    Description: Abstract 4348 Background: Acquired hemophilia A (AHA) is a rare but significant hemostatic disorder caused by inhibitory autoantibodies against coagulation factor VIII (FVIII:C). The annual incidence of AHA is low with about 1 to 4 cases per million individuals. However, the mortality rate due to severe hemorrhages and comorbidity is high reaching 22% in several series. In the past, only a few patients were reported in whom an association of AHA with respiratory disorders was observed. Patients, Methods, and Study Protocol: We have performed a monocenter study on 35 consecutive patients with AHA A who were referred for diagnosis and treatment to the Düsseldorf Hemophilia Comprehensive Care Center between March 2001 and June 2011. The cohort included 24 males (age: 44–86 years) and 11 females (age: 20–83 years). For laboratory evaluation, a standardized staged protocol of APTT, FVIII:C activity and concentration, mixing studies with patient and normal plasma, and quantitation of inhibitor titers (Nijmegen modification of the Bethesda assay) was used. Diagnostic work-up for any underlying disease was performed according to a standardized protocol of clinical examinations and imaging procedures (including X-ray examination of thorax, sonography of abdomen, retroperitoneum and thyreoidea and, whenever indicated, computerized tomography of thorax, abdomen, or pelvis). Therapy was performed according to a treatment algorithm consisting of (a) acute antihemorrhagic therapy (irrespective of residual FVIII:C activity and inhibitor titer), (b) immediate immunosuppression (individually tailored to the patients’ risks with regard to age and comorbidity), and, if life-threatening bleedings persisted, (c) inhibitor elimination by immunoadsorption or plasmapheresis, and (d) concomitant immunotolerance regimens. Predefined clinical endpoints were control of bleeding, eradication of the inhibitor, complete or partial remission (CR, PR), relapse, or early death (〈 30 days). CR was defined as no inhibitor detectable, FVIII:C activity 〉 80%, and withdrawal of immunosuppressive therapy. Results: In 21 (60%) of the 35 patients with AHA, an underlying disorder was identified, including 9 patients with respiratory diseases (26%), 8 patients with autoimmune disorders (23%), 3 with malignancies, and one with postpartum state, while in 14 patients (40%) AHA remained idiopathic. Upon admission, 16 of the 35 patients presented with life-threatening hemorrhages. In 13 of these 16 patients, control of bleeding was achieved by high doses of recombinant activated factor VII (rFVIIa; 90–120 μ g/kg every 2–3 h), while 3 patients required combined FVIII bypassing agents (rFVIIa plus bolus injections of activated prothrombin complex concentrates, aPCC; 100 IU/kg every 8–12 h). In the other 19 patients, bleeding also subsided in response to rFVIIa. Concurrent immunosuppression with prednisone alone (2 mg/kg/day) was performed in 11 patients, while 24 patients received cyclophosphamide (2 mg/kg/day) sequentially in combination with prednisone. In 5 patients in whom this first-line immunosuppression failed, 4 doses of rituximab (375 mg/m2) were administered as second-line therapy. Of the 35 patients, 13 required extracorporeal inhibitor elimination procedures due to persisting life-threatening bleeds. Exchange plasmapheresis was performed in 4, daily large-volume immunoadsorption (Ig-Therasorb) for up to 4 weeks in 9 patients. In 3 of them, immune tolerance was concomitantly induced by exogenous FVIII (100 IU/kg/day). Of the 35 patients in total, 28 individuals achieved CR (80%), 3 had PR, one relapsed, and 3 died within 30 days (one of acute myocardial infarction while on antihemorrhagic treatment, one of sepsis while on immunosuppression due to active AHA, one of lung bleeding in assocociation with pre-existing pulmonary sarcoidosis). Conclusions: This monocenter study demonstrates that control of life-threatening bleeding, eradication of the inhibitor, and induction of immune tolerance to FVIII have clearly improved the clinical outcome of AHA. Our data also suggest a shift in underlying disorders associated with AHA, whereby, in comparison to published studies, a relative increase in the proportion of patients with respiratory diseases is observed.     Notes: a:C–Nx films were deposited on Si(111) wafers at ambient temperatures by reactive magnetron sputtering of C in a mixed Ar/N2 discharge. The hardness (H) and elastic modulus (E) were assessed via nanoindentation using a Berkovich diamond indenter. The nitrogenated films exhibited increased hardness and elastic moduli values compared to the amorphous carbonated films, i.e., 25 and 240 GPa to 16 and 160 GPa, respectively. A sphere-on-flat (magnetic tape) wear tester was used to assess wear scar volume losses as a function of wear time. All films were in a state of compression (intrinsic and thermal stress). The bonding, composition, and structure of the films for all the deposition conditions were assessed by micro-Raman spectroscopy, energy dispersive x-ray spectroscopy (EDX), and x-ray diffraction (XRD), respectively, and subsequently correlated to overall film properties.© 1997 American Institute of Physics.

