Abstract The advantage of plasmapheresis over conservative therapies is well known in the treatment of severe Guillain-Barré syndrome (GBS). Although there are several modes of plasmapheresis applicable to GBS, i.e., standard plasma exchange (PEX), double filtration plasmapheresis (DFPP), and immunoadsorption plasmapheresis (IAPP), no definite guidelines for selecting an optimum mode have been established so far. In the present study, we reviewed 27 cases of GBS treated with PEX, DFPP, or IAPP. Although the latter two methods appeared reasonably effective, PEX with fresh frozen plasma (FFP) exerted a brisk effect in three out of four cases of severe GBS refractory of DFPP. In GBS with axonal degeneration, however, the efficacy of plasmapheresis including PEX was questionable.
