Résumé Nous rapportons le cas d'un enfant atteint de retard mental et psychomoteur important. L'étude caryologique faite à partir de fibroblastes et de lymphocytes du probant montre un allongement des bras longs d'un chromosome B, sans mosaïcisme. Par le G- et Q-banding, associés à la microdensitométrie, il a été possible d'identifier le B anormal comme étant un chromosome 4 présentant une insertion de matériel chromosomique après le premier tiers proximal des bras longs, vraisemblablement au niveau de la bande q23. L'origine de ce matériel (duplication partielle ou translocation intercalaire) n'a pu être déterminée car le segment inséré est trop court et ses caractéristiques de colorabilité peu définies. De plus, le reste du caryotype est tout à fait normal. Par ailleurs, l'étude généalogique effectuée chez les deux parents et le frère cadet nous a donné des résultats normaux.

Zusammenfassung Wir berichten über den Fall eines Kindes mit einigen angeborenen Mißbildungen. Die Karyotypen, erstellt aus Fibroblast- und Lymphocytenkulturen, zeigen eine Verlängerung des langen Arms eines B-Chromosoms in allen untersuchten Zellen. Durch die G- und Q-Bandentechnik, kombiniert mit Mikrodensitometrie, war es möglich, das abnorme Chromosom als ein B4 zu identifizieren. Für die Verlängerung wird eine Insertion von Chromosomenmaterial innerhalb der langen Arme verantwortlich gemacht. Diese Insertion ist nach dem ersten Drittel der langen Arme lokalisiert, offensichtlich bei der q23-Bande. Der genaue Ursprung des eingefügten Segments konnte wegen seiner Kürze nicht bestimmt werden. Die Möglichkeit einer partiellen Duplikation oder einer interkalarischen Translokation wird mit Hilfe der densitometrischen Aufzeichnungen erörtert. Andere Anomalien des Karyotyps sind nicht festzustellen. Genealogische Studien bei den beiden Eltern und dem jüngeren Bruder haben normale Ergebnisse gezeitigt.

Summary We report the case of an infant with some birth defects. The karyotypes established from fibroblasts and lymphocytes cultures show a lengthening of the long arms of a chromosome B in all the studied cells. By G and Q banding, associated with microdensitometry, it has been possible to identify the abnormal chromosome as a B4; the lengthening is due to an insertion of chromosomal material within the long arms. This insertion is located after the first third of the long arms, seemingly at the q23 band. The exact origin of the inserted segment could not be determined because of its shortness; the possibility of a partial duplication or of an intercalary translocation is discussed from the microdensitometric tracings. None other abnormality has been detected in the karyotype. Genealogic studies made on the two parents and the young brother gave us normal results.
