    Description: In this fascinating book David Ingram traces the history of information technology and health informatics from its pioneers in the middle of the twentieth century to its latest developments. The book is distinctive in its broad scope and coverage and as the eyewitness account of an author who became the first UK professor appointed with the mission to bridge information technology with everyday medicine, health, and care. In this role, he has been a co-founder and leader of two rapidly growing initiatives, openEHR and OpenEyes, which stem from international collaborations of universities, health services and industries. These open source and open platform technologies have struck a widely resonant chord worldwide through their focus on community interest endeavours and open access to their methods and outputs. Set against the history of extremely costly, burdensome, and serially unsuccessful top-down attempts of governments to tackle the domain, the book argues for a greater focus on shared endeavours of this kind, contributing towards a standardized care information utility that incorporates methods and resources evolved, shared, and sustained in the public domain. As information technologies are now at the very core of health care, shaping the relationship between medical services and communities, professions, organisations and industries this book is important reading for politicians, health care academics, administrators and providers, and to anybody interested in the future of health services in the digital age.

    Notes: Adverse event monitoring is a problem-oriented approach to clinicalaudit and health-care quality improvement, which was developed and hasbeen widely used in the USA. Briefly explores the technique itself andits evolution. Presents experience gained from the widespread use of theapproach in a British acute hospital, and results from one specialty– ophthalmology. Suggests that the study of adverse events inpatient care can produce significant improvements in patients' care,that it is particularly suited to some specialties, and that it shouldbe used alongside other techniques in hospital clinical auditprogrammes. Concludes that, as the demand for quality-monitoringinformation from purchasers and within providers grows, adverse eventmonitoring may become one of the key techniques for quality assessmentand improvement.

    Notes: The purpose of this paper is twofold. First, we report on the successful application of variable angle spectroscopic ellipsometry to quantitative thin-film hermeticity evaluation. Secondly, it is shown that under a variety of film preparations and moisture introduction conditions water penetrates only a very thin diamondlike carbon (DLC) top surface-roughness region. Thus DLC is an excellent candidate for use as protective coatings in adverse chemical and aqueous environments.

    Notes: Diamondlike carbon (DLC) is a hard, semitransparent material usually containing varying amounts of hydrogen. These materials have numerous potential applications, including use as coatings for infrared optics, and as such, the effects of damaging irradiation is of practical interest. In this paper we present results of variable angle spectroscopic ellipsometric (VASE) studies of ion-beam-deposited DLC films. These films have been further modified by directing 1-MeV gold ions, as well as 6.4-MeV fluorine ions, through the DLC and into the underlying silicon substrates, and the percentage of hydrogen in the film was measured versus fluence using proton recoil analysis. Optical analysis was performed assuming the Lorentz oscillator model. Three versions were used: one oscillator, two oscillator (with one fixed in energy), and two oscillator with all parameter variable. The latter model fits the VASE data extremely well, and the two oscillators can be interpreted as involving π to π* and σ to σ* band transitions. With ion modification the oscillators shift to lower photon energy, consistent with reduction in hydrogen concentration and possible increased graphitization.

    Notes: The recent writings of Thomas McGuane show a particular interest in environmentalist concerns, examining the role played by inherited mythologies of the frontier in the ecology and politics of the contemporary American West. McGuane's explorations reveal complex and ambivalent responses to these subjects, in part liberal, radical and conservative.This essay will discuss these issues in relation to contemporary American attitudes to nature. The basis for my approach will be to assume that conceptions of “nature” are socially constructed, and that “nature” and “culture” are separate but mutually interdependent. The relationship between human societies and the natural world will therefore be considered as an ongoing process in which the term “nature” must not be wholly subsumed under that of “culture.” Environmental history is, as Donald Worster puts it, “a story of reciprocity and interaction rather than of culture replacing nature.”Thomas McGuane's writings tend both to repeat and to question what may be called “traditional” or “Romantic” attitudes to nature. In particular, he combines both aesthetic and utilitarian perspectives: nature as a scene of spiritual restoration through the appreciation of beauty, and as the object of technological mastery and control. In practice, these two approaches are historically interrelated, in that they are both responses to, and constructions of, nature produced within an urban capitalist society. As Walter J. Ong has argued, a necessary precondition for Romantic attitudes to nature is a society that has confidence in its capacity to dominate nature, through the use of the very industrial technologies such attitudes ostensibly reject.

    Notes: Using statistical design of experiments, the effect of ultrasonic pre-treatment on the nucleation density of diamond was studied. The parameters investigated included ultrasonic excitation power, concentration of diamond powder in water, duration of ultrasonic excitation, and duration of cleaning with water after ultrasonic excitation. Diamond films were deposited on silicon (100) substrates using microwave assisted plasma chemical vapor deposition. The nucleation density varied from 106 nuclei/cm2 to 109 nuclei/cm2. The results illustrated that the dominant effect in ultrasonic pre-treatment was seeding. Moreover, scratches caused by the seeds during the treatment enabled more seeds to be retained on the surface. Based on these results, an optimized ultrasonic pretreatment has been developed. The new procedure yields a uniform nucleation density of 109 nuclei/cm2 on silicon (100) substrates. © 1995 American Institute of Physics.

    Notes: Scandium nitride (001) oriented layers have been grown on magnesium oxide (001) substrates by molecular beam epitaxy using a rf-plasma source and a scandium effusion cell. The Sc/N flux ratio is found to be critical in determining the structural, optical, and electronic properties of the grown epitaxial layers. A distinct transition occurs at the point where the Sc/N flux ratio equals 1, which defines the line between N-rich and Sc-rich growth. Under N-rich conditions, the growth is epitaxial, and the surface morphology is characterized by a densely packed array of square-shaped plateaus and four-faced pyramids with the terraces between steps being atomically smooth. The films are stoichiometric and transparent with a direct optical transition at 2.15 eV. Under Sc-rich conditions, the growth is also epitaxial, but the morphology is dominated by spiral growth mounds. The morphology change is consistent with increased surface diffusion due to a Sc-rich surface. Excess Sc leads to understoichiometric layers with N vacancies which act as donors. The increased carrier density results in an optical reflection edge at 1 eV, absorption below the 2.15 eV band gap, and a drop in electrical resistivity. © 2001 American Institute of Physics.
