    Notes: Summary The pharmacokinetics of the new beta-adrenoceptor blocking drug bevantolol and some aspects of its beta-blocking effect have been studied in healthy volunteers. Bevantolol had a short serum half-life (86±33 min) and high systemic availability after oral administration. The observed changes in heart rate, systolic blood pressure during excercise and plasma renin activity were all compatible with beta-adrenoceptor blockade. After 200 mg p.o. in the morning, the effects lasted for less than 24 h.

    Notes: Abstract The application of dithionitrobenzoic acid (Ellman's reagent) for the quantitative determination of penicillins in aqueous solution and in urine is described. The penicillins have to be converted to penicillamine by treatment with alkali and acid successively. Penicillamine, which is formed quantitatively, can be determined with dithionitrobenzoic acid (Dtnb). From urine the penicillins must be extracted. The method determines urinary concentrations as low as 300 mg.l−1 and is simple and rapid to perform.

    Notes: Abstract Evidence exists that the free plasma propranolol concentration may correlate with the pharmacological effect in a better way than the total plasma concentration. Three approaches to obtain the free fraction were compared,viz. 1) equilibrium dialysis, 2) calculation from the ratio erythrocyte concentration: plasma concentration ([R]/[P]-ratio) and 3) derivation from the salivary concentration. It is concluded that the [R]/[P]-ratio has a linear relationship to the free propranolol concentration, described by the equation [R]/[P]=6.9 (1-f)−0.25 (n= 46, r=+0.956; p〈0.01). This method is reliable and simple to perform. No differences were seen between fresh blood samples, 14 days oldAcd-blood or artificial blood. The presence of various other drugs (4-hydroxypropranolol, chlorthalidone, clonidine, K-canrenoate, hydrallazine) did not interfere with the relationship between [R]/[P] and (1-f). A constant relationship between the salivary and free plasma propranolol concentrations does not exist, mainly because of buccal contamination. Equilibrium dialysis may pose an analytical problem. Therefore, the authors prefer the method based on the [R]/[P]versus (1-f) relationship.

    Notes: Abstract A view is presented on a number of recent developments, the present state and the perspectives, especially from a pharmacotherapeutic viewpoint, for peptide and protein drugs. The expanding use and the increasing experience create new pharmacotherapeutic modalities. Peptide and protein drugs comprise among others proteins isolated from human sources, and peptides and proteins made by biotechnology including monoclonal antibodies, recombinant human hormones, cytokines and growth factors. In the field of vaccine development also innovation is taking place. Optimal application schemes of these drugs may not have been reached and (clinical) pharmacists should contribute to the optimization. Since recombinant technology has abolished scarcity for a number of these drugs-especially ‘physiological’ substances-special ethical problems regarding an unlimited application or expansion of the indications may arise.

    Notes: Abstract A prospective, double-blind investigation into the effectiveness of the addition to infusion fluids of an injectable containing 0.3 mg prednisolone and 10 units heparin per ml in a phosphate buffer pH=7·5 versus the phosphate buffer alone is described. 196 Patients were allocated alternately to each group. On a total number of 593 cannulation days, 16 cases of thrombophlebitis were diagnosed. Statistical evaluation revealed a significant difference between the two groups (p〈0.01) in favour of prednisolone + heparin sodium.
