polymer microspheres composed of various compositions of styrene and 2-hydroxyethyl methacrylate (HEMA) were produced by batch emulsifier-free emulsion polymerization. The HEMA content at the surface, [HEMA] s , of the microspheres powdered by freeze-drying was determined by both quantitativeC 1s /O 1s analysis andC 1s peak shape analysis of the x-ray photoelectron spectroscopic spectra. When the HEMA content in the microsphere, [HEMA] p , was less than about 5 mole%, the [HEMA] s values determined by the two different methods showed good agreement. At [HEMA]p above 5 mole %, [HEMA]s values determined by the first method were about 15 mole % greater than those determined by the second. They both showed a similar tendency with the [HEMA] s being higher than the [HEMA] p , e.g., when [HEMA] p was 1 mole %, [HEMA] s was 11 mole %. The intensity of the satellite peak due to theπ→π * transition of the benzene ring of the styrene component decreased with an increase in [HEMA] p , to zero at 5 mole % of [HEMA] p . These results indicate that the HEMA component is localized at the surface.

    Notes: Summary A 10-month-old Japanese boy who had interstitial deletion of the long arm of chromosome No. 14; 46,XY, del(14)(pter»q24.3::q32.1»qter) is reported. A peculiar facial appearance, including round face, frontal hypertrichosis with thick eyebrows, horizontal narrow palpebral fissures, a short bulbous nose with a flat nasal root, and mild micrognathia, appeared to be common with the two previously reported cases. We stress the significance of this peculiar facial appearance in the diagnosis of 14q-(q24.3 to q32.1) syndrome. The level of α1-antitrypsin in the patient was only about half of that of his parents and controls, and the Pi locus was tentatively assigned to band 14q32.1.

    Notes: Abstract Micron-size monodisperse polymer microspheres having chloromethyl groups thereon were prepared by two-step polymerization process as follows. First, micron-size monodisperse polystyrene particles were prepared by dispersion polymerization with 2,2′-azobisisobutyronitrile as initiator in ethanol-water medium in the presence of poly(acrylic acid) as stabilizer under various conditions. Secondly, in the presence of the 1.9-Μm monodisperse polystyrene particles produced under the optimum conditions, seeded copolymerization for styrene and chloromethyl styrene was carried out. The seeded copolymerization proceeded smoothly without producing new particles, and it was confirmed by x-ray photoelectron spectroscopy that the chloromethyl group existed more at the surface of the produced microsphere than at that of film cast from the benzene solution in which the microspheres were dissolved.

    Notes: Abstract Measurements have been done in the millimeter wave region on a composite waveguide which comprises a dielectric rod waveguide connecting two metal rectangular waveguides. Such a waveguide has been used by us in a Josephson harmonic mixer installed in a small metal cryostat, to prevent the thermal invasion from outside environment and to transmit both signal and LO waves with small losses. The measured transmission loss, that is caused mainly by the coupling loss between metal rectangular waveguides (TE10 mode) and a dielectric rod waveguide (HE11 mode), has been less than 2dB in the frequency range of 52–104 GHz.
