Abstract: This book contains the expert knowledge base of the field’s most experienced practitioners in the field of extradural bone and soft tissue malignancy. Chapters include modern classification, advanced anatomy, imaging, and the concepts around a multidisciplinary approach. Since treating primary tumors requires very different strategies than those used in metastatic tumors, the book devotes separate sections to each sub-discipline. For primary tumors, the text covers both benign and malignant entities and addresses unique anatomic zones such as the sacrum and skull base which require special technical expertise. For metastatic disease, the authors address the ever-important concept of prognosis, and discuss how to answer the eternal question: “How much should we do, and for whom?”. Chapters also explore the state of the art of treatment for the “big 5” histologies (renal cell, lung, breast, prostate, thyroid), with a special chapter emphasis on separation surgery and the now-standard combinatorial care between radiation and surgery. In addition, an entire section is dedicated to evolving surgical technology, which covers the use of minimally invasive techniques, navigation, robotics, 3D-printing, and other evolving technologies for spine tumor care. Infrequently-considered topics, such as how to evaluate a lesion which may be a tumor-mimic, and how to think about economic value in spine tumor surgery, are also presented. Surgical Spinal Oncology serves to help surgeons approach difficult clinical scenarios with a thoughtful, collaborative approach that leverages the best technology and thinking the field of spine oncology has to offer.

Description: Drawing upon an extensive range of archival and secondary sources from both sides of the Atlantic, this pioneering book provides the first full-length study of the controversial personal relationship between Prime Minister Harold Wilson and President Lyndon B. Johnson, often drawing on material that has only recently been opened to the public. Whilst Wilson was a firm supporter of the idea of a 'special relationship' between Britain and the United States and wanted to use his dealings with the White House to strengthen his credentials as a world statesman, Johnson held the British leader in low esteem and disdained the idea of a 'special' Anglo-American relationship. Difficulties stemming from the Vietnam War, British economic weakness and the UK's abrogation of its world power status exacerbated the strain between Wilson and Johnson, leading to what was probably the most fractious and troubled of all the relationships between British prime ministers and American presidents. The work also explores the more general nature of Anglo-American bonds in this period, arguing that the impact of the substantive issues, along with the limitations of the relationship between Wilson and Johnson, led to a serious weakening of the high-level ties between Britain and the United States. This book will be of vital interest to students of British and US foreign policy, of Anglo-American relations, and summit diplomacy, as well as to those with an interest in Harold Wilson and Lyndon Johnson themselves.

Description: Equilibrium Climate Sensitivity (ECS) – a measure of the sensitivity of global temperature change to a doubling of globally averaged atmospheric carbon dioxide concentration - is a critically important property of the climate system (IPCC, 2022). Unfortunately, the magnitude of ECS is not well constrained, its likely range currently sitting between 2.6 and 4.1°C. Narrowing this range is critical to reducing spread in projected climate change and has the potential to save trillions of dollars from better targeted adaptation and mitigation planning. Previous studies have suggested that this uncertainty might be reduced by exploiting relationships between model estimates of ECS and of observable quantities, including the magnitude of variability in globally averaged surface air temperature (GT). Here we estimate GT variability by the standard deviation in annual mean running GT trends and examine its links to ECS in CMIP5 and CMIP6 models. We show that model to model differences (M2MDs) in ECS are statistically significantly linked to M2MDs in the scaled magnitude of internally generated climate variability, with 0.4 correlation in both sets of models. Scaling is obtained by normalising the variability of GT by the variability in monthly Top-of-Atmosphere radiative imbalance. The resulting relationship, together with observations of scaled GT, is used to estimate real world ECS. In addition, we find strong evidence of a robust link between the statistical properties of GT and ECS. This strongly suggests that some of the physical processes responsible for internally generated climate variability are also partly responsible for setting ECS.

Description: Our understanding of the climatic teleconnections that drove ice-age cycles has been limited by a paucity of well-dated tropical records of glaciation that span several glacial–interglacial intervals. Glacial deposits offer discrete snapshots of glacier extent but cannot provide the continuous records required for detailed interhemispheric comparisons. By contrast, lakes located within glaciated catchments can provide continuous archives of upstream glacial activity, but few such records extend beyond the last glacial cycle. Here a piston core from Lake Junín in the uppermost Amazon basin provides the first, to our knowledge, continuous, independently dated archive of tropical glaciation spanning 700,000 years. We find that tropical glaciers tracked changes in global ice volume and followed a clear approximately 100,000-year periodicity. An enhancement in the extent of tropical Andean glaciers relative to global ice volume occurred between 200,000 and 400,000 years ago, during sustained intervals of regionally elevated hydrologic balance that modified the regular approximately 23,000-year pacing of monsoon-driven precipitation. Millennial-scale variations in the extent of tropical Andean glaciers during the last glacial cycle were driven by variations in regional monsoon strength that were linked to temperature perturbations in Greenland ice cores1; these interhemispheric connections may have existed during previous glacial cycles.
