    Abstract: This book is a practical guide on image-guided robotic (CyberKnife®) radiosurgery of the brain and the spine. The volume introduces the radiosurgical community to the potential of image-guidance in the treatment of neurosurgical diseases including neuro-oncological, vascular and functional disorders. Principles of image-guided radiosurgery, including physics and radiobiology are considered. Each chapter provides a critical review of the literature and analyses of several aspects to offer an assessment of single and hypofractionated treatments. Based on the authors’ experience, tables or summaries presenting the treatment approaches and associated risks are included as well. Providing a practical guide to define the selection of dose, fractionation schemes, isodose line, margins, imaging, constraints to the structures at risk will support safe practice of neuroradiosurgery. This book aims to shed new light on the treatment of neoplastic and non-neoplastic diseases of the central nervous system using the CyberKnife® image-guided robotic radiosurgery system. It will be adopted by neurosurgery residents and neurosurgery consultants as well as residents in radiation oncology and radiation oncologists; medical physicists involved in radiosurgery procedures may also benefit from this book. .

    Notes: Application of Executive Order 12898 to risk assessment of highway or rail transport of hazardous materials has proven difficult; in general, the location and conditions affecting the propagation of a plume of hazardous material released in a potential accident are unknown. Therefore, analyses have only been possible in a geographically broad or approximate manner. The advent of geographic information systems and development of software enhancements at Sandia National Laboratories have made kilometer-by-kilometer analysis of populations tallied by U.S. Census blocks along entire routes practicable. Tabulations of total or racially/ethnically distinct populations close to a route, its alternatives, or the broader surrounding area, can then be compared and differences evaluated statistically. This article presents methods of comparing populations and their racial/ethnic compositions using simple tabulations, histograms, and chi-square tests for statistical significance of differences found. Two examples of these methods are presented: comparison of two routes and comparison of a route with its surroundings.

    Notes: Calculation of accident dose-risk estimates with the RADTRAN code requires input data describing the population likely to be affected by the plumeof radioactive material (RAM) released in a hypothetical transportation accident. In the existing model, population densities within 1/2 mile (0.8 km)of the route centerline are tabulated in three ranges (Rural, Suburban, andUrban). These population densities may be of questionable validity since the plume in the RADTRAN analysis is assumed to extend out to 120 km from thehypothetical accident site. We present a GIs-based population model which accounts for the actual distribution of population under a potential plume, and compare accident-risk estimates based on the resulting population densities with those based on the existing model. Results for individual points along a route differ greatly, but the cumulative accident risks for a sample route of a few hundred kilometers are found to be comparable, if not identical. We conclude, therefore, that for estimation of aggregate accident risks over typical routes of several hundred kilometers, the existing, simpler RADTRAN model is sufficiently detailed and accurate.

    Notes: Truck transport of radioactive material (RAM), e.g., spent nuclear fuel (SNF), normally maximizes use of Interstate highways, which are safer and more efficient for truck transport in general. In the estimation of transportation risks, population bordering a route is a direct factor in determining consequences and an indirect factor in determining exposure times, accident probabilities and severities, and other parameters. Proposals to transport RAM may draw intense resistance from “stakeholders” based on concern for population concentrations along urban segments but the length of a route segment is also a determinative factor in estimating the transport risks. To quantify the relative importance of these two factors, a potential route for transport of SNF (strict use of Interstate highways) was selected and compared with a modified version that bypassed urban areas. The results suggest that emphasis on Interstate highways minimizes total route and urban segment risks.
