    Description: A study of safety reports was conducted to examine the hypothesis that fatigue and associated performance decrements occur in air transport operations, and that these are associated with some combination of factors: circadian desynchronosis, duty time; pre-duty activity; sleep; work scheduling; workload; and environmental deprivation. The findings are based on a selected sample of reported incidents in which the reporter associated fatigue with the occurrence. In comparing the fatigue reports with a control set, significant performance decrements were found to exist related to time-of-day, awareness and attention to duty, less significantly, final phases of flights. The majority of the fatigue incidents involved such unsafe events as altitude deviations, takeoffs and landing without clearance, and the like. Considerations of duty and sleep are the major factors in the reported fatigue conditions.

    Description: Air traffic control (ATC) operations were examined to learn what factors of controller performance should be given consideration in the design and development of future automation systems enhancing ATC. Contingencies were of two types: those constraining airspace usage or traffic flow (i.e., weather); and those related to system and equipment usage (i.e., radar/radio status). Examination of controller response to contingencies and workload pressures showed differing effects on controller allocations of effort among the three primary function of planning, monitoring, and informaton transfer. Automation advancements oriented towards aiding the controller in performing monitoring tasks may offer the most substantial safety benefit.

    Description: It is pointed out that advances in avionics and computer technology have reached the point where serious attention may be given to structuring a new controlled air traffic environment. The principal feature of such an environment is related to the capability of pilots to perform certain active ATC functions from the cockpit. The main function is concerned with the ability to maintain safe separation from other aircraft under all meteorological conditions. Technical features available to permit such a functional change include such concepts as the Discrete Address Beacon System, a full capability Beacon Collision Avoidance System, data links, computer generated displays, and flight management computers.

    Description: The origins and development of the NASA Aviation Safety Reporting System (ASRS) are briefly reviewed. The results of the first quarter's activity are summarized and discussed. Examples are given of bulletins describing potential air safety hazards, and the disposition of these bulletins. During the first quarter of operation, the ASRS received 1464 reports; 1407 provided data relevant to air safety. All reports are being processed for entry into the ASRS data base. During the reporting period, 130 alert bulletins describing possible problems in the aviation system were generated and disseminated. Responses were received from FAA and others regarding 108 of the alert bulletins. Action was being taken with respect to 70 of the 108 responses received. Further studies are planned of a number of areas, including human factors problems related to automation of the ground and airborne portions of the national aviation system.
