    Description / Table of Contents: Resumen Examenes histológicos rutinarios pulmonares condujeron en la segunda mitad del año 1970 al descubrimiento de una infección hospitalaria de aspergilosis bronco-pulmonar en niños entre 3 1/2 semanas hasta 2 años de edad, 1o que corresponde a un 10.5% de todos los niños autopsiados durante este período con una hospitalización mayor de 7 dias. En dos casos la aspergilosis no alteró todavía el tejido pulmonar en mayor grado; pero en dos casos los micelios aspergilosos invadieron la pared de arterias pulmonares adyacentes, originando arteritis, trombosis e infartaciones hemorrágicas consecutivas. Se cree que la micosis fué adquirida por aspiración de aire contaminada, pero se ignora las circunstancias que puedan robustecer este concepto.
    Notes: Abstract Routine histological investigations of the lungs of 38 children led to the discovery during the second half of 1970 of 4 cases of pulmonary aspergillosis in hospitalized children between 31/2 weeks and 2 years of age. Those cases represented 10.5% of all children, autopsiated during this period with an hospitalization period of more than 7 days. In two cases Aspergillus filaments did not invade pulmonary tissue, but in the two other cases Aspergillus mycelium had invaded the walls of adjacent pulmonary arteries with resultant arteritis, thrombosis and hemorrhagic infarction. The author believes, that the mycoses were acquired by inhalation of contaminated air. But he can not base this supposition on factual evidence.

    Description / Table of Contents: Zusammenfassung Die kritische Auswertung eines eigenen Beobachtungsgutes von 161 Biopsien und 36 Autopsien von Paracoccidioidomykose rechtfertigt eine Reihe Schlußfolgerungen, die zu der offiziellen Lehrmeinung über Entwicklung und Verlauf dieser Krankheit in Widerspruch stehen: a) Am häufigsten treten paracoccidioidomykotische Herde in Lungen und Nebennieren auf. b) Im eigenen Material spielen pulmonare Formen eine weit größere Rolle als die ganglionären oder viscero-abdominalen des Schrifttums. Leberschwellung, Milztumor und intestinale Herde, unmittelbar durch den Erreger verursacht, sind im eigenen Beobachtungsgut selten verglichen mit Lungen- und Nebennierenprozessen. c) Im Gegensatz zur gegenwärtigen Lehrmeinung glauben wir, daß die meisten Paracoccidioidomykosen mit einem fungosen Lungenbefall beginnen. d) Die Ausbreitung der Pilze im Lungengewebe erfolgt in erster Linie auf bronchogenem oder lymphogenem Weg; nur ausnahmsweise entwickeln sich Lungenherde durch haematogene Aussaat von mykotischen Prozessen anderer Organe aus. e) Die Schleimhautherde entstehen nach unserer Meinung erst sekundär im Verlauf einer zur Generalisation neigenden Erkrankung. f) In Fällen geringfügiger Ausweitung der Mykose fehlen spezifische Schleimhautherde in der Regel. g) Schwere Nebennierenzerstörungen durch Paracoccidioidomykose sind häufig. In Bezirken, in denen die Krankheit endemisch ist, stellt sie daher eine wichtige Ursache der chronischen Nebenniereninsuffizienz dar. Das Fehlen typischer Schleimhautveränderungen darf die Diagnose nicht ausschließen. h) Eine weitere wichtige, wenn auch nicht so häufige, Folgeerscheinung der Krankheit ist der mykotische Befall der Bauchaorta und der iliakalen Arterien. Die dadurch ausgelösten Thrombosen können Beingangrän oder ein Leriche syndrom hervorrufen.
    Abstract: Summary Critical evaluation of our observations from 161 biopsies and 36 necropsies of South American Blastomycosis justifies some conclusions which are in opposition to official doctrines about development and evolution of this disease: a) Paracoccidioidomycotic foci appear most frequently in lungs and adrenal glands. b) In our own material pulmonary forms play a much greater role than the ganglionary and viscero-intestinal forms of literature. Hepatosplenomegalies and intestinal manifestations, caused directly by the fungi, are seldom found in our material compared with the foci in lungs and adrenal glands. c) Contrary to the common doctrine, we believe therefore that the Paracoccidioidic granuloma begins with a mycotic attack on the lungs. d) The spreading of the disease in the lungs occurs firstly by bronchogenic or lymphatic ways; only exceptionally develops pulmonary involvement by haematogenous dissemination from mycotic foci of other organs. e) Our impression is, that mycotic lesions in mucosae develop secondarily during the generalisation of a paracoccidioidomycotic disease. f) In cases of little spreading, mycotic lesions in mucosae are mostly absent. g) Serious blastomycotic distruction of the adrenal glands are frequent. In districts where the South American Blastomycosis is endemic, Paracoccidiodic granuloma is an important cause of chronic adrenal gland insufficiency. The absence of specific lesions in the mucosae does not exclude the disease. h) Another important complication of South American Blastomycosis is the specific involvement of the abdominal aorta and the iliacal arterias. Following thrombosis can produce gangrenes of the legs or a Leriche-syndrome.
    Notes: Resumen En base de una revisión crítica de 161 biopsias y de 36 autopsias de paracoccidioidomicosis se llega a una serie de conclusiones que no coinciden con la opinión oficial actual de los textos micológicos conocidos sobre esta enfermedad: a) Las manifestaciones más frecuentes orgánicas de la paracoccidioidomicosis son las pulmonares y las suprarrenales. b) Las formas pulmonares juegan en nuestro material un papel mucho mayor que las formas ganglionares y víscero-abdominales. Hepatomegalias, esplenomegalias y lesiones intestinales causadas por accíon directa del germen son raras en nuestro material en comparación con las manifestaciones pulmonares y suprarrenales. c) Al contrario a la doctrina actual creemos que la gran mayor parte de los casos de paracoccidioidomicosis empieza con una infección fúngica pulmonar. d) La propagación de los hongos en el tejido pulmonar se realiza por las vias broncógena y linfática; raras veces manifestaciones micósicas pulmonares se deben a diseminaciones secundarias hematógenas a partir de focos fúngicos localizados en otros órganos. e) Se considera las manifestaciones micósicas en las mucosas generalmente como lesiones secundarias. f) En casos de escasa generalización faltan muy frecuentemente manifestaciones específicas en las mucosas. g) La paracoccidioidomicosis produce frecuentemente graves lesiones destructivas suprarrenales. En zonas endémicas de la enfermedad, esta micosis es una causa importante de la insuficiencia suprarrenal crónica. La ausencia de manifestaciones en mucosas no descarta en tales casos la etiologia fúngica. h) Entre las complicaciones importantes de la enfermedad figura la colonización fúngica de la aorta abdominal y de las arterias ilíacas. Las trombosis consecutivas pueden dar origen a trastornos circulatorios graves hasta gangrenas en los miembros inferiores y al síndrome de Leriche.