    Notes: This paper presents experimental results for adhesion assessment of 20 nm ultrathin SiC, amorphous carbon and carbon nitride films deposited on silicon (111) substrates by sputtering. In the experiment, the ultrathin overcoats were scratched by a face-forward Berkovich diamond indenter with a continuous depth sensing nanoscratch system. Experimental results indicate that the nanoscratch system has very good sensitivity for detecting ultrathin overcoat cracking, delamination, and brittle fracture caused by scratching. A well-defined critical load for each film was determined by the nanoscratch techniques. The highest critical load (800 μN) is found for the CN film and the lowest (500 μN) is for SiC. Scanning electron microscopy (SEM) investigations to the surface damage of the samples reveal that the SiC overcoat was damaged more severely than other films due to its brittle fracture. The adhesion properties of the three films tested in this work are best in carbon nitride, followed by amorphous carbon and then silicon carbide. © 1997 American Institute of Physics.

    Notes: Further insight into processing-structure-property relationships have been carried out for existing and candidate carbon-based protective overcoats used in the magnetic recording industry. Specifically, 5 nm thick amorphous diamond-like carbon (a:C) and nitrogenated diamond-like carbon (a:CNx) overcoats were deposited by low deposition rate sputtering onto a thin film disk consisting of either CoCrPt/CrV/NiP/AlMg or CoCrPt/CrV/glass. The wear durability and frictional behavior of these hard disks were ascertained using a recently developed depth sensing reciprocating nanoscratch test. It was determined that the CN0.14/CoCrPt/CrV/glass disk exhibited the most wear resistance, least amount of plastic deformation, and lowest kinetic friction coefficient after the last wear event. Core level x-ray photoelectron spectroscopy (XPS) results of sputter cleaned overcoats indicated that nitrogen up to 14 at. % incorporated into the amorphous network resulted in these improvements near the overcoat/magnetic layer interface, since there was an increase in the number of N-sp3 C bonded sites in a predominantly N-sp2 C bonded matrix. However, nonsputter cleaned overcoats exhibited a more graphitic pyridine-like (nondoping configuration) structure near the surface as evidenced by the increase in C(Double Bond)N versus C–N bonds and the valence band XPS determined appearance of the 2p-π band near the Fermi level (EF). Therefore, XPS sputter cleaning revealed a gradient in the chemical nature of the overcoats through the thickness. In addition, micro-Raman spectroscopy established that a further increase of nitrogen (≥18 at. %) weakened the overcoat structure due to the formation of terminated sites in the amorphous carbon network, since nitrogen failed to connect the sp2 domains within the network. This, in conjunction with an increase in the intensity of the 2p-π band from the valence band XPS spectra and the increase in the G-band position and ID/IG ratio from the Raman spectra, confirmed the increase in the size and number of sp2 bonds in the CN0.18 overcoat. © 1999 American Institute of Physics.

    Notes: Nanostructured contiguous networks of Fe well suited to fundamental studies of the effects of confinement are described. The Fe networks are prepared by sputter deposition onto the surface of nanochannel alumina. One of the most obvious consequences of confining the magnetic material to a nanostructured network is a dramatic enhancement in the coercivity when compared to a continuous Fe film of the same thickness. © 1996 American Institute of Physics.

    Notes: A novel technique for measuring temperature, based on pulsed photothermal radiometry and analysis of the spectral and temperature dependence of Planck's blackbody equation, is described. In this technique a body is irradiated by a laser pulse and its temperature inferred from the time dependence of the emitted infrared signal curve. This technique was used for near ambient temperature measurements using a CO2 laser, an infrared detector, and infrared transmitting silver halide optical fibers. The experimental results are consistent with theory. This technique is independent of changes in emissivity or geometric factors and it will be useful for accurate and fast noncontact temperature measurements. © 1997 American Institute of Physics.

    Notes: This paper provides an empirical assessment of the overall incentives generated by taxes with respect to the choice between extraction and recycling of basic materials in Canada. We calculate measures of the overall impact of the Canadian tax system on the incremental cost of (i) producing virgin material or recycled material that is to be used as an intermediate input in the production of a final product and (ii) producing finished products. The sectors that we examine include producers of primary virgin material (forestry, mining, oil and gas), producers of recycled material (scrap dealers) and producers of finished products (metal, paper, plastic and glass). Our results indicate that the Canadian tax system significantly favours the use of virgin materials rather than recycled materials in the case of metal and glass products, but the reverse is true for plastic products. Features in the Canadian tax system contributing to these findings are not limited to corporate income and mining tax incentives at the exploration and extraction stages of the production of virgin materials, but also include provincial sales taxes on capital, which are borne more heavily by scrap firms than by resource and manufacturing firms, and provincial sales taxes that apply to business inputs, which also fall more heavily upon the recycling sector.
